Advantages and pitfalls of ultrasound in the diagnosis of breast cancer.

Ultrasound has become more and more widespread in the diagnosis of breast disease. However, opinions still diverge concerning its exact place in the diagnosis of breast cancer. An understanding and awareness of the principal pitfalls and limitations of this modality is essential for its optimum and responsible usage. An analysis of four interesting cases illustrates the benefits and limitations of this technique.

Specificity of ultrasonography in the diagnosis of benign breast masses.

Ultrasound has become increasingly important in the management of benign breast disease. The main reasons are its ability to distinguish between cystic and solid lesions, its special advantages in examining the dense breast, and the absence of potential radiation hazard. This paper evaluates the specificity of automated ultrasound in the diagnosis of benign lesions. The usefulness of the signs established for the diagnosis of benign masses is analyzed. Atypical lesions are discussed and the role of ultrasonography in the differential diagnosis of benign lesions is reviewed.

Subcutaneous emphysema of the abdominal wall from diverticulitis with necrotizing fasciitis.

The case history of a patient with subcutaneous emphysema of the abdomen from sigmoid diverticulitis is presented. The mechanisms for developing abdominal wall emphysema are reviewed. Thus when the gas originates from the gastrointestinal tract, mechanical factors are mainly responsible for this phenomenon. These inculde a direct communication through a colonocutaneous fistula. Gas may, however, also be spread into the abdominal wall by high intraintestinal pressures. Infection with gas-producing organisms may be associated with subcutaneous emphysema. This is particularly true when the emphysema develops 48 h or longer after onset of symptoms. Infection, however, is not the major cause for gas in these tissues.
