[Mast cells in chronic gastritis of children].

Mast cell involvement in chronic gastritis in children was analyzed. 25 children with normal mucosa (controls), 20 children with chronic gastritis and 28 children with chronic gastritis and infected Helicobacter pylori were included to the study. Bioptic material from antrum and corpus of the stomach were stained with toluidine blue and anti-human mast cell tryptase to evaluate mast cell density. Changes in mast cells number were also estimated in 7 children before and after successful eradication of Helicobacter pylori infection. Mast cell density was significantly greater in children with chronic gastritis with or without Helicobacter pylori infection when compared to the controls. Mast cell degranulation was demonstrated by electron microscopy in children with chronic gastritis and infected Helicobacter pylori. Mast cell through it's numerous mediators may play a key role in chronic gastritis especially when Helicobacter pylori infection is present.

Natural course of sensitization in children: follow-up study from birth to 6 years of age, I. Evaluation of total serum IgE and specific IgE antibodies with regard to atopic family history.

The aim of the study was to evaluate the total serum IgE level and specific IgE antibody responses to alimentary and environmental allergens in the birth cohort of Polish children followed-up from birth to 6 years of age with regard to atopic family history. To study enrolled 52 children. Every child underwent physical examination and in vitro allergy testing at birth, in 6, 12 months of life and in 6 years of age. The mean total serum IgE level was higher in children with atopic family history compared to the children without genetic propensity in every time point, but difference was statistically significant only in six years of age. The highest incidence of positive RAST tests to food allergens was observed in one year of age, mainly in children with atopic family history (36.5%) compared to those without such a history (23.6%). By the age of six years, sensitization to food allergens was observed less frequent in both of the study groups (17.2% and 15%, respectively), whereas the sensitization rates to inhalant allergens were about 53% in tested children with atopic family history compared to 19% in children without such a history.

New cytokine dressings. I. Kinetics of the in vitro rhG-CSF, rhGM-CSF, and rhEGF release from the dressings.

Wound repair-stimulatory activities of various cytokines and growth factors depend on successful delivery of these factors to the injured sites. Here were present the design and preparation of the new collagen- and polyurethane-based dressings containing the recombinant human cytokines-rhG-CSF, rhGM-CSF or rhEGF. To test the efficacy of the retrieval of the incorporated cytokines, their controlled release from the dressings was carried out over three consecutive days using polyurethane sponge as a collector of the extracts. The maximum quantities of the released rhG-CSF, rhGM-CSF and rhEGF reached approximately 25, 50, and 10%, respectively, of the total cytokine contents of the dressings, as assessed by the specific ELISA tests. These data indicate that collagen- and polyurethane dressings containing rhGM-CSF and rhG/CSF may serve as effective tools for the topical delivery of cytokines to wounded tissues.

[Local application of G-CSF, GM-CSF and EGF in treatment of wounds].

G-CSF, GM-CSF and EGF play an important role in wound healing as components of the cytokine network which regulates cooperation of cells in the repair processes. The first two cytokines have been registered as hematopoietic drugs under the names Neupogen (Roche) and Leucomax (Sandoz). Both G-CSF and GM-CSF stimulate phagocytosis in maturated leucocytes and enhance proliferation of endothelial cells. G-CSF, GM-CSF and EGF which is known as mitogenic agent were applied as topical drugs for wound care; the application resulted in acceleration of wound healing. Preliminary suggestions as to the topical dosage of the cytokines in the treatment of wounds are put forward.

The gastrointestinal mast cell in health and disease.

Mast cells are metachromatic cells found widely throughout the body. In gastrointestinal tract they reside particularly in mucosa having close contact with external environment. Their certain role in health and disease remains unclear. Mast cells seem to be involved in lots of allergenic and non-allergenic inflammatory events taking place throughout the gastrointestinal tract including IgE-dependent hypersensitivity reaction, gastritis with or without Helicobacter pylori infection, Crohn's disease, ulcerative colitis, irritable bowel syndrome. Mast cell involvement in certain inflammatory processes in gastrointestinal tract were reviewed.

[Frequency of giardiasis in children with chronic abdominal pain coming from North-East Poland].

