[Standardization of invasive neuromodulatory procedures].

The Working Group on Neuromodulation of the German Association for the Study of Pain, composed of representatives from various scientific specialty societies, met on December 9, 2000, March 24, 2001, October 5, 2001, and December 8, 2001. As a result of these discussions grounded in current knowledge, the following guidelines were formulated for the standardization of invasive techniques of neuromodulation intended to serve as a systematic aid in decision-making and to provide recommendations for practice-oriented methods. The guidelines were based on both the clinical and practical experience of the group participants (see information box on the next page) as well as on the current scientific literature and guidance from the consensus report of the European Federation of IASP Chapters (EFIC) [23]. The guidelines serve the purpose of orientation and have no effect on either assumption of liability or discharge from liability. The guidelines were conceived for use by physicians in private practice,doctors in hospitals,and nonmedical personnel concerned with the care of chronic pain patients. The Working Group consists of unsalaried volunteers. The participants received no honorarium and were only reimbursed for normal travel expenses in accordance with customary directives. The guidelines will be revised should new scientific results become available, at the latest in 2 years. The plan exists to further develop the guidelines to stages II and III (AWMF). The Steering Committee of the DGSS appraised the guidelines and authorized the guidelines before publication.

Parasitism, host immune function, and sexual selection.

Parasite-mediated sexual selection may arise as a consequence of 1) females avoiding mates with directly transmitted parasites, 2) females choosing less-parasitized males that provide parental care of superior quality, or 3) females choosing males with few parasites in order to obtain genes for parasite resistance in their offspring. Studies of specific host-parasite systems and comparative analyses have revealed both supportive and conflicting evidence for these hypotheses. A meta-analysis of the available evidence revealed a negative relationship between parasite load and the expression of male secondary sexual characters. Experimental studies yielded more strongly negative relationships than observations did, and the relationships were more strongly negative for ectoparasites than for endoparasites. There was no significant difference in the magnitude of the negative effect for species with and without male parental care, or between behavioral and morphological secondary sexual characters. There was a significant difference between studies based on host immune function and those based on parasite loads, with stronger effects for measures of immune function, suggesting that the many negative results from previous analyses of parasite-mediated sexual selection may be explained because relatively benign parasites were studied. The multivariate analyses demonstrating strong effect sizes of immune function in relation to the expression of secondary sexual characters, and for species with male parental care as compared to those without, suggest that parasite resistance may be a general determinant of parasite-mediated sexual selection.

[Renal iodine excretion by students in East Germany. A prospective study 1989 to 1996].

AIM: A prospective longitudinal study was performed from 1989 to 1996 in order to check the effectiveness of prophylactic measures in Germany. TEST PERSONS AND METHODS: In 3 periods the urinary iodine excretion was examined in 1849 adolescents from 2 regions (north, south) of former Eastern Germany. RESULTS: The consumption of iodized salt has increased substantially since 1994. This coincides with an increase in urinary iodine excretion. The median iodine excretion found in 1989/92 (n = 744) was 39.0 micrograms/g creatinine (4.7 micrograms/dl), in 1994 (n = 123) 60.1 micrograms/g creatinine (6.0 micrograms/dl), and in 1995/96 (n = 982) 101.1 micrograms/g creatinine (9.8 micrograms/dl). However, the concentration was < 5.0 micrograms in 27.8% and < 10.0 micrograms iodine/dl in 58.0%. Regional differences were not found. CONCLUSION: The iodine supply has been improved. However, the declared targets have not yet been met. The findings show that effective goitre prophylaxis can still be achieved while maintaining the voluntary principle.

Damalinia (Tricholipeurus) zaganseeri, n. sp. (Phthiraptera: Trichodectidae), a parasite from Procapra gutturosa (Artiodactyla: Bovidae) in Mongolia.

During an expedition through the east Mongolian steppes, an undescribed chewing louse (Insecta: Phthiraptera) of the genus Damalinia was collected on Mongolian gazelles, Procapra gutturosa. This represents the 1st report of a chewing louse on this poorly known antelope. After a short review of the distribution of this genus and its subgenera among host groups, the new species D. (Tricholipeurus) zaganseeri, n. sp. is described and illustrated. Its distinct morphology is compared with that of similar species.

