[A pancreatic pseudocyst in the liver].

A large cystic formation in the liver of 43-years old woman was identified as pancreatic pseudocyst, using histologic examination during laparotomy. The pancreas was found quite normal without any relation to the pseudocyst. Therefore it seems to be acceptable such a pathogenetic reflection, that the pseudocyst developed as a result of pathologic changes in heterotopic pancreatic tissue (embryonic malformation). The pseudocyst was partly resected and the large cavity in the liver was drained into the small intestine using Roux loop. After 8 months uncomplicated follow-up of the patient, the CT picture of her liver was nearly normal.

Kidney transplantation and malignant diseases.

Basing on three types of questionnaires, the authors summarized the incidence of all malignant diseases in kidney transplant patients in CSFR. The total number of registered tumours was 34. Previously to the transplantation, two patients with malignancies were found, another one was transplanted for a kidney tumour and 31 patients manifested malignant diseases in various time intervals following the transplantation. This number is five times greater than the incidence of tumours in the standard population. At classical immunosuppression, the mean time interval between transplantation and tumour formation is 48 months; at cyclosporine application, it decreases to 19 months. The most frequent sites of tumour formation are the skin and the kidney. The early diagnosis of tumours and the surgical treatment will increase the survival period up to 58 months. The systematic oncologic follow-up and preventive controls of transplant patients represent one of the possibilities of the prolongation of the patient's life with the functional graft.

[Acute states in colorectal surgery].

The authors evaluate their own group of 80 patients treated in the acute stage of colorectal disease during the period between 1980-1987 at the First Surgical Clinic, Faculty Hospital with Policlinic, Olomouc. Fifty-five patients treated by the classical three-stage operation with a lethality of 51%, 25 patients treated by primary resection with a lethality of 31%. The encouraging immediate and long-term results of rationally indicated acute resections are not in favour of three-stage operations.

Our experience with surgical management of myasthenia gravis.

The authors' own experience obtained from surgical management of the myasthenia gravis in the First Department of Surgery, Teaching Hospital, Palacký University in Olomouc, within 1963 to 1985 are analyzed. The total of 50 operations on the thymus were performed in this period. Five myasthenics were operated on. Four of them had the thymoma associated with MG, one had no thymoma either prior to or during the surgery. Once the MG was associated with a benign thymoma, three times with a malignant thymoma. All the thymomas could be radically extirpated. The late results of the treatment are given. Some more recent examination methods are evaluated. The conclusion sums up the results of surgical MG management, indication to operation and its tactics stressing the necessity of complex multidisciplinary care of MG patients.
