[Circulating immune complexes in airborne allergy--a protective role?].

Circulating immune complexes (KI) were analyzed in 78 patients with airborne allergy and compared to 34 persons of control group. KI were isolated by precipitation with 3% polyethylene glycol. KI-IgG, KI-IgA, KI-IgM were measured by radial immunodiffusion, KI-IgE by an immunofluorometric assay. Staphylococcal Protein A (SpA) binding to KI was measured by ria. Relations between the level of various KI and the diagnosis of allergic disease, the kind of allergen (related also to the exposition) and the symptoms of atopy were evaluated. Serum levels of KI-IgE, KI-SpA, and KI-IgM were distinctly elevated in airborne allergy, with the positive correlation between KI-SpA and KI-IgM. KI-IgE were related to the kind of sensitizing allergen and to the exposition to the pollen allergens. Most distinct differences in KI level were observed not in relation to the kind of disease or of the sensitizing allergen, but in relation to the symptoms and intensiveness of atopy. Patients showing a greater number and intensiveness of symptoms exhibited higher levels of KI-SpA. However, one of the symptoms scored (dyspnea) was connected with the decreased level of KI-SpA in these patients. We interpret the results in favour of the hypothesis of the dual role of KI in airborne allergy. We propose a protective role for KI-Spa, which contain IgG and may bind IgE, thus protecting the target organs of allergy.

[Evaluation of bone loss in postmenopausal women during the first 5 years after menopause].

Aim of the trial was estimation of the degree of bone loss in first 5 years of menopause. 184 postmenopausal women were evaluated. Bone status was assessed by using quantitative ultrasound. Both parameters decreased in term of 1-5 years of menopause, what was especially expressed in first 2-3 years of menopause. Densitometric values were lower in comparison to British, French and Japan women population. Also dynamic of bone loss was greater in Polish women population. Results confirm necessity of early prevention of osteoporosis in postmenopausal women.

Effect of antituberculous drugs, isoniazid, pyrazinamide and rifampicin, on chemiluminescence of the human polymorphonuclear leukocytes.

Luminol-dependent chemiluminescence (CL) was used to assay the effect of three antituberculous drugs [isoniazid (INH), pyrazinamide (PZA), rifampicin (RM)] on the production of reactive oxygen species (ROS) by polymorphonuclear leukocytes (PMN) isolated from blood of 5 healthy donors. Drugs were added directly to the medium in concentrations; INH 5 micrograms/ml, PZA 40 micrograms/ml, RM 7 micrograms/ml. These concentrations correspond to peak serum concentrations after usual doses. RM was found to inhibit FMLP (N-formyl-methionyl-leucyl -phenylalanine)-induced CL response of PMN, PZA did not influence the metabolic activity of neutrophils, and INH stimulated PMN CL.

Lymphocyte mitogen-induced proliferation in patients with allergic rhinitis.

Lymphocytes play a central regulatory role in mechanisms contributing to impaired function of immune system in atopy. The aim of our study was evaluate the mitogen-induced proliferation of lymphocytes in a group of asymptomatic, seasonal allergic rhinitis patients. A highly significant lower mitogen-induced proliferation and, in contrast to other studies, significantly lower background proliferative activity of lymphocytes were found in the atopic persons, comparing to the controls. We concluded that the decreased mitogen-induced proliferation of lymphocytes observed in allergic patients reflects abnormal T cell function, which is due to the atopic status, and not only as it was believed to the antigen-induced lymphocyte activation.

[Occlusion of the posterolateral ciliary artery--preceding cerebral stroke].

A case of acute occlusion of the posterolateral ciliary artery which occurred in a 57 years old man, habitual smoker, with II. obesity is presented. Diagnosis was made during emergency basing on fluorescein angiographic examination. The authors concluded that in each case of vascular accident there is a necessity to explore its cause. In the presented case it had been insufficiency of the internal carotid artery which in a short time, after ocular accident, caused a cerebral stroke.

[Platelet aggregation and receptor expression of low affinity to IgE (Fc epsilon RII CD23) in patients with seasonal allergic rhinitis].

