Muscular hyperplasia of the lung: a clinical, radiographic, and histopathologic study.

The clinical, radiographic, and histopathologic characteristics of pulmonary muscular hyperplasia were analyzed in 13 patients who were seen over a 13 year period. The average follow-up period was 7.2 years (range, 1-13). Eight patients initially demonstrated interstitial disease radiographically and the radiographic diagnosis was "interstitial fibrosis." The radiographs of the other five patients exhibited only localized infiltrates initially--produced by bronchiectasis in three, inflammatory mass in one, and bronchial carcinoma in one. Histologic features were consistent with desquamative interstitial pneumonia in eight, usual interstitial pneumonia in one, and unspecified interstitial pneumonia in four. When tuberous sclerosis is absent, pulmonary muscular hyperplasia is probably an end stage of interstitial pneumonia.

In vitro culture of peripheral blood and bone marrow cells in patients with sarcoidosis with and without special preparations of Kveim antigen.

Colony-forming cells (CFC) and colony-forming ability in peripheral blood and bone marrow were studied in 22 patients with different stages of sarcoidosis, with or without the addition of different Kveim preparations. The study showed no statistical differences between the colony growth in healthy persons compared to patients with sarcoidosis in stages I and II. The counts in these groups were, however, lower than in patients with sarcoidosis in stage III (P < 0.01). The lowest counts of CFC were found in the highly active cases suffering from fever, joint pains and erythema nodosum, and these counts were also lower than generally found in normal cases. After stimulation with mononuclear cells from healthy controls the number of colonies in the acute stages, however, reached normal values. This suggests either a low autostimulation or a neutralization of an inhibiting factor in the blood. It is suggested that if an inhibiting factor is present it is principally localized to the sarcoid granuloma. A pronounced inhibition of colony growth was seen when the Kveim membrane fraction prepared from sarcoid lymph nodes and spleen, was added to the cell cultures.

Hexoprenaline and terbutaline administered by inhalation. A comparison between two beta2-adrenoreceptor agonists with respect to effect on bronchial obstruction, heart rate, blood pressure and muscle tremor.

In a controlled double-blind cross-over study in 15 patients with reversible bronchial obstruction hexoprenaline (Ipradol, 0.20 mg/puff) and terbutaline (Bricanyl, 0.25 mg/puff) when administered as metered aerosol in a dosage of one puff followed by two puffs 30 min later produced the same maximum improvement of ventilatory function. However, after inhalation of terbutaline a more sustained broncholytic effect was obtained than after hexoprenaline. Similar results were obtained in six patients after doubling the dose of each agent. The influence on pulse rate and blood pressure was negligible with either agent. The amplitude of finger tremor was measured after 2 + 4 puffs. No significant changes in mean values occurred after either of the two agents although after hexoprenaline an increase in amplitude of more than 100% was recorded in two out of six patients 30 min after the second dose.

Acute phase of sarcoidosis with splenomegaly and hypercalcemia. Description of a case, including a report about splenectomy and the preparation and testing of a Kveim antigen from the spleen.

A young man had a mild, slightly progressive pulmonary sarcoidosis. After 4 years he developed an acute disease with splenomegaly, anemia, marked elevation of ESR, hypercalcemia and mild renal insufficiency. The anemia and ESR elevation disappeared after splenectomy, whereas the hypercalcemia still needs corticosteroid treatment. Attempts to withdraw this treatment resulted in recurrence of the hypercalcemia, but no other abnormalities. In contrast to other organs examined, the sarcoid tissue in the spleen revealed necrosis formation, consistent with a recent process. A Kveim antigen preparation from the spleen was less potent than antigen from mediastinal lymph nodes. It is suggested that the acute phase of the disease involved mainly the spleen. Speculations about the possible role of infectious agent(s) are put forward.
