[Second-line irinotecan chemotherapy in the treatment of metastatic colorectal cancers: phase III trials].

Treatments of advanced colorectal cancer progressing after a 5FU based chemotherapy have not been extensively studied. However, 5FU in continuous infusion, L-OHP alone or in combination with 5FU and CPT11 have proved their efficacy in terms of tumor growth control and symptomatic effect. Irinotecan only has been evaluated prospectively in phase III studies. This paper reports the results of the two randomized European studies which have demonstrated the efficacy of irinotecan used at the dose of 350 mg/m2 administered over 90 minutes every 3 weeks in patients progressing after a 5FU-based chemotherapy. The first study compared irinotecan versus best supportive care in a group of 279 patients. It demonstrated an overall survival benefit (9.2 versus 6.5 months, p < 0.0001) with a one-year survival of 36.2% for patients treated by irinotecan versus 13.8%. There was also a quality of life benefit, especially for asthenia and pain in favor of the irinotecan arm. The second study compared irinotecan to a second-line infusional 5FU and randomized 260 patients. Irinotecan treated patients lived for significantly longer than those on 5FU: median time of survival was 10.8 months versus 8.5 months (p = 0.035). Survival at 1 year was increased from 32% in the 5FU arm to 45% in the irinotecan arm. Pain-free survival and symptom-free survival were better for patients treated by irinotecan and the global quality of life score was in favor of irinotecan when assigning null value to missing data due to death in both arms. Both treatments were equally well tolerated. These two randomized studies have proved the efficacy of irinotecan as second line chemotherapy for colorectal cancers progressing under 5FU. Combination of irinotecan to 5FU and/or L-OHP have now to be evaluated in this situation.

[Judgement criteria in treatments of advanced colorectal cancers and methods of evaluation].

Because of their bad prognosis, the effectiveness assessment of advanced cancer therapy is particularly important. A strict measurement of the treatment's benefit is necessary in order to consider its true interest objectively. The assessment of the tumor response allow an appreciation of the immediate effectiveness of the treatment on the lesions. An objective response doesn't mean that there is a survival improvement, which remains the primary aim of all treatments. Statistical methods are used so as to estimate overall survival or relapse-free arrival. Evaluating the quality of life is now essential in advanced cancer management. This is a complex, subjective and multi-dimensional concept and its evaluation requires some specific and validated instruments.

[Diagnostic efficiency of histological and simple microbiological tests for Helicobacter pylori detection in a community hospital].

The objective of this study was to compare histology and microbiology (direct method, urease test, culture) for the detection of Helicobacter pylori in antral biopsies. Thirty-six patients, aged between 18 and 82, with a peptic ulcer (active or not), or gastritis or duodenitis were studied. Eighteen patients (50%) had a positive culture, 17 (47%) had a positive urease test, 16 (41%) had positive direct microbiology and 15 (42%) had positive histology. Correlation between histology and culture was 84%. This study confirms the good sensitivity of histology and the urease test for the detection of Helicobacter pylori. Combination of both methods could optimise detection of the organism.

[Locoregional recurrence of adenocarcinomas of the rectum treated with irradiation combined with or without excision surgery].

PURPOSE: Retrospective study to analyze the results of external beam radiation treatment with or without surgery for loco-regional recurrence of adenocarcinoma of the rectum following previous surgery without pre- or post-operative radiotherapy. PATIENTS AND METHODS: Between March 1973 and November 1991, 211 patients with loco-regional recurrence of rectum cancer were treated with external beam radiation treatment. Radical surgery was the only initial treatment modality. Surgical resection of local recurrence was done in 36 patients and only 17 patients could undergo complete resection. Forty-seven patients underwent radiotherapy (RT) combined with surgery and 164 received external beam radiation treatment alone to a mean total dose of 46 Gy. RESULTS: Among the 151 patients whose recurrence was revealed by pain, 64 (42%) were considered to have a complete symptomatic response after loco-regional treatment with radiosurgery or RT alone. The mean duration of response was 12 months. The 3-year overall survival rate was 16%. Five prognostic factors decreased the overall survival rate in multivariate analysis: high age, sex (male), concomitant distant metastasis, no tumor resection, and low total radiation dose with external beam radiation treatment alone. The 3-year overall survival rate for patients with completely resected recurrences was 39%. CONCLUSION: External beam RT treatment can only be considered a palliative symptomatic treatment. New techniques of early detection of local recurrence and new combined modalities approaches (radiation sensitizers or intra-operative radiotherapy) with surgical resection in some favorable cases should be studied.
