Aplasia cutis congenita, cleft palate, epidermolysis bullosa, and ectrodactyly: a new syndrome?

Aplasia cutis congenita (ACC) is a skin disorder in which localized or widespread areas of skin are absent at birth. It has been associated with numerous anomalies and recognizable syndromes. We report a newborn infant with ACC of the scalp, multiple facial abnormalities including cleft palate but not cleft lip, hypoplasia of the distal phalanges of the hands, ectrodactyly of the feet, and epidermolysis bullosa of the extremities and lower trunk. Although this patient had some features that overlapped with ectrodactyly-ectodermal dysplasia-clefting syndrome, the absence of cleft lip and presence of additional skin abnormalities made that diagnosis unlikely. This could represent a newly recognized syndrome of numerous malformations in which ACC is associated with a constellation of previously undescribed structural anomalies.

Collagen vascular diseases of childhood.

This article is devoted to collagen vascular diseases of childhood. The connective tissue diseases that are discussed are lupus erythematosus, scleroderma, and dermatomyositis. Of the vasculitic syndromes, Henoch-Schönlein purpura, acute hemorrhagic edema of infancy, and polyarteritis nodosa are discussed. Although the collagen vascular diseases are rare in the pediatric age group, when they occur they can be very serious.

Interhospital relocation of long-stay psychiatric patients: a prospective study.

The reorganization of psychiatric services in the United Kingdom has resulted in relocation of many long-stay patients, but such transfers are seldom evaluated. Twenty-four chronic psychiatric patients destined for interhospital relocation were investigated prospectively. After transfer, no fatality occurred, but the group showed a significant deterioration in behaviour, with some changes persisting at 6 months.

Sources of carbon for hepatic glycogen synthesis in the conscious dog.

To identify the source(s) of carbon for the indirect pathway of hepatic glycogen synthesis, we studied nine 42-h fasted conscious dogs given a continuous intraduodenal infusion of glucose, labeled with [1-13C]glucose and [3-3H]glucose, at 8 mg.kg-1.min-1 for 240 min. Glycogen formation by the direct pathway was measured by 13C-NMR. Net hepatic balances of glucose, gluconeogenic amino acids, lactate, and glycerol were determined using the arteriovenous difference technique. During the steady-state period (the final hour of the infusion), 81% of the glucose infused was absorbed as glucose. Net gut output of lactate and alanine accounted for 5% and 3% of the glucose infused, respectively. The cumulative net hepatic uptakes were: glucose, 15.5 +/- 3.8 g; gluconeogenic amino acids, 32.2 +/- 2.2 mmol (2.9 +/- 0.2 g of glucose equivalents); and glycerol, 6.1 +/- 0.9 mmol (0.6 +/- 0.1 g of glucose equivalents). The liver produced a net of 29.2 +/- 9.6 mmol of lactate (2.6 +/- 0.8 g of glucose equivalents). Net hepatic glycogen synthesis totaled 9.3 +/- 2.5 g (1.8 +/- 0.4 g/100 g liver), with the direct pathway being responsible for 57 +/- 10%. Thus, net hepatic glucose uptake was sufficient to account for all glycogen formed by both the direct and indirect pathways. Total net hepatic uptake of gluconeogenic precursors (gluconeogenic amino acids, glycerol, and lactate) was able to account for only 20% of net glycogen synthesis by the indirect pathway. In a net sense, our data are consistent with an intrahepatic origin for most of the three-carbon precursors used for indirect glycogen synthesis.

Mental health and acquired hearing impairment: a review.

Recent work demonstrates that hearing impairment is much more common than previously suspected. The disability may be unrecognized or denied by the sufferer and may attract social stigma rather than sympathy from others. The effect on mental health is surprisingly neglected. Early studies of psychiatric patients suggested hearing impairment is an important cause of paranoid illness, but more recent studies of wider populations have failed to confirm this association. Hearing impairment is unrelated to intellect when controlled for age, but is an important differential diagnosis of dementia in the elderly. Evidence accumulates to suggest that the hearing impaired are vulnerable to depression, social stress and isolation, but reliable controlled studies of psychiatric sequelae are needed.

Neuroleptic malignant syndrome: a case report with post-mortem brain and muscle pathology.

The neuroleptic malignant syndrome is a rare but dangerous complication of treatment with neuroleptics. The aetiology and pathophysiology of the syndrome are reviewed, and a fatal case is presented where both brain and muscle pathology are described. Striking myopathic changes in this case, accompanied by only minimal and non-specific brain abnormalities, support a peripheral rather than central mechanism for the hyperthermia.

Fetal heart rate monitoring by telephone. I. Development of an integrated system in Cardiff.

We report the development of a practical dedicated system for domiciliary fetal monitoring integrated in a scheme for its rational application. From experience of 1120 domiciliary recordings in 74 women (64 with high-risk pregnancies), we suggest that domiciliary monitoring applied within a structured clinical context should be as safe as monitoring in hospital.

Physiologic and electrocardiographic responses of American river otters (Lutra canadensis) during chemical immobilization and inhalation anesthesia.

Rectal temperatures and heart rates of American river otters (Lutra canadensis) decreased significantly (P less than 0.05) during chemical immobilization with i.m. ketamine hydrochloride in combination with xylazine hydrochloride and acepromazine and during inhalation anesthesia with isoflurane. Anesthetized otters showed a tendency for apnea during induction and while dorsally recumbent, which was reflected by a respiratory acidosis on arterial blood gases. Declines in rectal temperatures and heart rates were not found to be a function of dosage (mg/kg) of the ketamine combination used except for rectal temperatures of otters in relatively poor body condition (inanition). The electrocardiograms of isoflurane-anesthetized otters were similar to those recorded on immobilized otters with the exception of an r' deflection in the ventricular depolarization complex (RSr'). Electrocardiographic criteria were not found which predicted the degree of right ventricular or generalized cardiac enlargement seen radiographically.

Morphology of the organic matrix of the spicule of the sea urchin larva.

The morphology of the organic matrix of the skeletal spicule of the sea urchin pluteus larva has been analyzed by light and electron microscopy. Purified isolated spicules can be demineralized, and they reveal lamellae of an irregular fibrillar nature with overall outlines similar to the shape of the intact spicule. Sections through the spicule show the fibrous lamella is probably composed of interconnected concentric sleeves.

Ocular penetration of topical chloramphenicol in humans.

Commercial ophthalmic chloramphenicol solutions and ointments were applied to human eyes, and the concentrations of chloramphenicol in aqueous humor and tear samples were analyzed using gas-liquid chromatography. Within several minutes after a single application of 0.5% chloramphenicol solution, the tear concentrations of the drug had fallen below 1 mg/liter, the minimal bacteriostatic concentration for many ocular pathogens. Repeated drops of 0.5% chloramphenicol solutions during several hours were required to produce an aqueous humor concentration of 1 mg/liter. A single application of 1% chloramphenicol ointment gave prolonged drug concentrations in the tears and aqueous humor, falling to 1 mg/liter in two to four hours. The repeated use of chloramphenicol solution or ointment was well tolerated by the patients. We conclude that the penetration of chloramphenicol in the anterior segment of the eye is best carried out by the repeated application of chloramphenicol ointment.
