Simple and rapid two-step polymerase chain reaction for diagnosis of Pneumocystis carinii infection.

Two amplification steps were made to detect Pneumocystis carinii DNA by polymerase chain reaction (PCR). pAZ102-E and pAZ102-H (standard PCR), pAZ102-L2 (sense), and pAZ102-E (antisense) (two-step PCR) were used as primers. The amplification products were analyzed by ethidium bromide. After the two-step PCR, ethidium bromide detected all samples positive by oligohybridization after one amplification step. Our two-step PCR is a rapid, cost-effective, and clinically suitable method for the detection of P. carinii infection.

Pneumocystis carinii stimulates in vitro production of tumor necrosis factor-alpha by human macrophages.

The ability of Pneumocystis carinii to induce tumor necrosis factor (TNF)-alpha release by monocytes/macrophages from immunocompetent humans was investigated. Monocytes and monocyte-derived macrophages from healthy individuals produced an increased amount of TNF-alpha when exposed to P. carinii cysts obtained from rats with steroid-induced pneumocystosis. The cysts induced increased TNF-alpha production in a dose-dependent manner; baseline TNF-alpha production was restored after addition of an anti-P. carinii hyperimmune serum. Kinetics experiments showed that the secretion of TNF-alpha occurs early and reaches a maximal peak after 8 h. Since TNF-alpha is directly lethal to P. carinii in vitro, it is suggested that the production of this cytokine in response to the cysts may be one of the mechanisms for the control of this parasitic infection.

Detection of antibodies against Echinococcus granulosus major antigens and their subunits by immunoblotting.

An immunoblot assay was tested to evaluate its ability to diagnose human hydatidosis and to analyse the reactivity of hydatid patients' sera with the subunits of the 2 major Echinococcus granulosus antigens (5 and B). In all, 308 sera were examined: 166 sera from patients with clinically diagnosed hydatidosis, 100 sera from healthy control subjects, and 42 sera from patients with diseases other than hydatidosis. The sensitivity of the method was 90%, as compared to 78% with the immunoelectrophoresis/double diffusion test for antigen 5. No reactivity was found with 15 sera from patients with schistosomiasis, 7 sera from patients with trichinellosis, or 20 sera from patients with non-parasitic diseases. Analysis of serum reactivities showed the presence in all positive sera of antibodies directed against the 39 kDa molecule of the antigen 5 complex. A lower reactivity (55% of all hydatid sera) was observed with the subunits of the antigen B complex.

Detection of Pneumocystis carinii in oropharyngeal washings by PCR-SHELA and nested PCR.

Oropharyngeal washings (Ophs) from 27 HIV infected patients (18 with P. carinii pneumonia, PCP, and 9 without PCP) were examined for P. carinii using morphological staining and DNA amplification with PCR-SHELA and nested PCR methods. The comparison of these techniques shows that 1. the amplification of P. carinii DNA is more sensitive than (and as specific as) morphological staining; 2. PCR-SHELA is less sensitive than (and as specific as) nested PCR.

AIDS-Related Opportunistic Infections↗

Typing with ITS regions of P.carinii from AIDS patients with recurrent pneumonia.

To understand the way of reinfection of Pneumocystis carinii we have analyzed the genetic variation at the internal transcribed spacer (ITS) in DNA samples from bronchoalveolar lavage fluid of Italian HIV patients who had multiple episodes of P.carinii pneumonia. The presence of the same and/or a new type in both episodes suggest the possible occurrence of both reactivation of a previously acquired infection and reinfection from an exogenous source. Furthermore the occurrence of two different types in the same episode indicate that a mixed infection is common.

[Macrophages from healthy adults release TNF-alpha after exposure to Pneumocystis carinii of murine origin. Preliminary study].

The ability of Pneumocystis carinii to induce TNF-alpha release by macrophages from adult healthy humans was investigated. Monocytes and monocytes derived macrophages produced an high amount of TNF-alpha when exposed to P. carinii cysts obtained from rats with steroid induced pneumocystosis. TNF-alpha release was P. carinii specific as shown by the inhibition exerted by the anti-P. carinii hyperimmune serum and it was not mediated by putative traces of endotoxin.
