A study on clinical profile of falciparum malaria in a tertiary care hospital in south India.

Malaria continues to be a major problem in tropical countries. To study the clinical features and complications of malaria in a tertiary care hospital in south India, records of 183 patients were analysed. Among 86 patients with P. falciparum and mixed infection, 24 (28 per cent) had cerebral malaria and 32 (37 per cent) had hyperbilirubinemia. Twenty-three out of 32 (72 per cent) patients with jaundice had direct hyperbilirubinemia and elevated liver enzymes suggesting hepatocellular damage. Mortality of the order of 10 per cent was seen only in P. falciparum malaria. High incidence of hepatic involvement and hepatorenal failure were the unusual features observed in the study.

Wuchereria bancrofti induced pancreatitis.

A case of pancreatitis of filarial origin is reported for the first time. The patient presented with chronic pancreatic pain. Laparotomy revealed an inflammed pancreas and fine needle aspiration cytology confirmed pancreatitis. Imprint of a peripancreatic lymphnode revealed a microfilaria. He was successfully treated with Diethyl Carbamazine.

Emerging importance of Pneumocystis carinii among indian immunosuppressed patients.

Pneumocystis carinii is known to cause both significant morbidity and mortality in immunosuppressed individuals. In a six and a half year period starting with 1992, a total of 204 samples including bronchial aspirates, induced sputum and suction catheter tips were examined for P. carinii by direct microscopic examination of Giemsa's and toluidine 'O' stained smears. At a later stage of the investigation, immunofluorescent staining using monoclonal reagent (Meriflour, USA) was also used for examination of the specimens. In all 24 (11.8%) of 204 samples were positive for P. carinii. In addition, Pneumocystis carinii pneumonia (PCP) was diagnosed in five (33%) of 15 patients of acquired immunodeficiency syndrome (AIDS) with opportunistic infections. All these 15 patients had CD4 T cell counts of less than 500 T lymphocyte equivalent (TLE) /ml as measured by a Trax ELISA assay. Laboratories in India have to be better equipped for an early and correct diagnosis of PCP that is bound to rise with the increase in the number and variety of immunosuppressed patients.

Development of vaccines against falciparum malaria.

Falciparum malaria is a leading cause of death in many countries. Drug resistance has emerged as a major cause for concern, increasing the pathogenic potential of the parasite. So far vaccines have been an elusive option. We discuss here some of the antigens and vaccines that show promise and on which studies are in progress.
