Major enteropathogenic bacteria isolated from diarrheal patients in Bolivia: A hospital-based study.

A total of 1,234 fecal samples from diarrhea cases were examined for etiological bacterial agents at medical facilities in La Paz and Sucre, Bolivia. Eighty strains of Shigella spp., 39 strains of Salmonella spp., 29 strains of Vibrio cholerae, and 222 strains of enteropathogenic Escherichia coli (139 EPEC, 55 ETEC, 29 EIEC, and 1 EHEC) were isolated. With regard to the serovars of Shigella, S. flexneri 2a, 3a, and 1b were predominant. In the case of Salmonella, S. enteritidis was the most common, followed by S. typhi, S. poona, and S. paratyphi B. Out of 29 cholera strains, 25 belonged to biovar El Tor, serovar Ogawa while the remaining 4 were serovar Inaba. Among 55 strains of ETEC serotypes, 5 showed ST producers but none showed LT producers. Likewise, among 55 strains of enterohemorrhagic serotypes, only one strain (O157:H7) produced verocytotoxin (VT 2). The results of drug sensitivity tests revealed the predominance of Shigella, EPEC, and ETEC strains resistant to aminobenzil-penicillin (ABPC) and trimethoprim. Since diarrheal patients in Bolivia are treated mainly with ABPC or sulfamethoxazole/trimethoprim (SXT) and rarely with gentamicin, kanamycin, or other drugs, it is possible that ABPC- and SXT-resistant strains will increase and persist in the near future.

[Virulence factors and 0 serogroups of Escherichia coli as a cause of community-acquired urinary infections].

BACKGROUND: The aim of this study was to determine the virulence factors and 0 serogroups of E. coli strains that cause community acquired urinary tract infections (UTI). METHODS: We examined 103 E. coli strains isolated from the urine of patients with UTI. The following virulence factors were investigated using phenotypic techniques: the alpha-haemolysin (Hly), the cytotoxic necrotizing factor type 1 (CNF-1) and the mannose-resistant haemagglutination (MRHA) types III, IVa and IVb expressed by P fimbriated E. coli. Serotyping of 0 antigen was carried out by means of a microtechnique using 163 antisera. RESULTS: Fifty-five (53%) of the 103 E. coli strains examined showed some of the virulence factors investigated in this study; 41% of the strains were Hly+, 28% were CNF-1+ and 48% expressed MRHA types III, IVa or IVb. The uropathogenic strains characterized belonged to 27 different 0 serogroups. However, 68% were from one of 10 serogroups (01, 02, 04, 06, 09, 018, 027, 073, 075 and 077) and 36% were from one of only 3 serogroups (02, 04 and 06). Furthermore, the virulence factors were concentrated in strains belonging to the 3 serogroups most frequently detected. Thus, 36 (97%) of the 37 strains of these 3 serogroups showed virulence factors, versus only 19 (29%) of 66 belonging to other serogroups (p < 0.001). CONCLUSIONS: Our results support the special pathogenicity theory and suggest that many cases of community acquired UTI may be caused by a limited number of uropathogenic E. coli strains that produce toxins (Hly+ and/or CNF-1+) and possess P fimbriae or P-related adhesins (with MRHA types III, IVa or IVb), and that usually belong to 02, 04 and 06 serogroups.

[A method for the predictive estimation of the surgical risk in adult cardiac pathology].

INTRODUCTION: In order to test the efficiency of statistical predictive models, we compare the results of a standard method (Parsonnet) with the model created through the data of our population. MATERIAL AND METHODS: We used the chi 2 univariate model, lineal and logistic regression with the data of the whole population receiving cardiac surgical procedure from January 1, 1990 to December 31, 1993 (total 1626 patients). The population was divided into a control group (1100 cases, 68%) and a study group (526 cases, 32%). The coefficients of the control group were used to estimate the results in the study group. RESULTS: Univariate model p value. Significant (p < 0.001) for emergency, age, pulmonary hypertension, left ventricular failure, preoperative use of intra-aortic balloon pump; p < 0.05 mitral valve disease, aortic aneurysm and reoperation. No significance (p < 0.01) was found for gender, aortic or tricuspid disease, percutaneous transluminal coronary angioplasty, unstable or postinfarction angina, transplant, left main or vessel disease number, and mitral, tricuspid or aortic procedure. MULTIVARIATE MODEL: Emergency, pulmonary hypertension, age, left ventricular dysfunction and aortic aneurysm. We estimated a 5.2%, 5.2% and 11.4% mortality with linear, logistic and Parsonnet method respectively with a real group mortality of 6.5%. The average error of the observed and predicted mortality after risk stratification was 5.7%, 6% and 12%. CONCLUSION: A model for risk prediction based on the data of the own institution is more accurate for that population than a model created for comparison between institutions, because the former takes account of the center and population peculiarities.

