[Transpupillary thermotherapy in the treatment of choroidal neovascularization in age-related macular degeneration--one year's results].

THE AIM OF THE STUDY: Authors refer to one year's results of the study of 56 eyes of 50 patients with the exsudative form of the age-related macular degeneration treated by means of transpupillary thermotherapy at the Department of Ophthalmology, School of Medicine, Charles University, Hradec Králové (Königgrätz), Czech Republic, EU. PATIENTS AND METHODS: In all patients, the best-corrected visual acuity (BCVA) before treatment was established. Beside the ordinary examination methods, the digital color photography of the fundus with topography of the lesion, fluoresceine angiography (FAG), or possibly indocyanine green angiography and optical coherence tomography (OCT) were performed. After the treatment, in given intervals, the BCVA was established, and retinal findings were evaluated by means of biomicroscopic examination, comparative color photography of the fundus, optical coherence tomography, fluoresceine angiography, or possibly indocyanine green angiography. RESULTS: During one year after the treatment, the visual acuity decreased by 2 or more lines on Snellen optotypes in 38.2% of patients, remained stable in 52.9%, and improved in 8.8% of patients. The FAG findings improved in 83.9% and the OCT findings in 84.8% of patients. This improvement was statistically significant. In patients with classical choroidal membrane, the BCVA decreased in 35.8%, remained stable in 57.1% and improved in 7.1% of patients. The FAG finding improved in 78.7%, and the OCT finding improved in 90.8% of patients. The improvement of the OCT findings was statistically significant. In patients with occult choroidal membrane, the visual acuity decreased in 45 %, remained stable in 45%, and improved in 10% of patients. The FAG findings improved in 88.9%, and the OCT findings in 78.9% of patients. The improvement was statistically significant. CONCLUSION: Our results show that transpupillary themotherapy belongs to important methods of treatment of the classical and occult membranes of the choroid in exsudative forms of the ARMD.

[The intravitreal application of triamcinolone in the treatment of the macular edema].

The aim of this study was to evaluate the efficacy of intravitreal application of triamcinolone in macular edema of different etiology. In this prospective study, 20 eyes were evaluated: 12 with diabetic macular edema, 6 with macular edema due to venous occlusion, 1 with pseudophakic cystoid macular edema, and 1 with cystoid macular edema in case of uveitis. Four milligrams of tramcinolon acetonide were injected in to the vitreous cavity. Before and in one month intervals after the application, the intraocular pressure was measured, visual acuity on the ETDRS optotypes was established, the macula was examined biomicroscopically, examination of the photography of the fundus was performed, and macular thickness was measured by means of optical coherence tomography. The follow-up period was 3 months. The mean visual acuity was 0.16 at the beginning of the study, 1 month after the application 0.32, and after 3 months 0.31. The mean thickness of the macula decreased from the initial value 537 microm by 249 microm after one month (improvement by 46.6%) and by 203 miceom after three month (improvement by 39.7%). As a complication, the elevation of the intraocular pressure was noticed in 20% of eyes, and this was compensated in all cases by means of local therapy. The intravitreal injection of triamcinolon seems to be a promising part of treatment of the macular edema of different origin.

[The results of radiotherapy 24 months after treatment by patients with age-related macular degeneration].

The authors evaluate results of the radiotherapy by age- related macular degeneration. 114 patients (123 eyes) were examined. According to the total dose, we have divided the patients in 3 groups. Group A (12 Gy) consisted of 68 patients (74 eyes) and group B consisted of 46 patients (49 eyes), group C was designated as a control group, consisting of 21 patients (22 eyes). They have been performed in Department of Ophthalmology of Teaching Hospital of Charles University in Hradec Králové from February 1999 to April 2003. All examinations were performed before and at 24 months- follow up the radiotherapy. Radiotherapy seems to be a very promising and efficient method for treatment of the wet form of ARMD.

[Specific chorioretinitis--a case report].

