[Muscular retraction in the elderly. Effect on posture maintenance and mobility. Proposed global physiotherapeutic treatment].

Muscle retraction at the level of the trunk and the lower limbs represents a curable cause of balance and walking disorders, as well as falls, in the older person; therefore it deserves to be recognised, assessed and treated so that elderly patients' independence can be preserved. After making the diagnosis of muscle retraction it is possible to implement a treatment regime which we have evaluated using two indicators which clearly show a functional improvement in walking and risk of falling.

[Spiritual needs and palliative care: assessment of a new care package].

The approach taken in our palliative care document regarding spiritual needs is under three headings: 1) illness in the personal and sociocultural dimension 2) communication and self-esteem 3) spirituality: religions, philosophical or other beliefs, with twenty-eight items, including several sub-items. This approach has been used in sixty-four cases. In the first quatitative analysis, we determined which were the most utilized items, that is: the stages of bereavement, the patient's awareness of his/her illness, the observation of the means of communication he or she uses, and his/her mental state. In the second qualitative analysis, we considered the responses given for each item and sub-item. This enabled the enhancement of the first analysis, by highlighting other items ou subitems, such as beliefs, or messages and touch. From those two analysis, we sought to establish new re-groupings of items. Eleven themes have been highlighted, of which the three most important are: acceptance of the illness and self-image communication relationships with proxies On this basis, we propose to reformulate our approach to spiritual needs in palliative care.

[Icterus and cirrhosis: physiopathology].

The various factors are reviewed which may contribute to the appearance of jaundice in patients with cirrhosis. During the prehepatic phase, hemolysis, spenomegaly and a drop in coagulation factors constitute the main physiopathological mechanisms, whereas intrahepatic cholestasis, alcoholic hepatitis, hepatoma and terminal hepatic insufficiency are the principal mechanisms cited for the hepatic stage. In the posthepatic stage, attention is drawn to the increased frequency of lithiasis in cirrhotic patients and the choledochal lesions seen in chronic pancreatitis.

[Usefulness of the immunoglobulin-albumin ratio compared to electrophoresis in the diagnosis of multiple sclerosis].

The results obtained with gel electrophoresis on CSF samples of multiple sclerosis patients are compared with measurements of CSF IgG indexes (IgG/alb and IgG/total protein). The correlation between these parameters is good, provided the upper normal limit of the IgG/alb index is over 35% and an "overlapping" zone between 21 and 35% is introduced where the IgG/alb index has not the same diagnostic value.