[The emergency heart transplant].

From the International Registry on Cardiac Transplantation we can not infer a higher mortality in urgent or emergent Cardiac Transplantation. The data in the Spanish Registry and in the literature show that the risk is higher in these patients compared to non urgent transplantation, implying ethical considerations which are discussed in the article. A different approach to urgent transplantation could be based on previous circulatory support, or heart assist systems as a bridge to transplantation.

[Validity of intrathecal secretion formulas of IgG in the follow-up of the response to neurosyphilis treatment].

The different formulas on intrathecal secretion of IgG were evaluated in patients with active neurosyphillis (NS) who did not have HIV infection with the aim of knowing their usefulness and sensitivity in the follow up of response to treatment. Five HIV negative patients with defined criteria of active NS were analyzed. The QAib ratio (albumin LCR/serum x 1,000) and Tourtellotte, Schuller and Reiber formulas of intrathecal secretion and the Link's index were studied prior to and at 15 days and three months following treatment with high intravenous doses of penicillin G over 12 days. A decrease in the QAlb ratio and all the formulas of intrathecal secretion of IgG was observed after treatment. The mean decrease was statistically significant on the third month after treatment for the Tourtellotte formula (116.6 +/- 44 mg/dl in basal control, 60.7 +/- 38 mg/dl at 15 days and 46.5 +/- 59 mg/dl at three months) (p < 0.05). The same evolutive pattern was not observed with the Link's index 1.75 +/- 9 mg/dl in basal control, 1.16 +/- 5 mg/dl at 15 days and 1.79 +/- 1 mg/dl at 3 months) in this series. The formulas of intrathecal secretion of IgG are useful to evaluate the response to treatment of patients with active neurosyphilis. The Tourtellotte formula was shown to be better than the Link index in this evaluation.

[Intravenous cannulae: complications arising from their use and analysis of their predisposing factors].

BACKGROUND: The aim of this study was to analyze the complications derived from the use of intravenous cannulae with particular attention being paid to the possible predisposing factors. METHODS: A prospective follow up of 569 intravenous cannulae placed in the emergency department or the medical ward of a county hospital was carried out. 492 of these cannulae were peripheral (PC) and 77 central inserted through peripheral veins (CPVC). RESULTS: Fifty-one percent of the catheters were withdrawn due to complications with the most frequent being phlebitis (35%), followed by extravasation (11.5%). The mean time in situ was 3.61 +/- 3.2 days. The daily risk of complications ranged from 2 to 9% for the CPVC (non significant differences) and between 15% (day 1) and 30% (6 or more days) for the PC. In the latter case there were no differences in the daily specific rate of complications after the first 24 hours. The probability of a catheter remaining in place more than three and six days without complications was 51% and 31%, respectively for the CP and 91% and 87% for the CPVC (p < 0.0001). Of the variables analyzed, age > 65 years, female sex, insertion of the cannula into the back of the hand, the fact of the route being peripheral without the administration of heparin were significantly associated to the development of complications, while the intravenous administration of aminophillin was associated with the appearance of phlebitis. Thirteen percent of the catheters withdrawn due to phlebitis were infected. CONCLUSIONS: The complication most frequent observed in intravenous cannulation was phlebitis, an entity which is generally not of an infectious nature. The risk of unspecific complications every day during catheterization is constant in the case of central peripheral vein cannulae as well as in peripheral cannulae which do show problems during the first 24 hours after placement. Insertion of the cannulae into the veins of the back of the hand, female sex, advanced age and intravenous administration of certain drugs influence in the development of phlebitis.

[Influence of meat consumption in relation to various cardiovascular risk factors in the elderly].

It has been analyzed food intake, energy and nutrients, Quetelet index, blood pressure and lipid profile in 180 elderly people from Madrid (71 institutionalized and 109 free-living), in order to get the relationship between meat intake (< 150 or > or = 150 g/day) and the different risk factors from cardiovascular disease and other foodstuff intake. Those elderly people who eat 150 g/day or more (AC) take more food in general. There is a significant difference for meat and cereals, eggs, fruits, beans, and fish products. They have only a poor intake of dairy products when comparing with those who eat less than 150 g/day (BC). As a result of these differences in food habits, the AC group has a superior intake of energy and most part of other nutrients. Nevertheless, it was only significant the difference for the niacin and for percentage of energy derived from saturated fatty acid. There are no significant differences in blood pressure. AC elderly have more Body Mass Index and show serum lipids more satisfactory than the BC ones. High meat intake is not related to high risk from cardiovascular diseases in our elderly people group. The reason is that this high intake is associated with a better intake of other foods which give them, in general, a better nutritional status (with better intake of vitamins related to lipidic metabolism) which may be observed at biochemical level.
