[Effect of right-sided hippocampotomy on memory and learning ability].

The authors report the results of memory and learning ability investigations in a patient treated with hippocampotomy (uncotomy) for severe epileptic seizures. In the seeg investigations the changes were most pronounced in the hippocampus. In psychological investigations before the operation deep disturbances of verbal memory with unchanged (normal) level of mental efficiency were found. After stereotaxic treatment epileptic seizures disappeared completely, the patient completed a vocational school and started working. Follow up examinations during 6 years after the operation failed to demonstrate any decrease in mental efficiency. The impairment of verbal memory and learning ability was decreased significantly.

[Early signs of cognition disorders in children with subacute sclerosing panencephalitis in initial stage].

The purpose of the study was to find some characteristic features of cortical defects in the neuropsychological examination of children with SSPE. These findings could be very useful in early differential diagnosis. We analysed the results of the neuropsychological assessment of 13 children selected from a group of 100 patients. The most frequent defects were found in the visuo-spatial functions. The clinical analysis of the obtained results was studied in view to establish, whether there are the signs of the dysfunction of the right or both cerebral hemispheres.

[Cerebral functional asymmetry in face perception by the patients with focal brain damage].

The results are reported of the study of face perception in a group of 18 healthy subjects and 10 patients with lesions in the right or left cerebral hemisphere. The task given to these subjects included identification of a face photograph exposed for 20 msec in the right or left field of vision. The index of perception coefficient was the number of errors made while addressing the material to the left or right cerebral hemisphere. The results of the experiment confirmed the hypothesis of the dominant role of the right hemisphere for face perception. The results suggest also that face perception disturbances occur without respect to the site of lesion in the right hemisphere.

[The process of learning verbal material in patients with various damaged cerebral structures].

The results are described of a study of the process of verbal material learning and its recall after distraction in 20 patients with involuntary movements treated by thalamotomy, and in 20 epileptic patients treated by amygdalotomy and hippocampotomy. In both groups learning difficulties had been noted already before the operation. In postoperative tests these difficulties were found to have increased immediately after the surgical intervention in patients with lesions in the left cerebral hemisphere. On the other hand, late after the operation they were particularly pronounced after lesions placed in the right thalamus. Disturbances of material recall after distraction were shown in both groups independently of the side of the lesion.

[Verbal, visual and aural memory in the early postoperative period in patients treated by thalamotomy].

Verbal memory and learning ability were determined before the operation and immediately 3-4 weeks after stereotactic surgery in 11 patients with extrapyramidal motor disturbances treated by ventrolateral (VL and Vim) thalamotomy. Preoperative examinations demonstrated disturbances of short-term verbal memory with narrowing of its extent and increased susceptibility of the trace to interference. Long-term memory was disturbed also, with presence of difficulties in recall and difficulties in memorization of verbal materials. Directly after the operation, in relation to the preoperative testing, increased disturbances were demonstrated in all stages of verbal memory, after damage to the border area of VL and Vim nuclei in the left as well as in the right thalamus. It was possible to detect some functional differentiation between thalamic lesions, with a tendency for increased disturbances of short-term verbal visual memory after right-sided thalamotomy and verbal auditory memory after left-sided thalamotomy.

[Effect of amygdalotomy on behavior disorders in epileptics].

Results of psychological testing in 46 epileptics with severe behaviour disturbances treated surgically, by the stereotaxic method (selective exclusion of the dorso medial parts of amygdalae) are reported. After the operation there was a significant improvement of behaviour and seizures of temporal-lobe type were no longer observed. Comparative tests demonstrated a significant reduction of emotional tension, development of ability to behaviour and emotional reactions control, reduction of emotional instability and motor hyperactivity. Aggressiveness in difficult or conflict situations was markedly diminished or eliminated.

[Processes of verbal memory in patients with involuntary movements treated by thalamotomy].

The authors tested verbal memory and learning in 15 patients with motor disturbances treated by ventrolateral thalamotomy before operation and late after it, after a mean time of 5.8 years. Disturbances of verbal memory in the form of retrieval of information from the memory stores and disturbances of learning of verbal material were found already before the operation. In comparative investigations late after the operation greater disturbances of verbal memory were observed with disturbances of verbal retrieval from memory stores after damage to the VL nucleus in the left as well as right thalamus. On the other hand, no decrease of the mental efficiency of these patients was demonstrated.

Follow-Up Studies

[Effect od amygdalectomy on memory and learning in patients treated surgically for epilepsy].

The results of neuropsychological investigations of 40 patients with severe epilepsy and behaviour disturbances treated by stereotactic methods are presented. In all cases memory disturbances were found before the operation, in 45% of the cases mental sluggishness was present. After amygdalotomy, together with a reduction of emotional tension and psychomotor agitation, the range of direct memory extended and difficulties in learning verbal material decreased. In some patients a tendency was observed also for a rise in intelligence quotient.
