Serum neuron specific enolase: can it be a tumour marker for renal cell carcinoma?

Neuron specific enolase (NSE) is an isoenzyme of the glycolytic enzyme enolase. It is not only a marker for all types of neurons but also for all neuroendocrine or paraneuronal cells and various malignant tumours, even of non-neuroendocrine types. We had studied serum NSE in 25 consecutive patients with renal cell carcinoma (RCC). The study included 10 stage I, 3 stage II, 3 stage III-B and 9 stage IV-B patients. Both pretreatment and posttreatment levels were evaluated. Regardless of stage, overall we observed elevated levels of NSE in 80% (20/25) at diagnosis. After the appropriate treatment, according to the stage, there had been a statistically significant (p < 0.05) decrease in the serum levels in all stage I, II and III-B patients. The posttreatment values were not available for stage IV-B patients because they did not come for follow-up. The preliminary results of our study revealed that serum NSE may be considered as a useful marker in the evaluation and surveillance of RCC.

Augmentation duracystoplasty in neurogenic bladder dysfunction.

BACKGROUND: Augmentation cystoplasty is the treatment of choice for patients with neurogenic bladder dysfunction in cases that are unresponsive to other medical treatment. Although intestinal segments as bladder substitutes are preferred over the other alternatives at present, they are not ideal bladder replacements due to several potential hazards. The purpose of this study was to determine whether or not augmentation duracystoplasty can be an alternative to augmentation enterocystoplasties. METHODS: Ten patients with neurogenic bladder dysfunction unresponsive to conservative measures, underwent augmentation duracystoplasty by using the modified Bramble-Clam technique. The follow-up period was T-28 months. RESULTS: At present, seven of 10 patients are completely continent for with clean intermittent catheterization. The remaining three patients required oral oxybutinin therapy, postoperatively, to achieve continence although lower dosages than those required in the preoperative period. We did not observe any serious pre- or postoperative complications. CONCLUSIONS: Based on these preliminary findings we think that duracystoplasty can be considered as a treatment alternative for hyperreflexic and/or low compliant neurogenic bladders.

Urological evaluation of Behçet patients and the effect of colchicine on fertility.

Behçet's disease is a multisystem disorder in which urogenital involvement mainly consists of genital aphthous ulcers, epididymitis, urethritis and recurrent cystitis. Colchicine is commonly used in these patients for the prevention of arthritic episodes. In this prospective study, 62 male patients under colchicine therapy for Behçet's disease underwent a complete urogenital evaluation. 33 (53.2%) had no urologic complaints. Physical examination revealed genital aphthous ulcers in 42 patients (67.7%) and epididymitis in 5 (8.1%). Analysis of urine specimens demonstrated proteinuria in 54.8% and hematuria in 30.6%. To establish any possible adverse effects of colchicine administration on male fertility, sperm analysis was performed: 23 patients (37.1%) had oligonecrospermia, 2 patients (3.2%) had azoospermia. Penile color flow Doppler sonography revealed arterial insufficiency in 17 patients (27.4%). Other urological findings were interpreted as coincidental. Our results indicate that urologic manifestations of Behçet's disease, and especially the possible adverse effects of colchicine administration on the reproductive potential of the treated patients should be carefully monitored.

Extracorporeal shockwave lithotripsy in anomalous kidneys.

Traditionally, stones in anomalous kidneys have been removed by open or percutaneous surgery. Extracorporeal shockwave lithotripsy (SWL) with the Dornier MPL 9000 lithotripter was performed in seven patients with horseshoe kidneys, four with pelvic ectopic kidneys, and six with malrotated kidneys. Twelve patients (71%) needed repeated treatments. A total of 11 patients (65%) in all the groups were stone free, and four patients had asymptomatic residual fragments no more than 5 mm in diameter. In the remaining two patients, no sign of stone disintegration was observed, and they underwent open surgery. Extracorporeal lithotripsy is the treatment of choice for stones in horseshoe or malrotated kidneys but is not useful for stones in most pelvic kidneys.

