[Hereditary chronic relapsing calcifying pancreatitis].

The clinical picture of the hereditary chronic relapsing calcifying pancreatitis is demonstrated on the basis of a family. In 7 members of the family the disease could be ascertained, in other 3 members it is probably existing. Main symptom in all patients are severe relapsing epigastric pains with manifestations already in childhood or time of youth. Calcifications of the pancreas were always found and except one female patient an exocrine insufficiency of the pancreas with superficial steatorrhoea. A diabetes mellitus became manifest only in three of the seven patients. A larger attention is to be paid to the diagnosis of this special form of the chronic relapsing pancreatitis, in order to avoid long diagnostic wrong ways and non-indicated laparotomies in etiologically not clarified epigastric complaints. The therapy of the hereditary pancreatitis is conservative. Only in complications or in a not controllable so-called syndrome of pain it is recommended to decide on an operative approach, as in the family demonstrated in two female patients.

[Pathogenesis and prognosis of acute hepatitis in diabetics].

After hospitalisation in a diabetes ward from 1973 to March 1978 altogether 21 diabetics fell ill with an acute virus hepatitis B. Comprehensive after-examinations on 16 patients were carried out on an average 25 months after the clinical beginning of the hepatitis B. In half of the patients an HBsAg persistence was present. Chronic hepatides were established in a quarter of the cases. The findings of the after-examination in 4 sisters who simultaneously fell ill with hepatitis B remained unsonspicuous. The possible causes of the more unfavourable courses in the diabetics are discussed, also the increasing negative influence by the therapy with corticosteroids in prolonged course and with azathioprin in chronic course partly performed up to 1976.

[Insulin lipoatrophy and its elimination through the use of monocomponent insulin].

Since their first description 50 years ago severe forms of lipoatrophy appearing above all in girls and women are occasionally a cosmetically significant and therapeutically problematic side effect of the insulin treatment of diabetes mellitus. On two own female patients is shown, how the hitherto recommended measures failed in distinctive cases and only the treatment with monocomponent-insulin of the pig led to the restoration of the fatty tissue. The successful therapy of lipoatrophy with monocomponent insulin in all cases hitherto published confirms the hypothesis that a local antigen-antibody-reaction to highly molecular constituents of the commercial mixed insulins causes the atrophy of fatty tissue.