The Mongolian gazelle (Procapra gutturosa, Bovidae) as an intermediate host of three Sarcocystis species in Mongolia.

The sarcocysts of three Sarcocystis species (Sporozoa: Coccidia: Sarcocystidae) found in the Mongolian gazelle are described using light and transmission electron microscopy. The host-parasite interface and the ultrastructure of the cyst wall are represented for the first time in the macrocysts of Sarcocystis mongolica Machul'skii, 1947, localised in connective tissue cells. The cyst wall is attributed to type 1 of the classification by Dubey et al. (1989). Two other species are described for the first time from Procapra gutturosa in Mongolia: S. sp. with hair-like villar protrusions (type 7) and S. danzani n. sp. with villar protrusions possessing a structure hitherto unknown, characterised by a hollow.

Establishment and characterization of a novel CD34-positive human myeloid leukemia cell line: MHH225 growing in serum-free culture.

A new human multilineage myeloid leukemia cell line, MHH225, has been established in our laboratory from the bone marrow of a 60-year-old patient suffering from acute megakaryoblastic leukemia (M7); it provides a unique model for studying the effect of biologic and chemical agents on the lineage specificity of a multipotent myeloid leukemia clone containing a mixed population of megakaryoblast, erythroblast, and myeloblast cells in a serum-free culture. Morphologically, all 225 cells are large blast cells with basophilic cytoplasm containing no granules, large round nucleus containing 2-3 prominent nucleoli, and fine chromatin structure and a large nuclear/cytoplasm ratio. The MHH225 cells are CD34+HLA-DR+CD33+CD13+ with 57.6%, 28.3%, and 7.8% of them being CD41+, glycophorin A+, and CD15+, respectively, and all lymphoid-specific antigens are negative. The karyotype analysis of MHH225 cells revealed a deletion of the short arm of chromosome 7: del(7)(p13)-, a whole-arm translocation between the long arms of chromosomes 9 and 21: t(9;21)(q10;q10), and a chromosome 11 with an elongated long arm due to duplication of chromosome 11 material as well as to translocation of part of chromosome 9 onto 11q+. Also, chromosome 21 was deleted in some metaphases or showed a ring formation in other metaphases. Utrastructurally, MHH25 cells display a strong platelet peroxidase activity in the nuclear envelope and the endoplasmic reticulum. The MHH25 cells have been grown exponentially without growth factors or conditioned media or serum only in RPMI1640 culture medium. None of the myelopoietic growth factors, i.e., interleukin-3, GM-CSF, G-CSF, erythropoietin, or interleukin-6, has any effect on the proliferation and differentiation of MHH25 cells. The two, hematopoietic inhibitory cytokines, interferon-alpha and tumor necrosis factor-alpha, have only minimal growth inhibitory effect. Stem cell factor showed only weak growth-stimulatory effect on MHH225 cells but significantly inhibited chemotherapy-induced apoptosis in these cells. The new cell line MHH225 should constitute a useful model for studying stem cell antigen (CD34)-positive human multilineage myeloid leukemia cells carrying a deletion in the short arm of chromosome 7 and an aberration in chromosome 11 and provide a unique tool for investigating human hematopoietic stem cell biology and its cytokine regulation in serum-free cultures. To our knowledge, the MHH225 cell line is the first human CD34-positive leukemia cell line growing in serum-free cultures to be established.

A new fibroblast growth stimulating activity from the human megakaryoblastic leukaemia cell line ELF-153: in vitro and in vivo findings.

