Hair casts.

A case of hair casts in an 8-year-old girl is described. The disorder is rare, the first description dating back to 1897 with a total of only 31 cases reported in the literature since then. The results of research conducted using the optic and electron microscopes (transmission and scanning) are recorded. These studies show that the typical lesion consists of two concentric layers of keratinized cells of differing structure: one resembling the inner layer of Huxley's sheath and the outer layer resembling Henle's layer.

Tinea capitis in the adult: two case studies.

The authors describe two cases of tinea capitis in adults, one due to Microsporum canis and the other to Trichophyton rubrum. The atypical clinical presentation of the dermatophytosis in these two cases is stressed. It is hypothesized that in both cases the disease spread from primary lesion on the face.

Therapeutic experience with ketoconazole.

The efficacy of ketoconazole was evaluated in forty-five patients with dermatophytoses, in twelve with candidosis, in two with fixed cutaneous sporotrichosis, in one with primary cutaneous cryptococcosis and in one with pulmonary aspergillosis. Patients received 200 mg daily of ketoconazole until complete cure, except for patients with vulvovagnitis who received 400 mg daily for five days. All patients with dermatophytoses were cured within 20 to 40 days. No recurrences were observed at post-therapy follow-up. Patients with candidosis were also cured, with only one recurrence. Patients with deep mycoses recovered, except for one with fixed cutaneous sporotrichosis, who showed slight improvement. No significant side-effects were observed during therapy. In conclusion, ketoconazole is an effective therapeutic agent for fungal infections.

Mycosis fungoides with monoclonal gammopathy, hypereosinophilia, and hyper-IgE.

A 61-year-old female with a well-established Mycosis fungoides also had a marked hypereosinophilia, a monoclonal gammopathy IgGk type, and an elevated IgE level. Such unusual association of abnormalities of the immune system in a given Mycosis fungoides patient has not been described before in the absence of any other underlying cause.

Dermatophyte infection in ichthyosis vulgaris.

A 31-year-old male who had been suffering from ichthyosis vulgaris developed hyperkeratosic lesions on the palms and soles and ungual alterations of both hands and feet. These lesions were resistant to topical corticosteroid and emollient cream treatments and were found to be caused by Trichophyton rubrum. Association of ichthyosis vulgaris and dermatophytosis--both very common disorders--is to our knowledge a very rare event.

Tinea capitis in the Florence area between 1985 and 1993.

This paper describes the main epidemiological findings in 203 patients with tinea capitis and in seven other patients with dermatophytosis that had also extended to the scalp. The patients were observed over a 9-year period (from 1985 to 1993) at the Department of Dermatology of the University of Florence. Tinea capitis was the fourth most frequent dermatophytosis. Most of the patients were children, although 23 were adults. The most frequent aetiological agent was Microsporum canis. It is interesting to note that there were 12 cases of tinea capitis due to Trichophyton violaceum in African immigrants. This dermatophyte was believed to have disappeared from Italy. One case was due to Epidermophyton floccosum.
