A revised group exercise program for osteoarthritis of the knee.

The purpose of this study was to determine the effectiveness of a group exercise program for subjects with osteoarthritis of the knee referred for physiotherapy in terms of pain, physical function and gait. This study was initiated after an initial group exercise program at this centre failed to demonstrate significant changes in pain, physical function or gait. The study was based on repeated measures with a two month follow up, carried out in the outpatients department of a large public hospital. Pain levels in various functional situations were assessed on visual analogue scales, physical function with the Stanford Health Assessment Questionnaire, and quantitative gait variables at a normal and a fast self-selected speed with an electric foot switch walkway. On completion of the program (n = 40) more than 90% of the pain and physical function scores demonstrated significant improvements without increases in medication, use of walking aids or fatigue. The gait variables of velocity, cadence and stride length at both self-selected speeds demonstrated significant increases at this assessment without deterioration of gait symmetry. All improvements were maintained at the two month follow up assessments. This study suggests that a clinically realistic group exercise program supplemented with a home program can reduce pain, increase physical function and objective gait measures in subjects with osteoarthritis of the knee.

A double blind randomised trial of low power laser treatment in rheumatoid arthritis.

OBJECTIVES: To define the value of low power laser treatment in small joint rheumatoid arthritis. METHODS: Twenty five women with active disease were recruited. The metacarpophalangeal and proximal interphalangeal joints of one hand were treated with 12 J/cm2 for 30 s with a gallium-aluminium-arsenate laser. The other hand received a sham laser treatment designed so that neither therapist nor patient could distinguish the active laser from the sham laser. Each patient received 12 treatments over four weeks. The following parameters were measured: pain as assessed by visual analogue scale; range of joint movements; grip strength; duration of early morning stiffness, joint circumference, Jebsen's hand assessment; drug usage; total swollen joint counts; Arthritis Impact Measurement Scales; three phase bone scans; haematological and serological tests. RESULTS: A total of 72% of patients reported pain relief but this reduction was reported equally in both hands. No significant changes were seen in other clinical, functional, scintigraphic, or laboratory features. Neither patients nor staff were able to detect which hand was treated with the active laser. CONCLUSION: When this specific laser and dose regimen was used, low power laser treatment had no objective effect on patients with rheumatoid arthritis. It did appear to produce analgesia through a powerful placebo effect.

Type A-B behavior and self-reported health problems.

To measure the relationship between Type A-B behavior and stress-linked symptoms, a group of undergraduates were asked to respond to the Glass version of the Jenkins Activity Survey and a 25-item stress-linked symptoms checklist. Data from 472 students were analyzed. As predicted, the results indicated that Type A persons experience specific stress-linked symptoms more intensely than their Type B peers.

[The reimplantation of teeth avulsed by trauma].

The paper underlines the importance of the reimplantation of teeth following injury in young subjects. The positive results achieved within the short term encourage the extension of this method even to those cases with a poor prognosis (unsuitable means of conservation, long interval between injury and reimplantation). The tooth with in any case serve to retain the space until a prosthetic replacement is inserted when growth has ended.