Although the exact mechanism for the progression of myelofibrosis in acute megakaryoblastic leukaemia is unclear, certain humoral factors released from the proliferating megakaryoblasts that are unable to store these factors in their defective alpha-granules, including platelet derived growth factor (PDGF), fibroblast growth factors (FGF), platelet factor-4 (PF-4), transforming growth factor-beta (TGF-beta) and beta-thromboglobulin, could result in increased collagen synthesis by bone marrow fibroblasts. Recently, the human megakaryoblastic leukaemia cell line MEG-01 has been shown to produce both TGF-beta and PF-4 which have enhanced the growth of bone marrow fibroblasts. Therefore, we have examined the presence of a fibroblast growth stimulating activity and the humoral factors that might be responsible for it in the supernatant of the human megakaryoblastic leukaemia cell line ELF-153 recently established in our laboratory from a patient with acute myelofibrosis. A new fibroblast growth stimulating activity has been identified in the supernatant of the ELF-153 human megakaryoblastic leukaemia cell line that is independent of the percentage of fetal calf serum in NRK-49F fibroblast agar clonogenic assays and is not due to any of the known fibroblast growth stimulating humoral factors including PDGF, epithelial growth factor, TGF-alpha or beta, tumour necrosis factor-alpha, interleukin-1, 2, 4 or 6, FGF, fibronectin, PF-4 and factor VIII AG. Also, in vivo, subcutaneous injection of ELF-153 megakaryoblastic leukaemia cells into nude mice formed, in three out of the five mice after 6 weeks, subcutaneous tumours with a very rigid texture whose histological examination revealed dense infiltration by blast cells and pronounced reticular fibrosis. Immunohistochemistry demonstrated exclusive deposition of collagen III in the extracellular matrix whereas laminin and collagen IV were absent.(ABSTRACT TRUNCATED AT 250 WORDS)

Incidence of memory of transoperative events during total intravenous general anesthesia with propofol.

The incidence of memories of events during the total intravenous general anesthesia is studied in a 400 adult patient group in order to establish the influence of the included doses of drugs in this scheme, upon the mentioned incidence. The anesthetic method included propofol, fentanyl and vecuronium. Two groups of patients were constituted according to the propofol infusion rate (PIR) required for the anesthetic maintenance: group I required a PIR < 6.0 mg.kg-1.h-1, and group II required a PIR > 6.0 mg.kg-1.h-1. Transanesthetic monitoring was done with cardiac frequency, systolic and diastolic arterial pressures, ECG, pulse oximeter oxygen saturation and end tidal CO2. The memory incidence of transoperative events was obtained through postoperative interviews about intraoperative events. Patients in group I had a significantly greater remembrance incidence both in real events and in dreams than the patients in group II. We conclude that a PIR of propofol < 6.0 mg.kg-1.h-1 can be associated with a high incidence of remembrances of transoperative events.

A constant flux system using the 'magnetic cell separator' (MACS) for highly efficient T-cell depletion.

A method for highly efficient T-cell depletion using a 'Magnetic Cell Separator' (MACS) is described. T-lymphocytes were depleted from peripheral blood samples of 37 patients with chronic lymphocytic leukemia (CLL). The flow speed in the MACS column was adjusted by a syringe-driver allowing a constant flow speed of about 100 microliters/min. The mean content of T-cells after depletion was 0.17% (S.D. +/- 0.27). The effectiveness of depletion was calculated to a mean 93.7%. There was no correlation between the proportion of CD3-positive cells before and after depletion.

Population dynamics and interrelationships of some Dactylogyrus and Gyrodactylus species on Cyprinus carpio.

During 1986/87, 922 carps have been examined for ectoparasitic helminths. Altogether 6 species of Gyrodactylus and 4 species of Dactylogyrus were found. The most frequent parasite on the gills was D. extensus; it shows a great tolerance to the water temperature with two peaks of infestation between 2 and 9 degrees C. Additionally, relationships between the seasonal dynamics of the invasion cycle of D. extensus and D. anchoratus were recognized. In addition to Dactylogyrus 2 species of Gyrodactylus, G. shumani and G. sprostonae occurred on the gills. G. shulmani tends to show rapid population growth in the warmest months in summer. The highest number of helminths were found in the beginning of September. It is conceivable that G. shulmani will be an agent of a new helminthosis in the GDR. The infestation by G. shulmani is often accompanied by the occurrence of D. vastator. Both species reach the maximum values approximately at the same time. G. sprostonae reached high intensities already in June and August. The common infestation increased by G. katharineri, G. stankovici and G. kherulensis on skin nd fins. G. katharineri seems to have the most favourable conditions in April. The indices dropped to a minimum in summer and autumn. G. stankovici has likely similar ecological claims as G. katharineri and shows a peak of incidence in April, too.

