The philosophy of practice governs the rate of obstetric intervention: analysis of 212 units in the United Kingdom.

OBJECTIVE: To investigate the relationship between the practices of obstetric intervention in individual units. STUDY DESIGN: A retrospective analysis of the annual statistical returns to the Royal College of Obstetricians and Gynaecologists of 222 units in the UK for 1997-98. RESULTS: A total of 212 units that delivered more than 1000 babies were analyzed. A positive correlation was found between the rates of assisted vaginal delivery and emergency Cesarean section (Spearman's rho = 0.207, p = 0.003) and the rates of emergency and elective Cesarean section (Spearman's rho = 0.205, p < 0.001). There was a significant difference between unit size and the rate of spontaneous vaginal delivery (Kruskall-Wallis chi2 = 14.5, p = 0.002) with larger units having a lower spontaneous vaginal delivery rate. CONCLUSION: These results indicate that those units which have a high emergency Cesarean section rate also have a high elective Cesarean section rate and assisted vaginal delivery rate. The philosophy of an individual unit towards intervention is a general philosophy and determines the intervention rates for all procedures.

A randomized trial of amifostine as a cytoprotective agent in patients receiving chemotherapy for small cell lung cancer.

A randomized trial was conducted to determine whether administration of Amifostine with chemotherapy for small cell lung cancer could decrease the toxicity. 84 patients with small cell lung cancer of favourable prognosis (limited disease, performance status 0-1; limited disease with performance status 2 but normal sodium and alkaline phosphatase, or extensive disease with performance status 0-1, normal sodium and alkaline phosphatase) received treatment with Ifosfamide 3 g/m(2)intravenously, Carboplatin (Glomerular filtration rate + 25) x6 mg intravenously, Etoposide 50 mg orally, twice daily, for 7 days, every 3 weeks. Patients were randomized to receive amifostine 740 mg/m(2)immediately prior to the intravenous drugs (n = 42) or to receive chemotherapy alone (n = 42). The two groups were similar with respect to baseline prognostic factors. There was no significant difference in the occurrence of grade III or IV neutropenia or thrombocytopenia between the two groups, nor in the response rate or overall survival, for which the median was 11 months in the chemotherapy only group and 14 months in the group treated with amifostine. This study has not shown a protective effect from the use of amifostine with this regimen and there does not appear to be any effect upon the efficacy of treatment.

"Sparse" temporal sampling in auditory fMRI.

The use of functional magnetic resonance imaging (fMRI) to explore central auditory function may be compromised by the intense bursts of stray acoustic noise produced by the scanner whenever the magnetic resonance signal is read out. We present results evaluating the use of one method to reduce the effect of the scanner noise: "sparse" temporal sampling. Using this technique, single volumes of brain images are acquired at the end of stimulus and baseline conditions. To optimize detection of the activation, images are taken near to the maxima and minima of the hemodynamic response during the experimental cycle. Thus, the effective auditory stimulus for the activation is not masked by the scanner noise. In experiment 1, the course of the hemodynamic response to auditory stimulation was mapped during continuous task performance. The mean peak of the response was at 10.5 sec after stimulus onset, with little further change until stimulus offset. In experiment 2, sparse imaging was used to acquire activation images. Despite the fewer samples with sparse imaging, this method successfully delimited broadly the same regions of activation as conventional continuous imaging. However, the mean percentage MR signal change within the region of interest was greater using sparse imaging. Auditory experiments that use continuous imaging methods may measure activation that is a result of an interaction between the stimulus and task factors (e.g., attentive effort) induced by the intense background noise. We suggest that sparse imaging is advantageous in auditory experiments as it ensures that the obtained activation depends on the stimulus alone.

Emergency contraception: a survey of women's knowledge and attitudes.

OBJECTIVES: To assess women's knowledge and attitudes in relation to emergency contraception and to identify ways in which these might be improved. DESIGN: Postal survey: questionnaire seeking level of knowledge of emergency contraception, and attitudes to use, publicity and availability. POPULATION: A stratified random sample of 2000 Grampian women aged 18 to 47. Women were identified through the Community Health Index. RESULTS: Most women (94%) were aware of emergency contraception and identified an appropriate source. Fewer (39%) knew the correct timing for its use. These figures were generally higher among younger, single women. The popular media represented the commonest source of information, and GPs and Family Planning Clinics were cited rarely. Increased advertising was considered desirable by 71% (mainly older women); only 36% (mainly younger, single women) considered over-the-counter availability desirable. Reasons for these responses and factors influencing them were explored. CONCLUSIONS: Knowledge of emergency contraception is greatest among those most likely to use it, but deficient mainly in relation to the correct timing for its use and to intrauterine methods. Publicity should concentrate on the timing of its use. The popular media are an important publicity vehicle, but health professionals appear to be under-used. Many women hold opinions on advertising and over-the-counter availability of emergency contraception which will have to be considered if deregulation proceeds.
