Nephrogenic adenoma of the urinary tract: a review of the microscopic appearance of 80 cases with emphasis on unusual features.

A detailed review of the microscopic features of 80 nephrogenic adenomas of the urinary tract was undertaken. The lesions occurred in patients who ranged in age from 15 to 94 (average, 52) years. There was a 2:1 male/female ratio. Fifty-five percent of the lesions occurred in the urinary bladder, 41% in the urethra, and 4% in the ureter. The most common microscopic pattern, present in 96% of the cases, was tubular. The tubules were usually small, hollow, and round, but some were solid and occasionally elongated. Their arrangement varied from orderly, sometimes laminar, to pseudoinfiltrative. A basement membrane was appreciable around the tubules in 25% of the cases but was rarely prominent. In two prostatic urethral cases tubules involved the fibromuscular prostatic stroma. A cystic pattern was seen in 72% of the cases. The tubules and cysts most commonly contained eosinophilic secretion but in 25% of the cases the secretion was basophilic. In 12% of the cases some of the tubules were tiny and when they contained basophilic secretion occasionally mimicked signet-ring cells. The third most frequent pattern, seen in 65% of the cases, was papillary to polypoid. Edematous polyps were commonest but thin, delicate filiform papillae were present in 10% of the cases. A complex branching pattern with prominent budding of small papillae was seen in three cases. The papillary-polypoid pattern was usually associated with a tubular component but in three cases it was pure. The final pattern, present in 14% of the cases, was diffuse, but it was prominent in only one case.(ABSTRACT TRUNCATED AT 250 WORDS)

Uterine papillary serous adenocarcinoma. A 10-case study of p53 and c-erbB-2 expression and DNA content.

BACKGROUND: Uterine papillary serous adenocarcinoma (UPSA) is a highly aggressive neoplasm with a great tendency for dissemination. p53 and c-erbB-2 immunoreactivity and DNA ploidy are considered to be indicators of prognosis for endometrial carcinomas. METHODS: Ten cases of patients with UPSA are reported. An attempt to relate pathologic findings with immunohistochemical stains for p53- and c-erbB-2-associated proteins, ploidy, and survival was made. RESULTS: Three patients were classified as having Stage I; three, Stage II; two, Stage III; and two, Stage IV. Myometrial invasion was present with nine tumors and involved over 50% of the myometrial thickness in five. Uterine lymph vessel invasion was detected in seven cases. Peritoneal spread occurred in six patients. Overexpression of p53 was observed in six tumors, immunoreactivity for c-erbB-2 in four, and aneuploidy in seven. However, only peritoneal spread correlated significantly with survival (P < 0.005). CONCLUSIONS: UPSA is a tumor with a high metastatic potential that exhibits immunoreactivity for p53 and c-erbB-2 and aneuploidy more often than that reported for conventional endometrioid adenocarcinomas.

Stromal tumours of the gastrointestinal tract: a clinicopathological and ploidy analysis of 33 cases.

The clinicopathological and DNA flow cytometric data of 33 patients with stromal tumours of the gastrointestinal tract (STGIT) were analysed to select pathological features of prognostic value. Tumours had been previously classified as benign (21 cases) or malignant (12 cases). Data relating to poor prognosis statistically were local invasion, pathological grade, size greater than 10 cm, mitotic index (MI) and necrosis. Pathological grade was related to local invasion. Aneuploidy did not correlate with poor survival although a common trend was detected between both. DNA content may help to predict prognosis of STGIT, but its real value has not yet been clearly established. Currently, stage (invasion), size, MI and pathological grade remain the most useful prognostic factors.

Development of post-traumatic cysts in the spinal cord of rats-subjected to severe spinal cord contusion.

To study the development of post-traumatic spinal cord (SC) cysts, and their fine anatomic characteristics, rats were subjected to severe SC contusion. Specimens were analyzed from day 1 to 1 year post-injury. Using conventional light, and transmission and scanning electron microscopy, three stages were typified, namely: necrosis, repair, and stability. The final cell composition and thickness of the cyst walls were not uniform. Astrocytes, fibroblasts, ependymal cells, and collagen fibers were the main constituents. Chronic inflammatory cells were also observed. The neuropathologic characterization of posttraumatic SC cysts could be useful in planning strategies for SC reconstruction at different times post-injury.

Malignant neuroectodermal tumour of the chest wall (Askin tumour): CT and MR findings in eight patients.

Malignant neuroectodermal tumour of the chest wall (Askin tumour) is an uncommon malignant neoplasm affecting young adults. We retrospectively reviewed the clinical and pathological information and radiologic studies in our archives on eight cases of this tumour. All cases were pathologically proven. Plain films, CT scans and radionuclide images were available in all cases. MR images were also available in two patients. Although rare, Askin tumour should be included in the differential diagnosis when a chest wall or thoracic paraspinal mass is identified in a young individual.

