[Prognostic role of pregnancy on the course of HIV infection: a retrospective cohort study (1985-92)].

In a hospital-based cohort study of HIV-infected patients, we compare the progression of the disease (occurrence of CDC group IV, death, decline of CD4 below 200/mm3) between 55 women with delivery and 89 matched control HIV positive women with no history of pregnancy. After a mean follow-up of three years, no significant difference was observed between the two groups even after adjustment on prognostic variables.

[Joint manifestations and parvovorus B19 infections: 8 cases].

Human Parvovirus B19 (PB19) may cause joint manifestations. We report eight cases of polyarthritis associated with PB19 IgM: six cases of acute polyarthritis spontaneously resolving within a few days, and two other cases suggesting that PB19 should be responsible for accutisation of chronic disease (lupus with Kikuchi's disease and HLA B27 related arthritis).

[Simultaneous vaccination against hepatitis A and yellow fever].

Simultaneous vaccination against hepatitis A and yellow fever has been studied in our unit: Santé-Voyage, Bordeaux. This randomised study included 108 healthy subjects: 36 received hepatitis A vaccine (Havrix, SKF), 36 received yellow fever vaccine and 36 received both. For hepatitis A vaccine protocol comprised one injection at day zero, one at D15, and one booster dose at 6 months. Those who received yellow fever vaccine had it at D0. Simultaneous vaccination is well tolerated and immunogenicity is as good as it is for each vaccine separately. This is particularly interesting for tropical travellers who are at risk for hepatitis A.

[Surveillance of the circulation of arbovirus of medical interest in the region of eastern Senegal].

A study about the circulation of arboviruses of medical interest in southeastern Senegal was conducted from 1988 to 1991, during and around the periods of transmission. Specific IgM antibodies were detected by ELISA test in human sera, as a sign of a recent infection within 2 to 5 months. The comparison of the serological IgM results from human surveys in different villages, and the isolations of arboviruses from mosquitoes during the same period of time permitted a rapid and global evaluation of the circulation of these viruses. A low level of yellow fever virus activity was detected both in humans and mosquitoes in 1988 to 1990. A dengue 2 epizootic occurred in 1989-1990. Dengue 2 virus was isolated from humans and mosquitoes in 1990. Some dengue 2 outbreak may occur in the upcoming years. A Zika virus epizootic outbreak was observed each year. A human strain was isolated in 1990. The other flaviviruses (West-Nile, Kedougou, Wesselsbron), Chikungunya virus, Rift Valley Fever virus and Crimean-Congo Hemorrhagic Fever virus did not seem to present a major public health concern in southeastern Senegal.

[Ventricular arrhythmia and halofantrine intake. Probable deleterious effect. Apropos of 3 cases].

Incidence and malignant forms of imported Plasmodium falciparum malaria are increasing, and chemoprevention is more and more replaced by stand-by treatment and radical cure in preventing access on return from malaria areas. Halofantrine is recommended for this radical cure: it's an habitually well-tolerated amino-alcohol with very few side-effects. We report three cases of long QT-interval due to halofantine: three different young women coming back from Africa took halofantrine (500 mg (2 tablets) six hourly for three doses on the first and the seventh day) and all presented with syncopal episodes. Serum electrolyte concentrations and echocardiograms were normal. In one case only, a diagnosis of Plasmodium falciparum malaria was made, without severe manifestations, and in the two other cases, treatment was a radical cure. In two cases, several bursts of torsades de pointes ventricular tachycardia due to halofantrine were proven and electrophysiological cardiac tests concluded that they had a congenital long QT-interval/Romano-Ward syndrome). So far halofantrine cardiac toxicity was unknown with single dose of 24 mg/kg/d. This phenomenon can be very severe in case of preexisting cardiopathy. In spite of the rarity on the congenital Romano-Ward syndrome, systematic electrocardiogram is necessary before giving halofantrine.

[Role of herpes virus simplex and cytomegalovirus as cofactors of papillomavirus in dysplastic and cancerous lesions of the uterine cervix].

Human papillomaviruses (HPV) play an important role in cervical carcinogenesis but are probably not the only factors. To assess the role of HPV 6 and 11, of herpes simplex virus (HSV) and of cytomegalovirus (CMV) as cofactors for HPV 16 and HPV 18, we used polymerase chain reaction (PCR). The genomes of these six viruses was searched for in cervical biopsies, inbred in paraffin, from 22 normal cervices, 21 low grade cervical intraepithelial neoplasia (CIN), 21 high grade CIN and 20 squamous cancers. HPV 16 DNA had the highest prevalence in cervical lesions. Its frequency reached 24% in low grade CIN, 57% in high grade CIN, 50% in cancers and 9% in normal cervices. The simultaneous presence of HPV 16 and/or HPV 18 DNA and CMV and/or HSV DNA was significantly more frequent in high grade CIN and cancers than in normal cervices. On the other hand the presence of HPV 16 and/or HPV 18 DNA in samples lacking the other viral genomes did not significantly differ between the groups. Consequently the disease-viral infection relation was not apparent when these two "potentially oncogenic" HPV are isolated without coinfection by HSV or CMV. Our results are in agreement with the hypothesis of the role of herpesviruses as cofactors in cervical carcinogenesis. These viruses may disturb the control mechanisms of cellular growth and differentiation in HPV infected cells.

Cardiac complications of halofantrine: a prospective study of 20 patients.

Halofantrine, increasingly used for treatment of Plasmodium falciparum malaria, is a normally well-tolerated amino-alcohol with very few side-effects, but torsades de pointes ventricular tachycardia due to halofantrine has been reported in a few patients with a congenital long QT interval (Romano-Ward syndrome). We performed a prospective study of the cardiac effect of halofantrine in 20 patients with 48 h ambulatory electrocardiographic (ECG) monitoring; the halofantrine levels in their serum were also determined. Minimal ECG changes were noted, with lengthening of the QT interval without clinical symptoms. This effect was dose-dependent and can be very severe in cases of pre-existing cardiopathy; it also occurs in patients without any pre-existing cardiopathy. In order to reduce the likelihood of such incidents, which are admittedly rare, we suggest performing electrocardiography on all patients before initiating treatment with halofantrine.

Atovaquone plus proguanil versus halofantrine for the treatment of imported acute uncomplicated Plasmodium falciparum malaria in non-immune adults: a randomized comparative trial.

In endemic zones, the atovaquone-proguanil (AP) combination is well tolerated and effective in treating acute, uncomplicated malaria. Trials involving non-immune patients are lacking, however. We conducted a randomized, multicenter open-label trial to determine the efficacy and tolerability of the AP combination (1,000 mg + 400 mg once daily for 3 days) in comparison with halofantrine (HF) (1,500 mg in 3 doses) in non-immune adults with imported uncomplicated Plasmodium falciparum malaria. Follow-up visits were programmed on Days 7, 14, 21, 28, and 35 after hospital discharge. Out of 48 patients enrolled in the study, 41 were assessable for the cure rate (21 in the AP group and 20 in the HF group). All the patients were cured. The mean parasite clearance time was longer (63+/-23 hours) in the AP group than in the HF group (48+/-15 hours) (P = 0.02). The frequency of gastrointestinal adverse events was higher in the AP group. No noteworthy electrocardiographic changes were observed, particularly in the QTc interval. The AP combination appears to be a valuable alternative treatment in non-immune adults.
