A prospective study of the hazards of multiple use of disposable syringes and needles in intensified insulin therapy.

The aim of the present study was to evaluate the potential hazards of multiple use of disposable syringes and needles for insulin injections over a long period of time. A special syringe container (pen-case) was developed to carry the syringe with insulin. In the microbial trial 24 patients, each injecting insulin for 1 week with one disposable needle, were studied for 201 patient-weeks with 5829 injections. Cultures of 154 needles, 155 syringe rinses, 154 pen-case rinses and the remains of insulin in 201 vials revealed no relevant contamination. In the clinical trial 100 diabetic patients were followed up for up to 7 years. Each syringe was reused for 1 to 12 weeks and each needle for 1 to 40 days (4 to 200 injections). Only sporadic redness not exceeding 4 mm2 was seen at sites of about 560,000 insulin injections. Thus, the repeated use of syringes and needles in one diabetic patient may be recommended as a convenient and safe approach in insulin administration.

Antiphlogistics in periodontology.

In 146 individuals with inflammatory periodontal disease the authors verified the therapeutical effect of Sanchelin (the mixture of sanguinarine and chelerythrine in 0.05% concentration) in gel (4% hydrogel carboxymethylcellulose) applied into pockets and in aqueous solution administered in the intrapapillar route. The effect of Sanchelin in solution was compared with the effect of 2% hydrocortison in the same way of administration. The clinical condition of periodontium and the quality of oral hygiene was evaluated by means of indexes and papillae were examined histologically. The authors demonstrated that Sanchelin in gel is an effective drug in treatment of inflammatory periodontal disease. They observed lesser effectivity when intrapapillar administration of Sanchelin in solution was used.

A comparison of antigenic structure and phage pattern with biochemical properties of Staphylococcus aureus strains isolated from sheep.

Of 84 Staphylococcus aureus strains isolated from the anterior nares of healthy sheep and from the udders of ewes suffering from purulent mastitis the 89 per cent belonging to the C biotype contained agglutinogen h2 as well as polysaccharide Abeta. Ninety-five per cent of the C biotype strains were lysed by the bovine phage 78, the human phage set giving only weak reactions at RTD X 100. Two pigment-negative deficit variants as well as three unclassified strains gave similar results while three A biotype strains and one E biotype strain were definitely different. The close correlation between biochemistry, serology and phage typing substantiates the practical usefulness of the subdivision of S. aureus into biotypes.

Staphylococci outside the hospital. Staphylococcus aureus in sheep.

Biochemical properties were studied in Staph. aureus strains obtained from the anterior nares of healthy sheep and from the udders of ewes suffering from purulent mastitis. Of the total number of 84 isolated staphylococcal strains 75 (89.3%) were classified as the C biotype. These undoubtedly sheep-adapted staphylococci produced pigment and beta hemolysin, they were growing on crystal violet agar as the negative type in violet colonies lacking both fibrinolysin and alpha hemolysin. All of them coagulated human plasma within one hour after inoculation. In bovine plasma 27 strains (36%) formed the coagulum within 3 hours, 16 (21.3%) within 24 hours, and the remaining 32 strains (42.7%) only within 72 hours. Mannitol was fermented after five days only by 33 cultures (44%). The staphylococci were sensitive to the applied antibiotics without exception. All these sheep-adapted staphylococci had analogous biochemical features to the earlier discussed staphylococcal strains obtained by the authors from the nasal cavities of cattle. Next two strains were denoted as deficit variants of the C biotype because of their lack of pigment. Of quite a different character were 3 strains (3.6%) of the A biotype and one strain identified as the E biotype. The former were presumably transferred to sheep from man while the latter from a dog. The remaining 3 strains could not be subdivided according to the classificatory criteria used here.
