[Techniques of continuous nerve block at the level of the wrist].

Functional treatment without pain is a condition for good results after operations at gliding structures (tendons) or periarticular structures. Good analgesia of the arm and hand is obtained with continuous axillary nerve blocks. Often there is concomitant paralysis and active motion is not possible. We therefore use continuous nerve blocks at the wrist. The technique of these blocks is described in the paper. We use a prefabricated catheter set to introduce the polyethylene catheter and to place it near the nerve as in single shot wrist block. This technique for continuous analgesia of the hand can be used for functional treatment after tenolyses, arthrolyses and stable osteosyntheses of fingers. Median, radial and ulnar nerves can be blocked all at the same time or alone.

[Intra- and postoperative effect of different hydroxyethyl starch solutions on the flow properties of the blood and on the oxygen partial pressure of the conjunctiva].

During lumbar disc operation in the flexed prone position, a decrease in conjunctival pO2 can be observed although there is an increase in systemic blood flow after infusion of colloidal volume expanders. The regions of the body situated below the heart do not profit from the increase in systemic blood flow due to compression. The pO2 decreases more significantly after the infusion of a medium-molecular-weight solution with a more marked hemodynamic effect than after the infusion of low-molecular-weight hydroxyethyl starch. As the decrease in conjunctival pO2 is considerably less after low-molecular-weight hydroxyethyl starch, improved blood fluidity may be responsible for the increased tissue perfusion in the compressed area.

[Respiratory volumes during anaesthesia (author's transl)].

The respiratory minute volume was determined in 144 normal persons during anaesthesia i a semi-closed system. The measured volumes were related to the bodyweight of the individual person: Quotient Q = Minute volume (ml/min.) over Bodyweight (kilogramme). For adults Q was found to be clearly dependent on bodyweight and sex; extreme weight groups required correction by a factor q which differed for men and women. A table for the calculated minute volumes is available for routine purposes.

[Complications and risks of brachial plexus anaesthesia with special reference to long-term damage (author's transl)].

During 1974-1978 620 regional anaesthesias involving the brachial plexus were performed. Three techniques, viz: supraclavicular, axillary, interscalene were employed. The data are analysed with a view to comparing the efficacy and safety of the techniques with a special reference to long-term damage. Transient injury to the brachial plexus was observed in 3 of 342 patients who had Kulenkampff's supraclavicular plexus anaesthesia. After 3 months all after-effects had disappeared. Plexus block via the axillary route proved to be the safest technique. Detailed inquiries and follow-up examinations established that none of the techniques had caused irreversible damage to the brachial plexus.

[Long-term intubation (author's transl)].

Establishment of a clear airway has become increasingly frequent and is of high importance, especially in patients with severe injuries, head injuries or intoxications. Though the endotracheal intubation-technique has been improved and soft intubation-tubes are now available, tracheostomy is not rendered unnecessary. However, its indications have changed and have become restricted. This trend was proved by the own experiences in 841 patients. The presuppositions for nasal long-term intubation are described in details.

[Anaesthetic methods for surgery of cerebral aneurysms (author's transl)].

The anaesthetist giving anaesthesia for the surgical removal of cerebral aneurysms faces a great variety of tasks. Experience gained in the course of 61 such operations has shown that a combination of controlled hypotension, careful monitoring of the function of the autonomic nervous system and a meticulous surgical technique with the aid of a microscope produces satisfactory results.
