[Cerebro-cardial disorders in hemorrhagic stroke].

AIM: To elucidate relationships brain-heart in hemorrhagic stroke depending on the size and location of hematoma and initial state of the heart. MATERIAL AND METHODS: Clinical and paraclinical methods were used in acute and rehabilitation periods in 160 patients with hemorrhagic stroke. RESULTS: Symptoms of cerebrocardial syndrome in hemorrhagic stroke include arrhythmias (tachycardia, bradycardia, extrasystole, cardiac fibrillation, sick sinus syndrome), blocks (transient bundle-branch block), myocardial dystrophy, in IHD--ischemic myocardial disorders up to subendocardial myocardial infarction. Its severity in massive hemispherical hematomas was due to compression of the brain trunk. CONCLUSION: Pathogenesis of cerebrocardial syndrome rests on impaired central autonomic regulation occurring in stroke.

[Nervous system changes in adrenal failure].

AIM: To study nervous systems in chronic adrenal failure (CAF). MATERIAL AND METHODS: 262 patients with CAF were studied clinically, biochemically and electrophysiologically before and after treatment. RESULTS: The patients were found to have syndromes of vegetovascular dystonia by hypotonic type, syncopal paroxysms, myastenic, minor strokes, dyscirculatory-dysmetabolic encephalopathy, polyneuropathy. A direct correlation was established between the severity and duration of adrenal failure (AF), lowering of adrenal cortex hormones, coefficient Na/K and clinical manifestations of the neurological syndromes. Adequate hormone replacing and symptomatic therapy promoted a regress of neurological disorders in most of the patients especially in mild and moderate AF. CONCLUSION: Adequate therapy corrects neurological disturbances in patients with chronic NN.

[The diagnosis, treatment and prevention of early stages of cerebral blood flow insufficiency].

Biomicroscopy of bulbar conjuctiva as well as oencephalography were quite informative for discovery of subclinical manifestations of disorders of cerebral circulation (DCC). That conclusion resulted from the observation of 133 patients with early forms of vascular pathology of brain. Besides, to find the early signs, it was also worth while to perform some biochemical studies (coagulogram, studies of both rheologic properties and lipids of blood). In order to establish discirculatory encephalopathy it was also expedient to use ultrasonic dopplerography and electroencephalography. Efficiency of some medical-prophylactic measures (normalization of the diet, weight, muscular activity, administration of antiatherosclerotic drugs) that prevented the progression of aorta's stenosis was also demonstrated.

[Spinal cord injury in multiple myeloma].

Myelomic spinal cord damage may be a result of both its compression and vascular disorders (squeezing of an artery, disorders of venous outflow). Diagnosis is based on evaluation of the following signs: normochromic anemia; increase of ESR, total protein and calcium levels; discovery of M-gradient in protein fractions of gamma-zone; multiple regions of destruction in spondylogramme. In doubtful cases magneto-resonance and computer tomography were the most informative. Highly effective was therapy with intensive chemoprogramme including either a range of preparations (alkeran, BGNU, cyclophosphan, adriblastina, prednisolon) or a combination of polychemotherapy with irradiation. The operation was indicated in both insufficiency of conservative therapy and increasing of the symptoms of the damage.

[The osteogenic mechanism of vertebrogenic radiculopathies].

Intraosteal block was conducted in 86 patients with pronounced neurological symptoms of lumbar osteochondrosis. To clarify the role of the intraosteal receptors in pathogenesis of the above symptoms and to evaluate the efficacy of the blocks, series of experiments were made on 8 cats. The authors studied evoked potentials in the cerebral cortex and thalamus in response to irritation of the sciatic nerve under the exposure of the sciatic receptors to electrostimulation, intraosteal introduction of physiologic salt solution and trimecaine, electrostimulation following the introduction of the above substances. The pilot experience has been gained on facilitation of afferent current passage from the sciatic nerve under conditions of intraosteal receptor irritation as well as on inhibition of the above effect under anesthetic block of intraosteal receptors. Basing on the above findings, the authors explain osteogenic mechanism of radiculopathy and pathogenicity of the intraosteal blocks.

[Intraosseous blocks--the pathogenetic treatment of the pronounced neurological manifestations of lumbar osteochondrosis].

Basing on the original and literature data, the author analyzes causes of rare application of intraosseous block (IB) in managing neurological manifestations of lumbar osteochondrosis (NMLO). IB is believed efficient and pathogenetically validated. A conception is suggested on intraosseous receptors and vessels participation in spinal osteochondrosis pathogenesis and neurological symptoms of such osteochondrosis. NMLO in 24 patients was treated with IB which permitted 20 of them to avoid scheduled surgery after inefficient conservative treatment. Details of IB usage are presented (indications, contraindications, regimens, pharmacology).

[The characteristics of the modulation of the afferent reactions during stimulation of the intraosseous receptors].

Acute tests in mature cats, demonstrated, evoked potentials in the cortex and medial center of the thalamus resulting from test irritation of sciatic nerves and conditioning irritations of intra-osseous receptors. The test proved that electro-irritation of intra-osseous receptors as well as higher intra-osseous pressure caused by the impregnation of physiological solution into the bone facilitated afferent reactions during threshold electrostimulation of sciatic nerves. The results of these experiments were compared to those of clinical monitoring of patients with radicular syndromes of osteochondrosis after intra-osseous blockades which led the authors to suggest a hypothesis about trigger mechanisms of neuralgias, based on segmental alleviation of nociceptive afferentation resulting from overstimulation of intra-osseous receptors.

[Intramuscular and intraosseous block in the combined treatment of the neurological manifestations of lumbar osteochondrosis].

The author has shown high efficacy of intraosseous blockades and determined indications for the use of paravertebral, intraosseous, and distant blockades. He has also developed a medicated mixture with autologous blood; it was first used in the treatment of neurological manifestations of lumbar osteochondrosis by means of intraosseous blockades in the posterior iliac bone.