Chronic or recurrent abdominal pain constitutes a serious diagnostic and therapeutic problem in childhood. The gastrointestinal, biliary tract or renal lesions often produce the chronic abdominal pain in children. Among causative factors of the chronic abdominal pain the infection with Giardia lamblia is more and more often taken into account. The sample of 112 patients hospitalized during the period from 1992 to 1993 in two departments of the University School of Medicine in Białystok and in three pediatric wards of the Municipal Hospital in Białystok, was assessed. In all children the duodenal aspiration was performed. The following questions were evaluated: 1. What is the frequency of giardiasis in children coming from north-east Poland and presenting chronic abdominal pain? 2. Is there any specific spectrum of symptoms of giardiasis in children? Based on the results of the duodenal fluid examination, the infection with G. lamblia was diagnosed in 77 (67.75%) children. The analysis of the clinical symptoms indicated that only failure to thrive and recurrent episodes of the loose stools were significantly more frequent in infected children compared with uninfected. The frequency of the other symptoms like nausea, headache, vomiting, constipation, fever or allergic skin lesions was similar in both study groups.

Release of antibiotics from collagen dressing.

Our new collagen dressing has been developed recently. Three types (A, B, and C) of the dressing were prepared in this study. Each type contained bacitracin, neomycin or colistin. The antibiotic was input into: i. collagen sponge (CS)--type A, ii. layer of limited hydrophobicity (LLH)--type B, and iii. into both CS and LLH layers--type C. The final concentration of the antibiotic that resulted from the loading level was 2 mg/cm2 for the dressings of type A and B and 4 mg/cm2 for the dressing of type C. The antibiotics were then extracted from the pieces of dressings for two days through dialysis membrane. Susceptibility of 54 bacterial strains (S. aureus, P. aeruginosa, and Acinetobacter) isolated from burn wounds were tested to the three antibiotics used for preparation of the dressings. The results of the study evidenced that efficiency of released of antibiotics into the extracts depended on the kind of antibiotic and on the type of dressing. The concentration of the antibiotics proved to be much higher than MIC90 values of the bacterial isolates tested in respect to their susceptibility. The dressing containing mixture of the three antibiotics in two layers--CS and LLH is now considered as potentially effective for care of infected wounds. It may be useful for the treatment of infected wounds or for profilaxis of contaminated wounds, ensuring: i. sufficient antimicrobial activity in wound, and ii. optimal wound environment for the presence of collagenic biomaterial on the damaged tissue.

Cord blood IgE levels as a predictive value of the atopic disease in early infancy a review article.

The article reviews the recent opinions concerning the predictive value of high cord blood IgE (cIgE) concentrations in the development of atopic disease in infancy and childhood. The results of data from different scientific centers remain still controversial. There is a general agreement that a high neonatal IgE concentration is connected with later allergic disease but low positive predictive value (PPV) of elevated cIgE makes cIgE determinations unuseful for general screening test.

The influence of tobacco parental smoking on serum IgE level of their offspring.

This study was designed to evaluate the influence of passive smoking on serum IgE level and on incidence of atopic diseases in infants. Eighty two infants and their families were studied. The cigarette smoking was recorded in 28% of mothers and in 62% of fathers. Both parents were smokers in 20 families (24%). An elevated total serum IgE level was demonstrated significantly more frequent in the children of smoking mothers (p < 0.05). Paternal smoking did not influence neither serum IgE level nor incidence of atopic diseases.

The incidence and clinical manifestation of food allergy in unselected Polish infants: follow-up from birth to one year of age.

This study was intended to evaluate the incidence of atopic diseases and spectrum of clinical allergic manifestation in Polish infants from North-East region of Poland in relation to genetic hereditance. The infants included in the study were unselected, born consecutively during the first three months of 1993. It was demonstrated that 39% of infants developed an atopic disease during first year of life, 70% in high-risk of allergy group and 18% in low-risk of allergy group. The difference was statistically significant (p < 0.0005). The common atopic manifestation in children was atopic dermatitis. It was recorded in 70% of babies with and in 12% of babies without a family history of atopy.

