Competing on the basis of fees?

The Department of Health, Education, and Welfare recently selected specific laboratory procedures for reimbursement under the lowest charge level criteria, mandated in Section 224 of the 1972 Social Security Amendment. According to the author, such government regulations could force hospital laboratories to compete with independent laboratories on the basis of fees, thereby threatening the former's financial stability. In this article, the author analyzes the problem and proposes a solution.

Guanosine triphosphate (GTP): the major organic phosphate in the erythrocytes of the elasmobranch Mustelus canis (smooth dogfish).

1. Ion exchange column chromatography of red cell extracts from the smooth dogfish (Mustelus canis) reveals a phosphate pool which includes inorganic phosphate, AMP, ADP and ATP. 2. GTP and an iron complex of this nucleotide (Fe-GTP) account for approximately 40% of the principal organic phosphates. 3. ATP accounts for 30%, and AMP and ADP approximately 10% each. 4. 2,3-DPG is not detectable by either enzymatic or chromatographic analysis. 5. The use of a "purified" buffer system consisting of redistilled formic acid and de novo synthesized ammonium formate increases the ratio of GTP to Fe-GTP from approximately 1.4:1 to 10:1. This suggests that all of the iron nucleotide complexes (ATP and GTP) previously reported in red cells may not represent a normally occurring intraerythrocyte complex.

Hydergine and vincamine derivative LD 4298 exhibit no anti-sickling properties in vitro.

Amongst the treatments proposed for Sickle Cell disease Ergot derivatives and extracts from Pervinca have been proposed. We have studied in vitro the effect of Hydergine and LD 4298 on Sickle Cells and Sickle Hemoglobin according to the following four parameters : percentage of sickling, potassium leak, minimum gelation concentration and speed of gelation. In these studies no anti-sickling effect was detected. Moreover Hydergine at 50 microgram/ml increased spontaneous potassium loss without any anti-sickling effect. One can conclude that these two drugs have no effect on sickling.

[Iatrogenic foreign bodies in heart (author's transl)].

Two cases are reported of migrating Kirschner wires, which had been inserted for stabilisation of bony defects of the thoracic cage. The wires migrated in both patients to the heart and caused life threatening complications like pericardial tamponade, arrhythmias and cardiac insufficiency. Indications for removal of foreign bodies from the heart are discussed. Attention is drawn to the fact that the ends of Kirschner wires must be bent in order to prevent migration.

[Necrophilia in a 17 year old girl].

Case report of true necrophilia in a seventeen-year-old girl, which is the first description of such a behaviour in a woman. The possible development of the abnormal behaviour is discussed. A complete theory of necrophilia semms to be not yet possible because of the few cases described.

Antibodies reacting with the glomerular mesangium. Isolation and immunopathology.

Antigens of porcine lung were prepared by homogenization and ultrasonic treatment of lung tissue. The antigens were digested by collagenase and trypsin and chromatographed on Sephadex G 200. After immunization of rabbits with certain fractions of the chromatographed material, antibodies could be obtained in rabbits which fixed in vitro and in vivo in the glomerular mesangium of rats, hogs and the human, but not in the basement membrane of glomeruli of these species. Following intravenous injection into rats, the antibodies could be observed in the mesangial area without an apparent loss of antigenicity for 55 days. While 5 days after the application of 20 mg of the anti-mesangial IgG-preparation, there were no glomerular changes histologically demonstrable, after 55 days small amounts of rat-IgG could be demonstrated histochemically within the mesangium. Deposits of rat-complement (C3) could not be demonstrated with certainty. Severe morphological lesions were absent. Rats injected with the same doses of antibody and simultaneously immunized with rabbit-IgG showed a substantially greater deposit of autologous rat-IgG and rat-complement within the mesangial area after 55 days. By histological examination focal and segmental scleroses of the mesangium were determined. A significant pathological proteinuria did not occur. The present model constitutes a new possibility for studying the function of the glomerular mesangium.
