It is known that some children produce parietal spikes, evoked by tactile stimulation to their feet or other parts of their bodies. This paper investigates whether this curious phenomenon remains with the children unchanged for several years and then disappears or whether it has an evolution towards epilepsy. It has been found that 36% of the subjects, who showed a normal EEG in the first examination, developed a spontaneous focus of spikes during their sleep or waking period; during a period of up to 3 years 16% of the children present a clinical state represented by seizures. In conclusion, it is felt that the cause of this condition may be due to hyperexcitability of a small cortical parietal area.
One hundred and thirty EEGs were analysed from 113 patients with acute secondary traumatic midbrain and bulbar brain syndromes. The EEG pattern was related to the stage of the midbrain syndrome caused by supratentorial brain shift. A decrease in the number of different EEG patterns was associated with increasing intracranial pressure. Unfavourable prognosis was indicated by the disappearance of sleep or sleep-like activities alternating pattern and loss of reactivity. Lateralization by the EEG proved to be superior to clinical findings in full stages of the midbrain syndrome. EEG abnormalities due to the herniation itself interfered with EEG changes due to secondary circulatory, respiratory and metabolic encephalopathies. In such cases, the above mentioned regularities were blurred.

RG 033 is a compound consisting of betametasone in resinic microcapsules which allow a slow and continuous dialysis of the active drug through the membrane in the digestive tract. The drug was studied with regard to its effects on the distribution of alveolar air and its gastric tolerance. In a preliminary study the drug release from the capsules in the pH-conditions of the digestive tract was studied by means of a physico-chemical model. Two groups of 15 patients with broncho-obstruction reversed by salbutamol, a potent beta 2-sympathomimetic, were included in the trial. One group received 1 mg and the other 2 mg of the drug. An alternate scheme was used for the treatments. The functional methods used to assess the ventilatory results were chosen according to the conclusions of a previous comprehensive study by one of the authors [7] [8]. The ventilatory results of both dosages were quite similar. This observation suggests that a maximum effect on alveolar air distribution was obtained with 1 mg. No unfavourable side-effect on the gastric mucosa was observed.
During treatment with clozapine 59% of 127 patients showed abnormal EEGs. Remarkable are paroxysmal disturbances with sharp waves and/or SW-pattern in ca. 13%. Generalized tonic-clonic convulsive cereberal seizures were observed only twice.

Seventy seven adult epileptics and 30 patients with clinically non epileptic seizures were subject to a 24 hours sleep deprivation. The waking records of these patients were normal or unspecific abnormal. After sleep deprivation 29 (37%) of epileptics had abnormal EEG with epileptiform or focal activity, more often in cases with frequent (26 (60%) patients out of 43) epileptic seizures. Patients with sporadic attacks only exceptionally showed specific EEG changes (3 patients (8,8%) out of 34). Drug induced sleep emerged to be diagnostically less helpful. Twenty nine out of 30 patients with clinical nonepileptic seizures had no additional abnormalities in the EEG after sleep deprivation.
Delusion of skin-parasitosis is a very characteristic disorder, and it can appear as an independent syndrome or as a concomitant phenomenon of other psychiatric illnesses. Like most delusional formations it can also be induced and appear as a symbiontic psychosis; the involutional age seems to be an essential precondition for this psychopathological phenomenon.

The EEG-pattern was related to the stage of midbrain of bulbar syndrome caused by supratentorial brain shift. There was a close relation between EEG-pattern and the grade of rostro-caudal impairment. A decrease in the number of different EEG-patterns was associated with increasing intra-cranial pressure. An unfavourable state of coma was indicated by the disappearance of sleep or sleep-like activities, alternating pattern and loss of reactivity. In deep stages of coma the neurological examination reveals no lateralizing signs at all. The EEG gives the only hint to a local cerebral lesion. EEG-abnormalities due to the herniation itself interferred with the EEG-changes due to secondary circulatory, respiratory and metabolic encephalopathies. In such cases the above mentioned regularities were blurred.
This study compares the characteristics of 492 patients discharged against medical advice from the Istituto Dermopatico dell'Immacolata, a Research Hospital (335 beds) in Rome specialized in Dermatology, Vascular Surgery and Plastic Surgery between 1995 and 1998, with those of 43,110 control patients discharged with physicians' approval according to a case-control model. In the multivariate analysis, male gender (OR 1.65; 95% CI 1.37-1.98) and residence in Rome area (OR 1.22; 95% CI 1.02-1.47) increased the odds of discharge against medical advice. The odds of such discharge decreased with increasing age (OR 0.994 per year; 95% CI 0.990-0.999). The proportion of patients discharged against medical advice decreased from 1.4% in 1995 to 0.4% in 1998. The average length of stay decreased from 1995 to 1998, with the exception of the Plastic Surgery unit. A retrospective review of the charts of 45 patients discharged against medical advice (AMA) within 48 hours from admission ascertained some of the stated reasons for discharge. Personal and family problems or refusal of treatment were reported for 35% of the patients. Of the 25 patients who gave no reasons, 11 asked again for hospitalization and 5 of these were rehospitalized by IDI within 10 days. This study also identified that the information on the medical record of patients who left against medical advice was generally poor. A standardized form for AMA discharges, including patient's understanding of the diagnosis, treatment, alternative therapies, consequences of refusing treatment and stated reasons for leaving against medical advice, might be of benefit to patients, physicians and hospital managers.
