Histologic aspects of the bone and soft tissues surrounding three titanium non-submerged plasma-sprayed implants retrieved at autopsy: a case report.

The authors report the histological features found around three non-submerged titanium plasma-sprayed implants retrieved, after a 10-month loading period, from an autopsy case. At the time of implant insertion, the clinician had noted a wide vestibular dehiscence of the central implant, and it was decided to use a bioabsorbable membrane for guided bone regeneration in this area. After specimen processing, it was possible at low magnification to observe that in the most vestibular slides, the central implant was almost completely surrounded by connective tissue, while in the most lingual slides, the quantity of bone around the implant tended to increase. The other two implants had a bone-implant contact percentage of about 60%. Only in a few areas was mineralized bone in direct contact with the metal surface, while around the major portion of the implant perimeter a layer of unmineralized, red-stained, osteoid material was present. No inflammatory infiltrate was present in the epithelium and in the supracrestal connective tissues. The fibers of this tissue had a different orientation: in the most coronal portion of the implants (smooth surface), they tended to run parallel implant's surface, while in the most apical region (plasma-sprayed surface), they tended to be arranged in a perpendicular fashion. These results, in man, were strikingly similar to those previously reported in dogs and monkeys.

Metastatic renal cell carcinoma involving Wharton's duct: a case report.

This article describes a case of renal cell carcinoma that metastasized to the right Wharton's duct of a 73-year-old man. Metastases of renal cell carcinoma to the oral tissues are rare. Involvement of salivary gland tissue appears to be exceedingly rare, and a metastatic focus localized only in the Wharton's duct never has been reported in the literature.

Aged

[Benign lymphoepithelial cysts of the parotid in a patient with HIV infection].

We report on a HIV-positive patient presenting bilateral enlargement of the parotid gland. The ultrasound examination demonstrated multiple lesions with cystic aspect. A partial surgical excision of the parotid gland was performed, and histologic examination confirmed the diagnosis of a cystic benign lymphoepithelial lesion. The specimens which were tested for Epstein-Barr virus and cytomegalovirus by in situ hybridization proved negative. The clinical and pathologic features of this unusual salivary gland lesion which affects HIV-positive patients are discussed.

Adult

[Multiple osteomas of the maxillary sinus. Presentation of a case].

This case report concerns a case of multiple osteomas of the maxillary sinus which probably arise from flogosis and discusses the clinical features. The review of the literature on the histogenesis of these rare lesions remains controversial and it is still difficult to formulate a precise definition.

Facial Pain

[Post extraction osteomyelitis. Report of a case].

A case of osteomyelitis with a typical sequestrum of the alveolar bone, occurred three months after the extraction of the corresponding tooth, is reported. Although the pathologic process is related with the extraction, it was not possible to clear up any well-known pathogenetic mechanism which are described in the literature.

Adult

[Maxillofacial lesions in road accidents].

Authors have compared the cases of maxillo-facial fractures that have been admitted in the Maxillo-Facial department during a six months period before and after the mandatory use of helmets; that is from January through June in the years 1986 and 1987. A decrease in the number of injured (- 13.3%) and also a rate change was noted: the motorcycle drivers lost a 20.1%, while the adults who drove the motorbikes increased; a raise was found in the bicycle too (+ 8.7%). The lesions of the mandibular increased from 36.7% to 46.1% due to the protection offered to the maxilla by the helmets.

Giant cell lesion of the jaw. Case report.

A case is presented of a male patient affected by a giant cell lesion of the jaw, which had two recurrences in 8 years. Histologically, the lesion appeared to be composed of giant cells and mononuclear cells. Histoenzymatic study demonstrated acid phosphatase in both types of cells, and beta-glucuronidase in giant cells only. In some nuclei of giant cells, ultrastructural investigation showed filaments or microtubular structures of variable length, with irregular transverse periodicity, in addition to other expected findings. These characteristic features, found in giant cells of some giant cell tumors of the long bones, have never before been reported in a giant cell lesion of the jaw. The results are considered in order to assess the diagnosis, and the pathologic profiles of giant cell reparative granuloma, and of giant cell tumor are critically discussed.

Diagnosis, Differential

[Solitary bone cyst. A case of dual mandibular and humeral location].

Among cystic lesions of maxilla the solitary bone cyst always raises diagnostic and etiopathogenic problems. Typically observed as single lesions in maxilla, reports have been published of multiple localizations. Similitudes with solitary cyst of long bones have even been underlined, but no reports exist in the literature of the double localization: maxillary and extra-maxillary. A young man presented mandibular and humeral lesions possessing characteristics of solitary bone cysts.