Treatment of renal stones in a solitary functioning kidney with extracorporeal shock wave lithotripsy.

Forty extracorporeal shock wave lithotripsy (ESWL) sessions were performed in the treatment of 36 stones in 22 solitary functioning kidneys between October 1990 and October 1991. We inserted a double-J catheter in all patients preoperatively and no serious obstruction such as stone street formation was observed after treatments. Following ESWL therapy, at the end of 3 months follow-up, 14 patients (63.6%) were completely stone free while in 5 patients ESWL therapy was unsuccessful. ESWL became the treatment of choice for stones in solitary kidneys as a result of its highly effective, safe and encouraging outcome.

Definition and treatment of chordee without hypospadias: a report of 5 cases.

The controversies in chordee without hypospadias are mostly focussed on the definition and treatment of type I and type IV chordee. In this respect we believe that surgical exploration is necessary to make a precise classification leading to the most appropriate therapy. We present 5 cases, 3 of which had type I chordee, 1 had type II and 1 had type III. In the 2 cases with type I chordee, type IV chordee coexisted. Although all the patients have had satisfactory cosmetic results, 3 of them should be reevaluated after puberty since they are prepubertal yet.

Long-term follow-up results of the MAGPI (meatal advancement and glanuloplasty) operations in distal hypospadias.

Between 1985 and 1989, 54 boys underwent a MAGPI procedure. Excellent cosmetic and functional results were achieved in 94% of our cases in the early period. In order to assess the long-term results of this procedure, 41 boys attended long-term re-evaluation between 6 and 40 (mean 29) months postoperatively. MAGPI was a very satisfactory procedure for micturition function. We observed in 22% of cases partial and in 15% complete regression. On the long term, excellent cosmetic results were achieved in all glanular cases but in only 65% of coronal and 15% of subcoronal cases. We believe that the MAGPI procedure is not a suitable operation for subcoronal hypospadias.

[Anterior body ostectomy for mandibular prognathism].

The mandibular body ostectomy is indicated for the correction of select cases of; Mandibular prognathism, Class III open-bite deformity, Anterior crossbite in the adult. The anterior body ostectomy is indicated primarily in selected cases of mandibular prognathism, class III malocclusion when the posterior teeth are not in crossbite or the crossbite is dental in nature and can readily be resolved with conventional orthodontics. As with the correction of other dentofacial deformities, carefully done model surgery and a cephalometric prediction tracing are imperative in planning the anterior body ostectomy. In this paper, a case of mandibular prognathism which is corrected by the technique of anterior body ostectomy is presented.

[Evaluation of skeletal structures in individuals with malocclusion].

This study was performed on 50 female and 45 male subjects with orthodontic malocclusions. The aim of the study was to investigate the relationship between sagittal and vertical growth patterns of the cranio-facial complex. The sample was divided into six groups, (class I, II, III type and neutral, deep and open type) according to the ANB angle and Jarabak rate. As a result, the anterior and posterior facial heights were significantly smaller in the girls than in the boys. The relationship between the sagittal jaw interrelation and different vertical growth patterns of the face was not significant. It has also found that the discrepancy of the sagittal jaw relation was mainly caused by protrusive or retrusive position of the mandible relative to the cranial base.

Microscopical splint epididymovasostomy.

Splint epididymovasostomy is a new approach in operative andrology. In 23 rats, a microscopical splint epididymovasostomy was done in two groups 40 days after sterilization using 5/0 polyglactic acid and 5/0 plain-catgut as a splint. 28 anastomoses could be evaluated. 3 months later, in 2 of the 16 animals (12.5%), spermatozoa were detected in the ejaculate after electro-ejaculation. In six of the specimens (27.3%), the methylene blue given from the vas deferens passed into the epididymis. The patency of these was also proved histopathologically. The catgut splints were absorbed within 3 months. Microscopical splint epididymovasostomy shows that this method can lead to a free passage of the spermatozoa.
