The Authors report a case of a folliculitis developed in a woman who used a spa with hydrojet circulation. This dermatitis is due to Pseudomonas aeruginosa, which can colonizes the closed-cycle water systems.
A 6-year-old girl with mild hepatitis was found to have an elevated urinary level of vanillylmandelic acid (VMA), but no cause for the elevation was found. The patient was receiving griseofulvin for treatment of tinea capitis, and this drug was suspected of causing a falsely elevated urinary VMA level. Four other patients receiving griseofulvin were also found to have elevated urinary VMA levels. In one patient, urinary VMA level determined by an alternate method was normal.

Two cases of recurrent digital fibromatosis of childhood were studied by electron microscopy and immunohistochemistry, using rabbit anti-actin antisera. The tumor cells were typical myofibroblasts, containing inclusion bodies and bundles of microfilaments. Immunohistochemistry showed the presence of actin in these cells, thus proving the myofibroblastic nature of the tumors. Inclusions were negative or showed a weak annular positivity. A possible explanation of these findings is discussed.

The vasoconstrictor activity of four steroids, administered in solution and in the commercially available form, were compared in healthy volunteer subjects. Evaluation was based on conventional visual observations and photometric measurement of reflectance. Statistical analysis showed that halopredone acetate had less vasoconstrictor action on healthy skin than fluocinolone acetonide, beta-methasone valerate and hydrocortisone acetate. The halopredone acetate results were identical to those of th excipient, although it was found that halopredone acetate cream (Topicon) had a marked anti-inflammatory effect in animals and on human dermatoses. The validity of the McKenzie test, as an unequivocal screening procedure for the potency of topical steroids is discussed, and the accuracy and precision of photometric evaluation is emphasized.
In basal cell epitheliomas the testosterone metabolism pattern differs from that of the normal surrounding skin. There is a significant reduction in 5 alpha-reductase activity with a consequent decrease in production of dihydrotestosterone and androstanediols. By contrast the 17 beta-hydroxysteroid dehydrogenase activity shows no consistent change.
The topical application of a 0.1% dimetindene maleate gel on the forearms of 11 subjects reduces the areas of both erythema and wheal produced by the intradermal injection of 0.025 mg of histamine hydrochloride to a statistically greater degree than 1% d-chlorpheniramine maleate cream (p less than 0.005 and p less than 0.0005, respectively). The quantitative method employed in this study has proved to be an excellent means for the evaluation of the antihistaminic activity of topical preparations.
A literature review tries to diminish the ambiguity between hormones and hairs. Therefore the hormonal action in general (regulation of the protein synthesis indirectly by enzymatical regulation of the AMP-system or directly by hormones as active metabolites) and the methods to explore hormones-hair-interaction are discussed. Hormones pertaining to the pituitary-adrenal-gonadal axis are regarded as the paramount hormones; therefore the results of research in testosterone, 5-alpha-dihydrotestosterone, estrogens, progesterone, glucocorticoids, the hypophysis and its tropins are recapitulated. The main disorders of hair-growth, pattern baldness and "idiopathic" hirsutism, which would be dependent on a similar disturbance of androgen metabolism, are discussed. Pathology in hair-growth may arise in any point of the cascade of hormone action.
17,21-Bis(acetyloxy)-2-bromo-6beta,9-difluoro-11beta-hydroxypregna-1,4-diene-3,20-dione (halopredone acetate; Topicon), a new corticosteroid for topical use, was compared with fluocinonide in a double blind sequential study. The tests were made on a total of 16 patients affected with psoriasis and exudative dermatitis. In each patient, the symmetrically distributed skin lesions were separately treated with either halopredone acetate or the reference compound. The two active substances were in the same concentration (0.05%) in the same carrier (cream), and both reduced the subjective and objective symptoms of these skin conditions. No statistically significant differences were observed between the two medicated areas. Haloperidone acetate, the excipients of the cream and the reference compound were all excellently tolerated.
A comparative clinical investigation with three different concentrations of 17,21-bis(acetyloxy)-2-bromo-6beta,9-difluoro-11beta-hydroxypregna-1,4-diene-3,20-dione (halopredone acetate; Topicon), a new topical corticosteroid, has been made to assess the optimum concentration of active substance to be incorporated into cream on the basis of the therapeutic effects achieved in dermatologic patients. The remarkable therapeutical effectiveness of the new steroid, however, has made it impossible to ascertain statistically significant differences between preparations containing either 0.01%, 0.025% or 0.05% of active substance, and to gain thereby a clear answer concerning the relationship between dose and response. Numerous haematochemical controls were made during the trials. In particular, we determined the plasma cortisol levels in patients suffering from large lesions requiring prolonged occlusive medication. No significant alterations of the basal values were observed. As already observed in animals, halopredone, acetate is devoid in humans also of the wellknown effects on the hypophysial-adrenal axis due to transcutaneous absorption of steroids.
Testosterone metabolism was studied in the skin and preputial glands of normal and prepuberally castrated male rats during the 2nd hair cycle. In catagen-telogen the 17-beta-OHSDH of dorsal skin was higher in castrated than in control animals; 5-alpha-reductase instead, remained unchanged through out the hair cycle. Also in the preputial glands of castrated rats 17-beta-OHSDH was higher than in normal rats. So was 5-alpha-reductase. A possible direct control of a substrate, like Testosterone, and/or of a hypophyseal tropin, like prolactin, on enzymes that direct the metabolism of steroids in target tissues is conceivable.