Small cell carcinoma of the ovary, hypercalcemic type. A clinicopathological analysis of 150 cases.

The clinical and pathological features of 150 cases of ovarian small cell carcinoma of the hypercalcemic type are described. The patients ranged from 9 to 43 (average 23.9) years of age. The serum calcium level was known to be elevated in 49 of the 79 patients (62%) whose preoperative calcium levels were measured. Four of these patients had symptoms of hypercalcemia, and one of them had undergone neck exploration with negative results before the ovarian tumor was discovered. At laparotomy the tumor was unilateral in 148 cases (99%). Extraovarian spread was present in approximately half the cases. The tumors ranged from 6 to 26 (average 15.3) cm in greatest dimension. Microscopic examination disclosed various patterns, the most common of which was diffuse sheets of cells punctured by variable numbers of follicle-like spaces; the tumor cells also grew in nests, cords, clusters, and singly. The follicle-like spaces, which were present in 80% of the cases, contained fluid that was almost always eosinophilic and rarely basophilic. Glands or cysts lined by mucinous epithelial cells were present in 12% of the neoplasms. The neoplastic cells were typically small and round with hyperchromatic nuclei and brisk mitotic activity. Fifty percent of the tumors, however, also had a variable component of cells with moderate to abundant amounts of eosinophilic cytoplasm, which sometimes contained large hyaline globules and large nuclei that were typically paler and had more prominent nucleoli than the small cells. Immunohistochemical staining confirmed the epithelial nature of the tumors, as did electron microscopy, which characteristically showed abundant dilated rough endoplasmic reticulum. Five of seven tumors investigated by immunohistochemical staining for parathyroid hormone-related protein showed positive results. All 23 tumors examined by flow cytometry with interpretable results were diploid. Fourteen of 42 patients (33%) with stage IA disease for whom follow-up information is available remained well and free of disease 1-13 (average 5.7) years postsurgery; 23 died of their disease, usually within 2 years; and five had recurrences but were alive at last follow-up. Almost all the patients with tumors of a stage higher than IA died of disease, but one patient with stage IIB disease who received intensive chemotherapy and radiation therapy is alive and apparently free of disease at 7 years. Features in stage IA tumors that appeared to be associated with a more favorable outcome included an age > 30 years, a normal preoperative calcium determination, a tumor size < 10 cm, and an absence of large cells.(ABSTRACT TRUNCATED AT 400 WORDS)

Luteinized adult granulosa cell tumors of the ovary: a report of four cases.

Four luteinized adult granulosa cell tumors of the ovary in women 22-76 (average 53) years of age are described. Two patients presented with postmenopausal bleeding, one with dysfunctional uterine bleeding and one with amenorrhea. The tumors were all stage Ia, ranged from 2 to 11 (average 5.1) cm in greatest dimension, and were typically solid, lobulated, and yellow. On microscopic examination the most striking feature was the abundant eosinophilic cytoplasm of most of the tumor cells, which resembled the granulosa cells of the corpus luteum. The tumor cells typically grew diffusely, but large nodules intersected by scanty fibrous septa were present in all the cases. Additional patterns present focally included cords or ribbons, nests, and thick trabeculae. The tumors had inconspicuous amounts of fibrothecomatous stroma; foci of stromal edema or myxoid change were present in three cases. The tumor cell nuclei were round, of moderate size, and pale, and many of them had nuclear grooves. There was no significant nuclear pleomorphism and the mitotic rate did not exceed 1 per 10 high power fields. Reticulum stains, available in three cases, confirmed the granulosa cell nature of the neoplasms by disclosing scanty intercellular fibrils in most areas. Immunostains, performed in two cases, showed positivity for vimentin in both of them and for cytokeratin, smooth-muscle actin, and S-100 in one. One tumor from a 75-year-old woman was associated with an ectopic decidual reaction in a periovarian adhesion. This patient had focal, severe atypical complex hyperplasia of the endometrium and focal secretory change in the endometrium uninvolved by the hyperplasia.(ABSTRACT TRUNCATED AT 250 WORDS)

Ploidy of 36 stromal tumors of the gastrointestinal tract. A comparative study with flow cytometry and image analysis.

DNA ploidy was investigated by flow cytometry (FC) and image analysis (IA) in paraffin-embedded tissue sections from 36 stromal tumors of the gastrointestinal tract. The results of both techniques were correlated with pathologic features of the tumors and survival. Ten (27.8%) tumors were aneuploid by FC and IA. Most of the diploid tumors were identified by both techniques, but FC appeared to be superior to tissue section IA for identification of aneuploid tumors (25% vs. 13.8%). Aneuploidy by FC correlated with pathologic grade and mitotic index (P < .05), and a trend to short survival was also detected (P < .1). No similar correlation was found by IA. Enlargement and variation of nuclei may explain the discrepancy between FC and IA.

