[A case of the successful use of regional thrombolytic therapy in acute ischemic stroke].

The paper reports a case of a positive response to fibrinolytic therapy with complete lysis of the thrombus in the left internal carotid artery (ICA) and significant reduction of the baseline neurological deficiency. The vessel later on underwent rethrombosis, but the patient's condition remained stable. Acute ICA occlusion, recovery of the major circulation and subsequent rethrombosis were confirmed angiographically. The intraarterial thrombolytic therapy started 12 hours after the apoplexy. The relief of the major artery occlusion in an acute period of cerebral ischemia prevented the development of infarction and promoted adequate collateral circulation due to which subsequent rethrombosis appeared asymptomatic. It is suggested that intraarterial fibrinolytic agents may be effective as well at later postapoplectic periods (after 6 hours and later).

[Cerebral, central and intracardiac hemodynamics in patients with progressive arterial hypertension following the surgical suppression of adrenal hyperfunction].

Forty-eight patients with progressive (malignant) arterial hypertension resistant to conservative therapy were subjected to surgical portalization of the adrenal blood stream in order to metabolize excessively produced aldosterone in the liver of the patients. Cerebral hemodynamics was studied before by tetra- and bipolar rheoencephalography, central hemodynamics was studied by tetrapolar transthoracic rheography, and intracardiac hemodynamics by echocardiography before and after surgery; blood plasma aldosterone and hydrocortisone concentrations and plasma renin activity were measured. A significant reduction of arterial pressure, elimination or alleviation of subjective and objective manifestations of chronic hypertensive encephalopathy were seen in the majority of patients after surgery. Cerebral blood flow improved, blood plasma aldosterone and renin activity reduced, myocardial hypertrophy decreased, and a trend to normalization of intracardiac hemodynamics was observed.

Adrenocortical Hyperfunction

[Status of cerebral and central hemodynamics in patients with ischemic stroke].

Overall 30 patients who suffered ischemic brain stroke were examined for cerebral and central hemodynamics by radiocerebrography (RC) with the aid of the radiopharmaceutical preparation Tx-99 and by radiocardiography (RCG) using 131I. These methods were employed simultaneously with computer-aided tomography, cerebral angiography, rheoencephalography. The control group was made up of 18 patients suffering from peripheral nervous diseases. In 11 patients with a favourable outcome of ischemic brain stroke and the recovery of work fitness, the RC and RCG readings did not practically differ from those in the controls. The remaining 19 patients with disability due to brain stroke demonstrated deceleration of the cerebral blood flow, most pronounced on the side of brain infarction. Asymmetry of the cerebral blood flow and deceleration of blood supply in the main and intracerebral vessels as shown by the RC readings turned out the signs of the hemodynamically significant stenosis of one of the internal carotid arteries, even in lack of the changes on the angiogram. A correlation was established between deterioration of central hemodynamics and deceleration of the cerebral blood flow, as was a possibility of an increase of the blood inflow to the brain after administering drugs that make the RCG readings return to normal.

[10-year experience with using Cavinton in cerebrovascular disorders].

Cavinton was used for 10 years in 967 patients with different cerebrovascular diseases. The highest effect was seen in patients with early forms and primarily chronic forms: vegetovascular (neurocirculatory) dystonia, initial manifestations of brain blood supply insufficiency, circulatory encephalopathy in the first and second stages. Improvement of the subjective status and a decrease of the intensity of vestibulocerebellar disorders were recorded by the end of the treatment in 75-85% of such patients. In ischemic brain stroke, regress of general cerebral and focal symptoms was more rapid and significant in the adequate reaction type of cerebral hemodynamics to cavinton administration (a rise of pulse blood content of the brain and a reduction of the vascular tone according to the REG data) and was less noticeable in the hypertonic and, in particular, in the hypotonic type. Cavinton should not be used in severe general cerebral hypertensive crises, as well as in elderly or senile patients with acute cardio-cerebral or cerebro-cardiac syndrome, postinfarction cardiosclerosis, marked disorders of heart rhythm.

[Problems of classification, clinical course and pathogenetic treatment of circulatory encephalopathy].

Based on the experience gained with psychoneurological examinations of 436 patients suffering from circulatory encephalopathy, the author reviews problems of its efficient classification and distinguishes the type of its clinical course (slow-progressing, rapid-progressing, galloping and remitting) and the clinical variety of the neuropsychic defect (mnemonic, affective, paranoiac). Describes methods of differentiated treatment of circulatory encephalopathy, carried out with regard to the patients' age, individual response to the drugs and selective influence of the latter ones on the structure of the neuropsychic defect.

[Diagnosis and prognosis of lesions of the nervous system in newborn infants in cases of maternal threatened abortion].

A study was made of the status of the central nervous system in 91 neonates born to mothers with a history of threatened abortion. Using consecutive Wald's analysis the role was estimated of the clinical characteristics of pregnancy and delivery, and of the content of hormones (estriol, estradiol, progesterone, testosterone) as well as of cyclic adenosine-3,5-monophosphate, detected in the future mother over pregnancy, in the genesis of neurological disorders in the child.

[Risk factors of acute and chronic cerebral ischemia].

A total of 112 patients with ischemic stroke and 115 patients of different sexes and age with dyscirculatory encephalopathy were examined for the main risk factors of cardiovascular diseases. Hypokinesia and obesity were most common in chronic vascular brain pathology whereas frequently occurring and prolonged psychoemotional overstrain, aggravated heredity and alcohol abuse promoted the occurrence of acute disorders of brain circulation. The combination of 4 and more risk factors significantly raises the probability of acute cerebral ischemia.

[The role of the reactivity of cerebral vessels in the diagnosis and treatment of circulatory encephalopathies].

The central and cerebral hemodynamics was studied in 47 patients with atherosclerotic discirculatory encephalopathy, grade 1-2. Functional challenges were used to characterize the metabolic, neurogenic and neurohumoral control mechanisms in cerebral circulation, revealing inadequate angiospastic reaction in the defined vascular beds. The data demonstrate the role of reactivity of the cerebral blood vessels in the pathogenesis of discirculatory encephalopathy, and hence the possibility of precise choice of the correcting treatment which conforms with the mechanisms of angiospastic disorders.

[Calcium antagonists (finoptin and senzit) in the treatment of cerebrovascular disorders].

Finoptin and sensit produced a favourable effect in the treatment of 290 patients with initial manifestations of deficient blood supply to the brain, encephalopathy attended by circulatory disturbances and ischemic brain insult. Changes in the systemic and cerebral hemodynamics were more pronounced in middle-aged and elderly patients suffering from hypertension or its combination with atherosclerosis.

[Experience in the use of finoptin (verapamil) in cerebrovascular diseases].

Twenty-seven patients with dyscirculatory encephalopathies grades I and II and 20 acute ischemic stroke patients received finoptin (verapamil) at daily 5 to 10 mg i. v. or 120 to 240 mg oral doses. The course lasted for 2 months. Rheoencephalography (RhEG) revealed an increase in cerebral blood filling 3 to 5 min after the single finoptin administration. The effect lasted for an average of 40 to 60 min. Central hemodynamic changes were represented by lowering blood pressure and total peripheral vascular resistance, increasing stroke volume and cardiac output. The effect was observable 40 min after a single i. v. injection of the drug. Beginning from the 2nd to 3rd day of the treatment, the patients' subjective conditions improved with reduced strength of general cerebral signs of stroke, RhEG and ECG changes indicating the permanent increase in cerebral circulation and reduced left ventricular overload.
