[Conservative therapy of scars and wrinkles].

We report on the treatment of keloids and scars by means of topical application and injection of active substances. The combination of cryotherapy along with local injection of glucocorticoids has proved the most successful treatment. In the case of depressed scars, the injection of collagen or silicon may bring on favorable results.

[Diffuse (presenile) hyperplasia of the sebaceous glands, a new entity? Successful treatment with 13-cis-retinoic acid].

We report the case of a young man with extreme sebaceous gland hyperplasia that occurred in a diffuse pattern of aggregated papular lesions involving the entire face, neck and upper chest, together with marked seborrhoea oleosa. Oral therapy with 13-cis-retinoic acid (isotretinoin) resulted in remarkable improvement within a few weeks. Parallels from our case are drawn to familial sebaceous hyperplasia, reported by Dypre et al. in 1980 [6], and to a case of a young man with severe sebaceous gland hyperplasia and facial seborrhoea, reported by de Villez et al. in 1982. We suggest that these types of seboglandular proliferative disorders be classified as diffuse (presenile) sebaceous gland hyperplasia in contrast to the well-defined senile circumscribed variant, and that they be regarded as a separate entity.

[Dermatosurgical treatment possibilities in leg ulcers].

On he basis of our own investigational findings in patients suffering from leg ulcers during the past 3 years, we report on general aspects regarding preoperative treatment of ulcus cruris. In addition, we give an illustrated synopsis of current dermatosurgical treatment, especially referring to leg ulcers in venous disorders. Aside from local surgical procedures, we discuss the various techniques of skin grafting as well as surgical possibilities with regard to the treatment of incompetent perforator veins.

Humans

[Dermabrasion and its problems].

The technique and indication as well as resulting problems of dermabrasion are discussed in detail. Furthermore rare indications suited for dermabrasion are pointed out. It is demonstrated, that dermabrasion represents a universal surgical procedure in dermatology.

[The value of diagnostic measures in erythema nodosum].

This article summarized the findings of check-ups of all our in-patients suffering from erythema nodosum during the past 15 years. A critical review showed that a third of the cases could not be etiologically defined. Based on our results and according to literature, we discuss the valence of diagnostic procedures in erythema nodosum patients without any history or physical evidence of underlying diseases.

Chronic Disease

[Mycosis fungoides with central nervous system involvement].

Mycosis fungoides at present is classified as a malignant cutaneous lymphoma of T-cell-type. Involvement of every organ is generally possible. Referring to autopsies involvement of the central nervous system is found by chance in about 10%. Since the first description of Mycosis fungoides there are only 20 reports published dealing with a clinical picture due to involvement of the central nervous system. Based on a survey of literature the scale of individual reports concerning involvement of the nervous system is analysed and a report is given on a patient with such a malady confirmed by autopsy.

Adult

[Leg ulcers in essential thrombocythemia].

By means of two case reports, essential thrombocythemia is discussed as an uncommon cause of leg ulcers, and the other possible cutaneous manifestations of this disease are briefly outlined.

Aged

[Hair transplantation: gelatin plugs for hemostasis].

Uncontrolled bleeding at the recipient site is a complicating factor in hair transplant surgery. As a consequence, time consuming procedures as well as further discomfort for the patient is inevitable. In order to achieve adequate hemostasis, we present an easy method using cylindrical gelatin-plugs.

Gelatin

[Keratosis lichenoides chronica].

Case report of a patient with keratosis lichenoides chronica and good response to a combined therapy with retinoids and photochemotherapy.

Chronic Disease

[Study on the efficacy of a combined treatment of psoriasis with dithranol and UV-B (selective ultraviolet-phototherapy)].

Within a clinical study, 12 patients suffering from generalized psoriasis were treated with 0,4% anthralin ointment, unilaterally applicated in combination with UV-B-therapy. Comparing the results, both modes of treatment revealed no considerable difference with regard to erythema, infiltration of the lesions, scaling, itchiness and irritation. The anthralin induced erythema did not show any augmentation by additional use of UV-B-radiation. The clinical improvement of both applications was considered satisfying in 67% of the cases. UV-B-radiation showed no additional therapeutical value if used in combination with anthralin.

Adolescent

[5-Fluorouracil therapy following curettage of warts].

Of our patients recorded so far 31% showed local recurrences after topical 5-fluorouracil treatment following curettage of warts. Based on our provisional results we do not consider this technique acceptable for further use.

Fluorouracil

[Facial pomade crust].

In a 33-year-old female patient a pomade crust occurred after using ointments on the face for three years. This phenomenon has only been described in the inguinal or gluteal folds of babies. We therefore suggest the term "facial pomade crust".
