Involvement of membrane galactose in the in vivo and in vitro sequestration of desialylated erythrocytes.

The influence of terminal beta-galactose residues for the in vitro and in vivo sequestration of sialidase-treated erythrocytes by macrophages was investigated. Preincubation of rat peritoneal macrophages with galactose, oligosaccharides, glycoproteins and glycolipids with terminal beta-galactose residues inhibits both binding and phagocytosis of sialidase-treated erythrocytes by masking a beta-galactose-specific lectin on the macrophage cell membrane. These inhibition studies show that binding via demasked erythrocyte surface beta-galactosyl residues to this lectin is necessary for the subsequent phagocytosis step. According to these observations, repeated injections of lactose (30mM serum concentration) and asialo-fetuin (10-30 microM serum concentration) into the blood stream of rabbits led to a reduction of the rapid sequestration rate of sialidase-treated erythrocytes. Asialo-fetuin proved to be a much more potent inhibitor than lactose, in accordance with the in vitro experiments. This inhibition is reversible, as after the disappearance of the inhibitory effect, the sialidase-treated erythrocytes were again rapidly removed from the circulation to an extent similar to that of the experiments without inhibitors. No significant influence on binding and phagocytosis was measured in the presence of sialyllactose and native fetuin in vitro, or of native fetuin on sequestration in vivo. The experiments with rabbits show that a beta-galactosyl-specific lectin seems to be involved in the mechanism of sequestration of desialylated erythrocytes in vivo, as has been observed in vitro with rat peritoneal macrophages.

[Psychosocial aspects of extremely tall girls with a treatment-induced precocious puberty (author's transl)].

Is psychosocial development influenced by tall stature and/or induced precocious puberty? This question was examined in 20 girls at 9,5 to 15,8 years of age who were admitted to estrogen treatment because of excessive constitutional tallness. Additionally, reference was made to the results of psychologically directed interviews with 86 girls who had undergone such treatment when 9 to 13 years old, and who belonged to a total collective of 387 tall girls cared for in the years 1966 to 1980. About half of the girls let discern symptoms pointing to neurosis, culminating in late puberty (age of dancing-lesion). Further essential psychical problems, originating from treatment-induced "early normal puberty", were not found. It is inferred that no serious objections arise, from the psychological point of view, against estrogen treatment of tall stature in prepuberal girls.

[Discriminating power of multivariate parameters of respiration in epidemiological studies (author's transl)].

Applying a broad scale of lung function tests in epidemiological surveys (929 metal workers of a big industrial shop, 211 of them with signs of chronic obstructive lung disease, COLD, and 1,266 pupils of 2 towns with different dust exposition) we found similar multivariate discriminant functions in all age groups between 7 and 79 years of age to differentiate between COLD-population and normal population. Residual volume in % of total capacity (RV/TC) seems to be the most relevant central parameter in all age groups, which should be combined with one of the inhomogeneity parameters of single breath or multi breath distribution tests ( delta F/ delta V or breath of inhomogeneity IHB) in juveniles between 7 to 16 years of age and with closing volume (CV/VC) in adults between 20 to 79 years of age. Conventional ventilation parameters and the arterial oxygen tension don't contribute additional information to multivariate analysis, which isn't already summarized by the parameters RV/TC.

[Airway-resistance in pregnancy measured by oscillation-technique (author's transl)].

Measurements of resistance were performed in 430 healthy pregnant women, 30 non-pregnant women and 33 puerperae by employing the oscillation-technique. The patients were studied in sitting and standing positions. It was found, that 2 maxima of resistance exist in pregnancy: the first in the 6th, the second in the 9th month. In the sitting position the resistance was higher than in the standing one.

[Measurement and model-interpretation of new parameters of lung mechanics (author's transl)].

In 34 subjects (healthy and patients with obstructive and restrictive lung diseases) the components R and X of the respiratory impedance were measured in relation to the oscillatory frequency f by means of the forced oscillations technique. Using a frequency range from 5 to 30 Hz we found significant differences in these three groups of subjects for the frequency dependence of the R(f) and X(f) curves. A model-analysis showed that frequency dependence of R and X is mainly caused by the influence of the compliance of the central and upper airways. The magnitude of this influence is a function of the peripheral airway resistance.

