[Automated analysis of changes in parameters of lung mechanics with alterations in breathing frequency, lung volume, and physical effort (author's transl)].

Changes in parameters of respiratory mechanics with alteration in breathing frequency, lung volume and physical effort were determined in 23 subjects using automated data collection and data processing. Resistance R, Compliance C and work of breathing W show great dependence on breathing frequency and lung volume. An objective measuring of their changes during exercise requires a constant lung volume and a standardized breathing frequency. Parameters R and C and the maximal pressure amplitude DPM divided by the tidal volume VT show no significant alterations in several degrees of exercise. Work of breathing and work of breathing per tidal volume are enhanced significantly with increasing exercise. These parameters may estimate the mechanics of breathing better than R and C in patients with dyspnea during exercise.

[Problems in the therapy of central arterial ectasias and elongations in long-lasting arteriovenous fistulas].

Acquired arteriovenous fistulas should be surgically cured as soon as possible after the diagnosis. In all the patients we treated, the arteriovenous fistula had existed for quite a long time, i.e. up to 26 years. These arteriovenous fistulas were surgically closed by the separation-method, the resection of the vessels and vascular reconstruction, or by the transvenous technique. - Arteriovenous fistulas, which have been in existence for quite a long time, result in cardiac dilatation, which can be cured after closure of the arteriovenous fistula. Furthermore, there can be pathologic changes of the vein and artery wall: chronic venous insufficiency and arterial ectasia can result. - After closure of the arteriovenous fistula our patients no longer complained of venous reflux disturbances. Even after closure of the arteriovenous fistula, an arterial ectasia is not reversible. The reasons for this are discussed. Symptoms, which occur after the closure of an arteriovenous fistula, which had existed for a long period and had produced a severe arterial ectasia, are looked upon as an autonomous disease. During the progress of this disease many complications can occur, even the rupture of the ectatic artery. The question is therefore discussed, whether it is justified to resect the ectatic arterial segment together with the arteriovenous fistula and to implant an alloplastic vascular graft in its place during the initial operation.

[Juvenile bone cysts and giant cell tumors (author's transl)].

The attempt is made to demonstrate clinical, radiographical, morphological differences between juvenile bone cyst and giant cell tumor as shown in the patients of the Surgical Department of the University of Dusseldorf and literature. It is possible to derive inferences in differential diagnosis from the age of the patient, clinical course and statistically most frequent locations. Radiographically the bone cyst distinguishes itself by its central localisation in the metaphysis, where as the giant cell tumor has an excentric position in the epiphysis with a tendency of extending into the metaphysis. Giant cells occur in both diseases. While the bone cyst is an tumor-like lesion, the giant cell tumor is a real neoplasia. The therapeutical procedure varies according to different prognostical significance of the two diseases. Therefore, it is necessary to draw an exact line of demarcation between the comparetively harmless juvenile bone cyst and the semimalign giant cell tumor.

[Screening tests for the determination of risk groups in respiratory insufficiency under conditions of epidemiological serial studies].

A report is given on a system of effective respiratory functional tests which proved its value when used in epidemiological mass examinations in connection with the mass miniature radiography in Berlin, Brandenburg and Halle as well as in mass examinations in Berlin enterprises. Thereby a new development - Pneumotest - is presented, an electronic spirometer for the recognition of ventilatory and distribution analytic preliminary parameters with digital indication of 5 sizes of the breath test manoeuvre which is to be applied as successor of the well-known spirotest device. In this paper the functional capacity of the further developed fluorographic densimetry as preliminary emphysematic test is proved. The application of the rapid helium or O2/N2 breathing back technique for the determination of residual volume and closing volume has in connection with the automatic establishment of the measuring values proved as basic diagnostics for emphysema with a large efficiency and at the same time with high reliability. The digestion of the epidemiological functional data with the help of electronic data processing is carried out in the own microcalculator system following immediately the application during examination.

[Allergic cutaneous vasculitis (purpura Schönlein-Henoch) (author's transl)].

The concept of allergic cutaneous vasculitis of the dermatologists is presented from the point of view of its extensive coincidence with the clinical concept of Schönlein-Henoch's purpura with particular emphasis on clinical morphology, histology and the more recent immunofluorescent optical findings.

Histochemical studies of 3 beta- and 20alpha-hydroxysteroiddehydrogenase in the adrenals and ovaries of the nu/nu mouse.

Adrenal glands and ovaries of nu/+ and nu/nu female mice were examined histochemically throughout the postnatal life for changes in distribution, intensity and appearance of activities of 3 steroid-converting enzymes. While the 3beta-OH-steroid- and pregnenolone-dehydrogenase did not reveal any difference, the 20alpha-hydroxysteroid-dehydrogenase (20alpha-OH SDH) showed a constant behaviour: the onset of the enzyme activity was detectable in the adrenals of nu/+ mice after the second week of age, in nu/nu mice 3--4 weeks later. The localization of 20alpha-OH-SDH was strictly confined to the zone of small cells, a zone lying between the x-zone and the zona fasciculata. In nu/+ mice, after reached sexual maturity at about 6--8 weeks of age, the enzyme progressively disappeared from the adrenals and simultaneously appeared in the ovaries. In nu/+nu mice, which had been kept separated from mating animals, the 20alpha-OH SDH persisted in adrenals even in adult stages. After pregnancy, both--nu/nu and nu/+, showed 20alpha-OH SDH-activity in ovaries and none in adrenals. In both, the enzyme was localized in involuting corpora lutea, whilst in nu/nu it was also present in the intersitial cells. This study indicates no disturbance in the formation of progesterone, but the process of its degradation, as indicated by 20alpha-OH SDH-activity, is definitely different in nu/+ and nu/nu.

[Surgical problems in the treatment of aneurysm of femoral artery bifurcation].

The different types and causes of aneurysm in the region of the femoral artery bifurication as well as the possibilities of surgical treatment are discussed on the basis of 4 patients, each with a differnt type of aneurysm. The examples show clearly that the basic of 4 patients, each with a different type of aneurysm. The examples show clearly that the basic disease can prevent successful surgical reconstruction of the arterial blood circulation. The use of synthetic prostheses is not possible in case of infection in this area because of the deleterious subsequences unless the entire infected area can be avoided with a bypass. (Fig. 2a). Trials with autologous veins and arteries, are , therefore, justified. The technical procedure for the use of the greater saphenous vein or deep femoral artery is described. In the authors' patients as well as those reported in the literature, it is clear than aneurysms in the region of the femoral artery bifurcation are sometimes difficult to treat. Hwerver, their localisation near the surface should allow early diagnosis and prompt treatment.
