Congenital ear lobe deformities are, fortunately, very rare. Most of these deformities will require, however, surgical correction due to the importance of the ear lobes in facial symmetry. The scarcity of soft tissues available for reconstruction in the remaining auricle led to the development of a multitude of local flaps. Results are hampered by unsightly donor-site scars and unnatural shape or size of the reconstructed lobule. A case of a unilateral cleft ear lobe is presented to demonstrate a two-stage reconstruction mode that we find to be simple, yet versatile, producing a good esthetic result.
Restoration of a well-defined, youthful-looking, cervicomental angle is not easily achieved. The classic rhytidectomy is now often supplemented by lipectomy and platysmamodifying procedures. Still, irregularities of neck contour are bothersome, and for the short, stocky neck, results at times fall short of a patient's and surgeon's expectation. Insertion of an E-PTFE (expanded polytetrafluorethylene [E-PTFE]; tradename, Gore-Tex, W. L. Gore Associates Inc, Flagstaff, Ariz) soft-tissue patch as a sling to support the floor of the mouth and redefine the cervicomental angle for a smoother neck contour is described. This method was used in seven patients presenting with problematic aging neck. The first three patients needed readjustment of the E-PTFE membrane. This evolving method seems to offer a superior, more lasting result for the correction of an obtuse cervicomental angle.
The prognostic value of nuclear morphometry in nasopharyngeal carcinoma (NPC) was investigated in 28 patients. Seven morphometric nuclear variables were measured on 100 randomly selected tumor cells in nasopharyngeal biopsies from 18 patients with NPC confined to the nasopharynx. The same variables were measured in 6 patients with metastatic NPC, as well as in lymph node biopsies from 4 patients with metastatic NPC. Nuclear area, nuclear perimeter, long and short nuclear axes, nuclear form factor, nucleolar area, and the ratio of nucleolar area to nuclear area were all measured. Volume-weighted mean nuclear volume was also obtained. Tumor cells from patients with NPC confined to the nasopharynx had significantly larger mean nuclear areas, perimeters, and volume-weighted mean nuclear volumes but significantly smaller nucleolar to nuclear area ratios than tumor cells from patients with nodal metastases. Assessment of nuclear form factor and diameters did not differentiate the two groups.
A 50-year-old man, presenting with headache, was suspected of having a malignant nasopharyngeal tumor on the basis of his CT results. A CT scan revealed diffuse calcification in a large nasopharyngeal polyp. Xeroradiography of the specimen demonstrated the calcifications to extend centrally along the stroma. The histology and the radiologic features of this choanal polyp, resulting from diffuse central calcification of the stroma, are a unique feature that might mimic malignancy.
The cheek-lip grooves, often referred to as nasolabial folds, are one of the facial characteristics most commonly complained about by patients in all age groups. A variety of methods for correction have been proposed, using implants or autografts. Some of the methods have relied on diminishing the crest lateral to the groove thus camouflaging the depth of the cheek-lip fold. These procedures have proven to be of limited effect, either because of the amount of correction obtained or because they are short-lasting. The Gore-Tex implant proved its safety and biocompatibility over the past decade as a vascular graft. The characteristics of this implant material are described and compared to other commonly used synthetic implants. A surgical technique of applying the Gore-Tex soft tissue patch in facial aesthetic surgery, particularly in cheek-lip rejuvenation, is presented. This technique has been applied in 17 consecutive patients for the correction of prominent cheek-lip grooves with a follow-up period of eight months. Results showed satisfactory improvement without significant complications.
The reconstruction of subtotal and total losses of the nose involves the use of local and regional flaps from a variety of areas. The forehead flap, using either a Converse reconstruction or a midline glabellar reconstruction, has been widely used for large losses. In order to obviate this type of reconstruction, we have used tissue expansion to gain large flaps for large defects. Our technique and three representative cases are presented.
We describe a patient with a highly unusual appearance of pyoderma gangrenosum. The pyoderma was located on the auricular region and preceded other manifestations of inflammatory bowel disease by 11 years. There was no correlation between the course of the pyoderma and the clinical activity of the associated bowel disease. Mycotic superinfections masked and delayed the diagnosis in our patient for several years. Only when typical pyoderma gangrenosum lesions developed on the legs at the site of trauma and responded dramatically to systemic corticosteroids was the correct diagnosis established. Pyoderma gangrenosum with secondary fungal infection was thus distinguished from deep ulcerated skin fungal infection simulating pyoderma.

Upper airway obstruction in cicatricial pemphigoid is rarely seen by the anaesthetist. A case of severe dyspnoea due to progressive laryngeal obstruction is presented. Tracheal intubation during induction of general anaesthesia for permanent tracheostomy was extremely difficult and was successfully performed by introducing a size 12 French suction catheter through the stenotic laryngeal orifice. Adequate ventilation was achieved by using the oxygen flush valve to deliver oxygen intermittently through the catheter.