Upper gastrointestinal bleeding as an unusual presentation of a duodenal carcinoid.

Carcinoid tumors may develop in any part of the gastrointestinal tract. They constitute 1.5% of all gastrointestinal tumors (1). The duodenum is one of the least common sites of this tumor, with a reported incidence varying between 0.2% and 8.9% (2, 3). We report on a case of a bleeding duodenal carcinoid in a patient who underwent bilateral nephrectomy for malignant hypertension.

[HDL cholesterol in patients with peripheral arteriosclerotic occlusive disease].

The HDL-cholesterol level in the blood serum was determined in 40 male control persons and 73 male patients with peripheral arteriosclerotic obstructive disease of the lower extremities. A reduction of the HDL concentration as a whole could not be proved. The influence of the various drugs applied to the therapy of the arteriosclerotic obstructive disease on the HDL-cholesterol level in the blood serum is different. With increasing severity of the arterial obstructive disease the HDL level significantly decreases. Patients with hyperlipoproteinaemias show lower HDL concentrations than test persons with inconspicuous serum lipid concentrations.

[Beta 2 glycoprotein I analysis in patients with hyperlipoproteinemia, arteriosclerotic occlusive disease and diabetes mellitus].

Beta 2-glycoprotein-I (apolipoprotein H) is an activator of the lipoprotein lipase. The concentration of beta 2-glycoprotein-I in the blood serum was determined with the help of the radial immunodiffusion. In patients with hyperlipoproteinaemia of type IIa and IIb, arteriosclerotic obstructive disease or diabetes mellitus the beta 2-glycoprotein-I-concentrations were increased. In hyperlipoproteinaemia of type IV can be concluded to a relative beta 2-glycoprotein-I-deficiency.

[Psychological and gynecological characteristics of rejection of the maternal role and its significance for sterility].

In connection with 204 cases the authors endeavoured to reveal the psychogenous causes of functional sterility by examining the personality structure of their patients. The patients visited the clinic because of their primary or secondary sterility. In 44 cases the husband or partner were also examined from psychological point of view, or were treated with psychotherapy. The psychogenous causes of functional sterility were found to be multifactorial and divided into 18 groups. Among these factors, which caused sterility directly or indirectly, there were some, which could be eliminated by psychotherapy, and there were some, which had already caused an irreversible change in the personality,--Beside the success of therapy the effectiveness of the gynecologist-psychologist team-work is remarkable.

[Personality and the social significance of hirsutism].

Cyproterone acetate treatment was applied with good success by the authors to 52 patients with hirsutism. Yet, the patients' confidence in the medicament was negatively affected by recurrence of symptoms during intervals of discontinued medication. However, effective help can be given to such women to get to grips with their own psychic conflicts by disclosing to them and discussing with them those environmental factors due to which insecurity was built up in them regarding their sexual role. Such psychotherapeutic approach will back up pharmacotherapy and mitigate regression during intervals. Virile women do not accept their own female role unless they are given in parallel both gynaecological and psychotherapeutic treatment.

[Embolization of renal artery -- an alternative in nephrectomy in the treatment of patients with dialysis and malignant hypertension (author's transl)].

Embolization of renal arteries (coil embolization by Gianturco) was performed in four patients with malignant renal hypertension as an alternative to bilateral nephrectomy. Three patients with successful renal transplantation developed intractable hypertension without arterial stenosis of the transplant. One patient treated with hemodialysis showed malignant hypertension refractory to volume depletion and antihypertensive therapy. In all cases hypertension could be improved and thus medical therapy be reduced.
