In 23 stable asthmatic patients sensitivity to inhaled beclomethasone dipropionate (BDP) was demonstrated during single blind gradual tapering off of BDP. After a 2 week stabilization period the patients were randomly allocated to receive either 4 puffs of 50 micrograms BDP twice daily (AM/PM) or 8 puffs of 50 micrograms once daily (AM). After 4 weeks the patients were crossed over. Fifteen patients preferred the twice daily regimen, 5 preferred once daily and 3 had no preference (p less than 0.05). During the once daily dosing regimen a decrease in morning and evening peak expiratory flow (p less than 0.05) and an increase in nighttime and daytime symptom score (p less than 0.05) was found, while the number of inhaled beta 2-agonist doses did not change significantly (p = 0.05). It is concluded that a twice daily regimen controls the asthmatic symptoms significantly better than a once daily regimen.

This study from Danish general practice gives figures about the simultaneous prevalence of asthma and allergic rhinitis and the order of onset among 7662 patients, who during 1 year consulted for one or both of these diseases. Twenty-eight percent of patients with asthma consulted because they also had allergic rhinitis, and 17% of patients with allergic rhinitis consulted because they also had asthma. Age- and sex-distributions are presented. In 25% of patients with both diseases the onset of both diseases occurred within the same year, while in 35% the onset of asthma occurred first and in 40% allergic rhinitis. Among patients with both diseases, who did not have onset of both within the same year, more than 75% of them had onset of one disease within 2 years of the other.
The methods used in a multipractice study about asthma and allergic rhinitis in 1977-78 is described. In total 153 general practices took part comprising 271 physicians. The total practice population was 450.271 persons as calculated from the number of persons insured in groups 1 and 2 of the public health insurance system plus their children. The general practitioners were for the most part younger than the average Danish general practitioners. The age of the practice population was younger than the age of the total Danish population. The region of Zealand was overrepresented, and Copenhagen underrepresented. These differences were, on the whole, relatively small. The course of recording was judged by comparing the doctors' self-estimated recording activity with their actual number of consultations per patient. This self-estimated recording rate was a useful indicator of recording quality. The recording rate seemed to be reasonably good in those two third of the practices, which estimated themselves to have recorded more than 90% of the consultations intended to be recorded. The interpractice variability of recording in multipractice studies is an important methodological problem of general practice research. In this study interaction was found with some practice characteristics, but major sources of variability remain unexplained. In spite of the difficulties mentioned conditions for collecting information about prevalence and course of diseases in general practice in Denmark are very favourable in comparison with most other countries. The reason for that is the very large proportion of the population, which is firmly registered with particular practices, the public personal identification number, the access to relevant registers, and the feasibility of handling substantial amounts of data.
The distribution by month of birth among 7796 patients, who consulted Danish general practitioners during a one year period 1977-78 because of asthma and/or allergic rhinitis is presented. Among allergic rhinitis patients births in March and April were significantly overrepresented, and July underrepresented. As a whole February-May were overrepresented and July-January underrepresented. The distribution of month of birth of asthma patients did not differ from the distribution of the total Danish population. Patients suffering from both asthma and allergic rhinitis presented a pattern similar to that of allergic rhinitis patients. Families with a high prevalence of allergy may take advantage of this knowledge when planning child births, as the findings may be explained partly by exposure to pollen during infancy.

This study from Danish general practice with the participation of 131 practices covering a population of 450,000 persons assesses all consultations concerning asthma during a 1-year period from June 1977. Asthma was defined as paroxysmal bouts of dyspnoea of an expiratory type. Results are presented from three groups of practices differing in their self-estimated rate of reporting. A total of seven to nine per 1000 of the population contacted a general practice during the study year (men: 7-10, women: 6-8). The prevalence may be described by a two-peaked curve with maxima at ages 5-14 years and 50-70 years. In the former group the prevalence rate among boys is double that of girls. In the later group asthma is more common among women until the age of 55, after that age it is more common among men. More than half of the patients consulted with symptoms during the study year. The isle of Fyn had a lower prevalence rate with and without symptoms that all other regions. The consultation rate was 5.7 per asthma patient per year. In about 25-30% of these consultations patients presented with symptoms. The isle of Fyn had the lowest consultation rate.
The prevalence rate of allergic rhinitis and the consultation rate caused by this disease were studied in 131 general practices covering a population of 450,000 persons in Denmark. During a 1-year period starting June 1977 all consultations with or without symptoms were recorded. Allergic rhinitis was defined as paroxysmal sneezing and rhinorrhoea with or without conjunctivitis, and without signs of infection. Results are presented from three groups of practices classified by self-estimated completeness of reporting. 11--15 per thousand of the total population contacted a practice during the 1-year period (males: 12--17%, females: 10--13%). The highest prevalence rate occurred at 10--19 years of age, and allergic rhinitis was common from five to 44 years of age. The prevalence rate was higher among males up to the age of 30, after which age it was a little higher among females. About two-thirds of the patients consulting had at least one consultation per year with symptoms present. The number of contacts came to 4.4 per patient per year and 20--25% of these were with symptoms.