[The so-called metastasizing thyroid adenoma from the orthopaedic viewpoint (author's transl)].

The so-called metastasizing thyroid adenomas, which are now know as follicular thyroid carcinomas, present diagnostic problems again and again on account of their high degree of differentiation, since the primary tumour is often recognized as malignoma only after metastases have put in their appearance in other parts of the body. In this article, the authors report on a case of a follicular thyroid carcinoma which became manifest via a tumour of the size of an infant's head at the right upper part of the ilium.

Site and type of impaired lung function in extrinsic allergic alveolitis.

We investigated 109 persons exposed and sensitized against organic antigens. 46 of them (group I) are affected with the typical clinical and roentgenological signs of allergic alveolitis. The other 59 subjects (group II) were sensitized only. Included in all examinations were immunological diagnosis by means of counterimmunoelectrophoresis, systematic interviewing for case histories, and registration of respiratory symptoms. Pulmonary function was recorded by means of the following techniques: analysis of ventilation (flow-volume indices), respiratory mechanics of the larger airways and lung (body plethysmography, esophageal balloon technique), respiratory mechanics of the small airways (closing volume, amplitude of cardiogenic oscillations), distributional analysis, diffusion analysis, blood gas analysis, and right heart catheterization. Extrinsic allergic alveolitis caused changes in lung mechanics, in airways as well as in lung tissue. The most impressive findings in allergic alveolitis are mechanical inequalities of ventilation. These inequalities may be caused by changed mechanics of overall airways indicated by obstruction parameters and by changed structure of lung tissue reflected in decreased compliance, transfer factors and increased pulmonary arterial pressure. Not only a restrictive but also an obstructive disturbance of ventilation is found. Impairments are less in sensitized persons. We find slight mechanical and ventilatory inhomogeneities. An overall airway obstruction is obvious but not to such a large extent as in allergic alveolitis. On the contrary, here the lung tissue is not affected. Compliance and diffusion are within the normal range. There is no typical functional pattern specific for the diagnosis of allergic alveolitis.

High incidence of extrahepatic carcinomas in idiopathic hemochromatosis.

In a series of 36 consecutive patients with proved idiopathic hemochromatosis and with a mean follow-up time of 8 years, 10 patients developed 11 carcinomas (30.6%). Five patients had hepatomas and six patients extrahepatic carcinoma involving lungs (four), oral cavity (one), and thyroid (one). The findings suggest a causal relationship between hemochromatosis and extrahepatic cancer. The evidence of an increased cancer risk in relation to cirrhosis and to iron toxicity are discussed.

[Intracellular and extracellular acid mucopolysaccharides in human bronchus carcinoma and lung metastases - prognostic parameters?].

Local cellular and extracellular (acid mucopolysaccharide) reactions in human lung cancers and metastases - prognostic parameters? The role of immunologic reactions in human malignant tumors is still unsettled. In the work presented, 72 primary lung cancers and 17 pulmonary metastases of 8 different organs of origin were studied with histochemical techniques for cellular infiltrates (eosinophils and macrophages) and extracellular acid mucopolysaccharides (AMPS). The results showed a positive correlation between eosinophils and macrophages and a negative correlation between cellular reactions and the deposition of AMPS. The clinical course of radically operable patients with primary lung cancer 2 to 3 1/2 years after operation showed that patients with the reactive pattern "strong local eosinophilia, no AMPS deposition" had the best prognosis with 12 of 14 patients free of tumor. Of the patients with the same tumor stage, but lacking local eosinophilia, more than half died from the tumor or had generalized metastases within the same period. The most frequent pattern in patients with pulmonary metastases was "no local eosinophilia, strong AMPS reaction". These observations suggest that cellular reactions in radically operable primary lung cancer express immunologic responses. These are often absent in metastases.