In 12 patients with pollinosis analysis of platelet function was carried out. Platelet function was assessed basing on the thromboelastogram curve and aggregation capability induced by ADP. Total serum IgE levels, as well as, specific antibodies to grass pollens, IgE and IgG4 levels and circulating receptor of low IgE affinity (sFc epsilon RII/sCD23) were also analysed. The study was carried out in a dynamic state, during natural allergen exposition and natural remission. A significant correlation was found between loss of platelet function and sCD23 levels. The observed relations were not influenced by seasonal changes.

[Circulating immune complexes in hypertrophic cardiomyopathy and ischemic heart disease].

In 32 patients (pts) with hypertrophic cardiomyopathy (HC), 20 pts with ischaemic heart disease (IHD) and 30 healthy controls, the levels of circulating immune complexes (CIC), immunoglobulins A, G and M, C3c and C4 components of the complement, as well as haemolytic activity of the complement were measured. CIC were assessed using two different methods: a) precipitation with 3% polyethylene glycol with subsequent spectrophotometric measurement of protein content in the precipitate, and b) binding with J-125 labelled staphylococcal +protein ++ A. Pts with HC showed a statistically significant increase in concentration of IgM and the immune complexes (shown with both methods) together with a decrease in C4 and hemolytic activity of the complement. In addition an analysis carried out for each individual patient showed that in some cases an increase in immune complexes concentration was paralleled by a decrease in IgG, C4 and haemolytic activity of the complement. This may suggest that in these pts activation of the complement through the classical pathway can occur. In pts with IHD an increase in immune complexes concentration was demonstrated by precipitation method only. Immune globulines and complement components were within limits for the control group. This suggests that in IHD the complement system is not engaged. Our findings indicate that in HC mechanisms other than those present in myocardial ischaemia must be engaged in inducing changes in the immune system.

Principles and results of corrective lower limb osteotomies for patients with vitamin D-resistant hypophosphatemic rickets.

Forty-four lower limb osteotomies were performed in ten patients with vitamin D-resistant hypophosphatemic rickets. The average follow-up period was 51 months. Osteotomies were combined with shortening and compression plating. This allowed corrections of angular deformities in the sagittal plane as large as 70 degrees and in the coronal plane as large as 78 degrees. There was one complication, a compartment syndrome that resolved completely following prompt fasciotomy. Recurrence of deformity occurred in 27% of the cases. While osteotomies were safe and provided dramatic improvement to limb deformity, postoperative control of vitamin D metabolism was the one constant factor for maintenance of correction.

The hemi 3 syndrome. Hemihypertrophy, hemihypaesthesia, hemiareflexia and scoliosis.

Three unrelated girls presented with a developmental syndrome of hypertrophy involving half or a quadrant of the body and not involving the face. The appearance was one of inappropriately large size of the affected side rather than contralateral atrophy. On the larger side, there was hypertrophy of muscle and increased power as well as an increase in diameter, but not in length, of long bones. There was areflexia and decreased pain and temperature sensation on that side. The patients also had progressive scoliosis and foot deformities on the enlarged side. One patient had a lumbar myelomeningocoele , and all 3 had a family history of neural tube closure defects. EMG, nerve conduction studies, EEG, skull x-rays, PEG, and cerebral CT scans were normal. Myelography did not demonstrate an enlarged cord, and in particular there was no evidence for syringomyelia. Chromosome studies revealed normal karyotypes. Sex chromatin was female on both sides in one patient. A defect of the dorsal lip of the neural tube or the neural crest is postulated to explain the abnormality. The association with closure defect in one patient and a positive family history of other neural tube defects in all 3 patients suggests that the developmental defect occurs at an early embryonic stage. Recognition of the syndrome is important. It can be distinguished clinically from hemiatrophy of cerebral origin. The neurological abnormalities are static, but the scoliosis is progressive and requires correction. The condition is associated with an increased prevalence of neural tube closure defects in the family, and forms part of a spectrum of genetically and embryologically related CNS malformations with multifactorial inheritance. Probands, parents, siblings and parents' siblings should be counselled that the risks of spina bifida and anencephaly in their offspring are the same as those in relatives of probands with classical neural tube defects, and should be offered prenatal diagnosis.