Ovarian carcinomas with choriocarcinomatous differentiation.

BACKGROUND: Ovarian carcinomas may produce human chorionic gonadotropin (HCG) or HCG-like substances and may even contain syncytiotrophoblast cells, but a true choriocarcinomatous component has not been described in these tumors. METHODS: Two cases of poorly differentiated ovarian carcinoma with choriocarcinomatous components are reported. Pathologic findings were correlated with immunohistochemical stains, hormonal effects, and clinical behavior. RESULTS: Each tumor contained a circumscribed, extensively necrotic and hemorrhagic brown nodule. Microscopically, the nodules exhibited a mixture of cytotrophoblast and syncytiotrophoblast. The syncytiotrophoblast capped cytotrophoblast and was strongly positive for beta-HCG. In one of the cases, a transformation zone composed of poorly differentiated carcinoma with occasional beta-HCG-positive cells was observed between a mucinous cystadenoma and the choriocarcinomatous elements. The two cases exhibited activation of the ovarian stroma in the form of condensation and luteinization. Extra-abdominal metastases developed early in both patients and, despite multiagent chemotherapy, they died shortly postoperatively. CONCLUSIONS: Choriocarcinoma may rarely develop as a result of dedifferentiation of common epithelial ovarian tumors. Recognition of choriocarcinomatous components in ovarian carcinomas is important because of its association with aggressive behavior.

[Hypertrophic obstructive myocardiopathy complicated by severe mitral insufficiency secondary to rupture of the chordae tendineae].

A case of suddenly increasing dyspnea in a patient with hypertrophic obstructive cardiomyopathy is reported. Transesophageal echocardiography showed severe mitral regurgitation secondary to ruptured chordae tendineae. Mitral valve replacement was done, correcting both mitral regurgitation and subaortic gradient. Utility of transesophageal echocardiography in diagnosis and therapeutic options are discussed.

Successful surgical management of rupture of the left ventricle and interventricular septum due to blunt chest trauma. Case report.

In a 35-year-old man blunt chest trauma caused rupture of the free wall of the left ventricle and the interventricular septum. Emergency pulse Doppler-2D echocardiography confirmed the clinical suspicion and immediate surgical repair was successfully undertaken. This appears to be the first reported case of survival after surgery for such combined cardiac rupture.

[Portasystemic shunt in the treatment of gastropathy caused by portal hypertension].

Gastropathy by portal hypertension constitutes the second cause of digestive hemorrhage in these patients following esophagogastric varices. Beta-blocker drugs seem efficient in treatment as occasionally does therapeutic endoscopy. The case of a patient with chronic liver disease with upper digestive hemorrhage is presented. The patient did not respond to medical treatment with beta-blockers nor to endoscopy (thermic and sclerosant). Portocaval anastomosis was performed with no posterior hemorrhagic relapse. The use of shunt surgery has been suggested in this pathology given the rarity of its presentation in patients with this type of operation. The satisfactory evolution of the patient seems to confirm this hypothesis.

[Blood donation in the Balearic Islands. Analysis of supply and demand of blood from 1982 to 1987].

BACKGROUND: The supply and consumption of blood products in the autonomous community of the Balearic Islands were evaluated for the years 1982 to 1987 to assess the degree of hemotherapy coverage. METHODS: The registries of blood banks from 1982 to 1987, the clinical records of patients with congenital coagulation deficiencies and the registries of the hospital pharmacies and pharmaceutic distributing agencies were reviewed. RESULTS: The rates of use per product (units) and per one thousand population were as follows: red blood cells 22-25, platelets 0.8-3.2, transfusional plasma 0.5-2.8, and cryoprecipitates less than 0,2. The consumptions of albumin and gammaglobulins were 80 and 7.7 kg per one million population, corresponding to 3.200 and 1.540 I of plasma, respectively. The consumption of factor VIII was 1.3-0.9 international units (IU) per inhabitant, in highly of very highly purified form, corresponding to 18.500-12.857 I of plasma. CONCLUSIONS: The results show that a supply of 25-27 blood donations per one thousand population and per year and with a fractionning of 80%, yield 5.000 I of plasma per one million population. This fulfils the needs of all blood products, except those of highly or very highly purified factor VIII.

[Gastric carcinoma with a lymphoid stroma].

Carcinomas with lymphoid stroma have been described in different locations: breast, uterine cervix, nasopharynx, esophagus and stomach. These neoplasms have a better prognosis probably due to the lymphoid reaction that takes place in the tumoral stroma. For this reason many authors prefer to consider this entity separate from the rest of gastric carcinomas. We present a case of gastric carcinoma with lymphoid stroma and neuroendocrine features. Such association has not been previously described in the Spanish literature.