The authors present an uncommon case of ocular involvement in extrapulmonary form of tuberculosis with cutaneous manifestation. A case of a patient who was treated for skin efflorescences on legs for several months is demonstrated. The eye examination showed the picture of chorioretinitis of suspicious tuberculous etiology. A prompt healing of cutaneous lesions occurred and retinal finding stabilized under the specific therapy. The authors emphasize that this infectious etiology should not be omitted in differential diagnostic considerations of uveitis.

[The surgical management of macular hole].

The authors evaluate results of the vitrectomy with the internal limiting membrane (ILM) peeling staining membrane blue by 49 patients. 81% (40) patients had idiopathic macular hole, 13% (6) traumatic case, 6 % (3) epimacular membrane. Anatomic closure of macular hole was achieved in 59% (29) of 49 patients by the operation. Visual acuity improved 2 or more Snellen lines in 28% (14) patients. Vitrectomy with peeling of the limiting membrane staining membrane blue of the better identification MLI is the effective operating technique to treat the macular hole.

[Terson syndrome. A case report].

OBJECTIVE: To explain the mechanism of the origin of unilateral retrohyaloid and vitreous hemorrhage in case of subdural haematom after a severe brain injury. METHODS: A detail examination by help of fluorescein angiography and funduscopy revealed a fluorescein leakage site in lamina limitans interna into the vitreous. RESULTS: Fluorescein angiography revealed a fluorescein leakage site at the margin of the optic disc. It was possible to find the source of hyperfluorescence that increased its intensity over a time after vitreous hemorrhage had been removed. CONCLUSION: The diagnosis of Terson syndrome was started on the base of eye examination. The damage to peripapillary tissues demonstrated by fluorescein leakage suggests that intracranial hypertension affects peripapillary structures through the intervaginal space of the optic nerve sheath.

[Posterior scleritis--a case report].

Authors present clinical manifestation and therapeutic possibilities in posterior scleritis, a disease, which may cause diagnostic dilemma. They also give an account of new examination methods which amend the diagnostic schema and safely confirm this diagnosis. In the article, a disease of a seventeen years old man is presented, in which posterior scleritis of unknown etiology was diagnosed. Following the systemic corticosteroid treatment the visual acuity of the affected eye improved, subjective signs disappeared, and the clinical manifestation returned to normal.

[The photopic ERG response in diabetes mellitus].

The authors made photopic ERG examinations according to the international standard in 82 diabetic patients. They recorded a significant reduction of amplitudes of the b wave in a group of 38 patients who at the time of the examination did not have yet any pathological changes on the fundus. Regressive analysis did not prove after laser photocoagulation in these patients, contrary to a control group and diabetic patients with simple diabetic retinopathy, any correlation between the patients age and the extent of the retinal response. Photopic electroretinograms must be included in the electrophysiological diagnostic arsenal in diabetes.

[Optic disc pits and maculopathy (electroretinography study)].

An ERG examination according to the international standard was made in 55 patients with pits in the optic nerve disc. 19 of them suffered from concurrent maculopathy. Electroretinography revealed in scotopic responses a significant reduction of amplitudes of oscillatory potentials and reduction of potentials of the b wave in standard (maximal) reactions. Even more markedly the cone reactions and flicker were reduced--responses to intermittent (30 Hz) photostimulation. The authors consider maculopathies with pits a separate nosological unit with possible hereditary transmission. ERG signalizes a more extensive alteration of retinal structures than affection of the centre, probably on the basis of the circulation. Maculopathies with pits are also characterized by the fact that the dramatic drop of amplitudes is associated with the patient's age or the duration of the disease.

Identification of bovine-specific DNA in feedstuffs.

