Dental management of Alzheimer patients. A predictive test of dental cooperation in individualized treatment planning.

The aim of the study was to investigate a possible relationship in Alzheimer patients between the stage of dementia, cognitive and functional capacity, and behavior as a dental patient. A special index for assessing behavior in the dental setting was used. The 40 participants were inmates of a nursing home and fulfilled the criteria for Alzheimer's disease in accordance with DSM-III-R. A deficiency in the dental behavior index (DBI) of 50% or less did not correlate with cognitive, functional, or graphic capacity. These subjects were generally aware of earlier regular dental treatment and behaved as if dental visits were familiar to them, although their cognitive, functional, and graphic capacities were more impaired than disclosed by the dental behavior index. A deficiency in the dental behavior index of 50% or more was more correlated with the other capacity assessments. Finding a proper treatment level for an Alzheimer patient is a delicate task in which it is essential to balance awareness of various aspects of impairment and realistic anticipation of benefit. The dental behavior index can be an appropriate instrument in this complex process.

Unexpected dental behavior in five Alzheimer patients. Differences between cognitive and functional capacity, graphic performance, and dental behavior.

The aim of the study was to analyze five patients with Alzheimer's disease in whom the degree of dementia assessed by cognitive and functional tests was at variance with the behavior in the dental setting and to relate these factors to the outcome of the graphic test. Possible reasons for the differences were discussed. The subjects were diagnosed as having Alzheimer's dementia on the basis of the DSM-III-R. Their cognitive and functional impairments were assessed with the Mini-Mental State Examination test and Katz's index of activities of daily living. Behavior in the dental office was registered in a structured form. The graphic function was assessed as Human Figure Drawing. Besides the stage of dementia, the patient's personality, social and medical background, and memories of previous dental care influence the demented patient's behavior in the dental office. To provide secure conditions for treating demented patients, the dentist must be able to interpret the patient's unique personality in the context of the dental situation.

Tolerability and steady-state pharmacokinetics of terodiline and its main metabolites in elderly patients with urinary incontinence.

The elderly form an important target group for the treatment of urinary urge incontinence with drugs such as terodiline (Mictrol, Terolin). In order to evaluate its steady-state pharmacokinetics and tolerability in geriatric patients terodiline 12.5 mg b.d. was given to 28 hospitalized patients with urinary incontinence (mean age 85 years) for six weeks. The patients were monitored during the study and for 6 weeks afterwards, blood samples being taken at regular intervals. In addition to these multi-diseased and polymedicated patients, a small, homogenous group of healthy volunteers (mean age 40 years) was studied as a reference group, being given terodiline 12.5 mg b.d. for 2 weeks. Terodiline was generally well tolerated by the patients and no significant change in blood pressure or heart rate were found. One patient was withdrawn due to adverse effects. The mean terminal half-life of terodiline was 131 h and the clearance after oral administration (clearance/systemic availability) was 39 ml.min-1. The corresponding figures for the healthy volunteers were 57 h and 75 ml.min-1. The average steady-state serum concentration was 518 micrograms.l-1 in the geriatric patients and 238 micrograms.l-1 in the healthy volunteers. Steady-state was reached within 3 weeks in 20 of the 28 patients and within 5 weeks in 7 patients. In the geriatric patients the steady-state serum concentration of the main metabolite p-hydroxyterodiline, during the last three weeks on terodiline was 45 micrograms.l-1, 57 micrograms.l-1, and 45 micrograms.l-1, respectively, and a similar value was found in the healthy volunteers, 47 micrograms.l-1.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of intravaginal oestrogen treatment upon the vaginal absorption of conjugated equine oestrogens.

Conjugated equine oestrogens (0.625 mg) were administered daily and intravaginally to 7 postmenopausal women (aged 70-93 yr) for 14 days. Blood samples were taken at days 1 and 14 immediately before and 2, 4 and 6 h after oestrogen application and at days 4, 6, 8, 11 and 13, 4 h after application. Serum samples were analyzed with respect to total oestrone (E1), unconjugated 17 beta-oestradiol (E2), FSH and LH. Serum total E1 and E2 increased rapidly at day 1 to luteal and follicular phase levels respectively. After that total E1 levels decreased to a plateau corresponding to follicular phase values and remained at that level throughout the treatment period. Serum E2 remained at the follicular phase level during the entire period of treatment. No increase in serum oestrogens could be detected after oestrogen application at day 14. Serum gonadotrophins were already suppressed at day 4 and further decreased to premenopausal values during the latter half of the treatment period. It is speculated that the effects of oestrogens upon a post-menopausal vaginal mucosa involves a diminished resorption of conjugated oestrogens. This effect is, however, not sufficient to avoid systemic effects at the dosage used.