[The evaluation of nonspecific immunoglobulin E in cord blood of newborns in relation to maternal cow's milk allergen intake during pregnancy].

The relationship between maternal cow's milk allergen intake and cord blood IgE levels in newborns was assessed. Pregnant women were allocated to three groups in regard to amounts of cow's milk intake during pregnancy: high-, low-dose and without any changes. The mean nonspecific cord blood IgE level, birth weight and length of the newborns were calculated in each of the groups. No statistical difference between amounts of the milk allergen intake by pregnant women and cord blood IgE levels was found. Maternal food-avoidance diets instituted in pregnancy significantly affected a decrease in infant birth weight (p < 0.05).

[Early central nervous system complications following purulent meningitis and encephalitis in children from 1 month to 2 years of age according to the etiological factor].

Open anterior fontanel cerebral ultrasonography was performed in 113 children aged between 1 month and 2 years, infants with purulent meningitis and encephalitis. It revealed early CNS complications in 50 patients (44.2%). The treatment protocol was adjusted for different types of complications. All children were discharged home and referred for further care to paediatricians and neurologists. Transfontanel ultrasonography is of great value for evaluating different types of complications following CNS infections.

[A case of encephalitis of mixed Coxsackievirus B4 and rubella viral etiology in a 9-year old boy].

A nine-old boy was presented with encephalitis of mixed viral etiology Coxsackie B4 and rubella virus. The beginning of disease was enough sudden with convulsion and lost of consciousness. Further development of disease mild-the patients was discharged home in good general condition but with nervous super-excitability symptoms. It was the first case of children's mixed viral encephalitis recognized and treated in our Clinic.

[The correlation between levels of IgE in cord blood in newborn infants and a family history of allergy].

The cord blood IgE level was assayed in 110 unselected newborn infants during the period from Jan. 15-March 24, 1993. Serum IgE levels ranged between 0.19-2.8 IU/ml with a median value of 0.34 IU/ml. The group of newborn was subdivided into two groups based on family history of allergy: high risk and low risk. The geometric means, range and median values of cord blood IgE levels were calculated in each of these groups. The median value of the cord blood IgE level was significantly higher in the high risk group (0.40 IU/ml) than in the low risk group (0.34 IU/ml) (p < 0.005).

[Effect of smoking in the families of newborn infants on levels of immunoglobulin E in cord blood].

The influence of active and passive parental cigarette smoking on cord blood IgE levels of their offspring was assessed. The geometric means of cord blood IgE levels in 110 of newborn infants of smoking and nonsmoking mothers were calculated in the entire analysed group and in families with a positive family history of atopy. The results were not statistically significant for the entire analysed group. The higher cord blood IgE concentrations in infants of nonsmoking mothers in the families with a positive family history of atopy were due to the influence of a genetic factor on IgE synthesis by the fetus.

[Adverse events following immunization: AEFI in 17 children between 0 and 2 years of age].

Adverse Events Following Immunization (AEFI) are disadvantageous side effects of preventive vaccination. In 1993 we found 17 cases of AEFI out of 1440 children between 0 and 2 years of age who had received BCG, diphtheria-tetanus-pertussis, measles or poliomyelitis vaccine. They were classified as reactions in 14 children (0.9%) or complications in 3 children (0.2%). Twelve adverse reactions followed DTP vaccination (0.8%), two followed BCG vaccination (0.14%), another two measles vaccination (0.14%) and one followed poliomyelitis vaccination (0.07%). Both generalized and local symptoms were present and they regressed with no further complications. Two children who had received BCG were noted to have a deeply placed abscess at the injection site remaining scar as well as axillary, submandibular and cervical lymph nodes enlargement within 6 months. In a 3 months old child, after the first injection of DTP vaccine, convulsions and consciousness disorder occurred. Transfontanel ultrasonography revealed intraventricular haemorrhage. After one year of intensive neurological care child's health state was improved. In spite of using still more and more safe vaccines none of them is the ideal one--the one with no adverse events following vaccination. Vaccination technics, distribution and storage of vaccines are to be improved which may result in decrease number of AEFI.