Considering the menace of transmission of bovine spongiform encephalopathy, feed components intended for cattle nutrition must be checked for the presence of bovine-derived materials. We have been using a method based on polymerase chain reaction for the identification of bovine-specific mitochondrial DNA sequences for this purpose. The specificity of the primers for polymerase chain reaction has been tested using samples of DNA of other vertebrate species, which may also be present in rendering plant products. The method allows the detection in concentrate mixtures of 0.125% of bovine-derived material. Bovine DNA at concentrations corresponding to less than 0.5% of bovine-derived material was detected in 3 of the 30 samples of concentrate mixtures collected from distributors' stores all over the Czech Republic. All 44 samples of fish meal collected from the same sources were free of bovine-derived material.

[Retinopathy in dermatomyositis].

The authors describe a patient with acute onset dermatomyositis, "probable", according to criteria of Bohan and Peter, as well as the development of the disease in the course of 21 months. A special feature was the decline of visual acuity (practically to the level of blindness). The cause was severe retinopathy with dominance of macular oedema, "cotton wool" spots and haemorrhages, as apparent from photographs of the fundus and findings obtained by fluorescein angiography. The problem of initial treatment was resistance to megadoses of glucocorticoid (i.v. pulses). A favourable change occurred only after i.v. administration of polyvalent immunoglobulin in a total dose of 600 mg/kg body weight. Then the authors observed the unique phenomenon of regression of retinal oedema incl. the macula, of the haemorrhages and "cotton wool" spots. Consistent with it vision off the right eye improved to 1.0 and the left eye to 0.66 in the course of 11 months. Stabilized retinal neovascularizations persist. On account of the latter the authors treated the retina by laser panretinocoagulation.

[Radiotherapy of age-related macular degeneration].

The authors evaluate results of the radiotherapy by age-related macular degeneration. They have been performed in Department of Ophthalmology of Teaching Hospital of Charles University in Hradec Králové from February 1999 to June 2000. Our group consisted of 23 patients (23 eyes), 10 males and 13 females in age from 41 to 83 years, with average of 69 yrs. They were evaluated 6 months after begin of radiotherapy. The patients were divided into 2 groups. 11 patients with the entry visual acuity 6/60-6/24 and 12 patients with the entry visual acuity 0.25/50-3/50. The study proved the fact that it is possible to treat the exudative form of age-related macular degeneration using radiotherapy.

[Hereditary occurrence of deep cavities in the optic nerve associated with Best's disease].

The authors present a pedigree where in three generations in male members deep excavations of the optic discs along with Best's disease of the centres in a variable form were recorded. The heredity is autosomal dominant. Retinal changes conditioned by defects of the pigmented epithelium reduce vision to a maximum of 6/60. The anomaly is associated with a pathological EOG record and normal ERG response. The observation is a contribution to discussions in the literature about the hereditary character of excavations of the optic nerve.

[Rare ocular complications in patients with acute necrotizing pancreatitis].

The damage of the visual functions represents a very rare complication of acute necrotic pancreatitis. We have seen it in two cases. The first one in form of toxic damage of the optic nerve, the second one in form of abscess of the vitreous humour and the retina. Both patients were in relative stabilized state, not in the first peracute phase of the disease.

[Dystrophic changes in the macular pigment epithelium as a sequela of exudative diabetic maculopathy].

This work is concerned with the treatment of diabetic exsudative maculopathy using hypolipemic and laser therapy. It shows correlation between the decrease of lipemia and the reduction of the area of hard exsudates in the central region of retina after the hypolipemic therapy. But the corresponding improvement of the vision could not be reached because in 31.2% of cases there were permanent changes of the pigment epithelium of central retina in the original locations of the hard exsudates.

Application of digital image processing to evaluation of eye fundus pictures. Preliminary report.

An application of computerized digital analysis to photographically recorded pictures of the fundus of the eye is described. This approach has been found as a reproducible and objective method for detection and description of different changes in ocular tissues (exudative diabetic maculopathy especially) for the diagnostic or prognostic purposes (progress of the illness, results of the treatment). Possibilities offered by this method in other relevant retrospective studies are also discussed.
