Clear cell epithelial-myoepithelial carcinoma arising in pleomorphic adenoma of the lacrimal gland.

BACKGROUND: A 63-year-old man had an 8-year history of painless proptosis, which had noticeably increased over the last 2 months. A mass was palpable in the left lateral canthus. Computed tomographic studies showed a globular mass with small foci of calcification involving the lacrimal gland. After an incisional biopsy, a histologic diagnosis of clear cell epithelial-myoepithelial carcinoma was made and an orbital exenteration was performed. FINDINGS: Results of histologic examination of the mass showed a partially encapsulated, clear cell epithelial-myoepithelial carcinoma with an associated pleomorphic adenoma (benign mixed tumor). Immunohistochemical studies disclosed strong immunoreactivity to cytokeratin (AE1/AE3), epithelial membrane antigen, S-100 protein, and alpha-actin. CONCLUSION: Although a clear cell myoepithelial carcinoma rarely has been reported in association with a pleomorphic adenoma of the submandibular gland, to the authors' knowledge, this combination has never been reported in the lacrimal gland, nor has a clear cell epithelial-myoepithelial carcinoma ever been reported in this anatomic location. The differential diagnosis of lesions with prominent clear cells involving the lacrimal gland is extensive and includes clear cell variants of acinic cell carcinoma and oncocytoma, mucoepidermoid carcinoma, and others.

Angiolymphoid hyperplasia with eosinophilia (Kimura's disease) of the orbit.

A 54-year-old man underwent enucleation of the right eye in 1975 for a malignant melanoma of the choroid, with placement of an Iowa orbital implant. He presented in 1985, with the clinical appearance of a late orbital recurrence of the choroidal malignant melanoma. However, histologic examination revealed the orbital mass to be angiolymphoid hyperplasia with eosinophilia (Kimura's disease). Only six cases of orbital involvement, proved by pathologic studies, have been published in the ophthalmic English literature. This case represented an unusual presentation of a rare disorder that involved the orbit and was probably related to the orbital implant.

The "corncrib" repair of senile entropion.

A new method of senile entropion repair has been described. The method corrects the entropion by affecting those conditions which create an entropion: enophthalmos, lid laxity, preseptal orbicularis movement, and retractor weakness. A 3.4% recurrence rate in 58 lids followed an average 17 months was found. This procedure is simple and applicable to all senile entropion patients.

Orbital mass as a complication of contact lens wear.

A 26-year-old woman, who was a hard contact lens wearer, had a cystic mass in the right orbit that was increasing in size. Although initial ultrasound evaluation was negative for a foreign body, a history of trauma to the right orbit, two years prior to examination, and clinical findings suggested a retained migrated contact lens with a secondary orbital cyst. Surgical exploration revealed the contact lens embedded in the supratarsal space with an associated sterile, inflammatory intraorbital cyst.
