[The laser therapy of children with Tourette's syndrome].

The method of pathogenetic treatment of patients with Tourette's syndrome (TS) by means of low-intensity infrared laser irradiation was elaborated. Both elevation of the level of xantine oxidase, the inducer of lipid peroxidation, and the decrease of activity of superoxide dismutase, enzyme of the antioxidative defence, were revealed. The correction of antioxidative system was carried out by means of method of over venous laser irradiation of blood together with administration of decreased doses of neuroleptics (orap and haloperidol). Thus, the compensation of patients' clinical state was achieved. The elevated doses of neuroleptics were necessary in control group (without laser therapy application). The stability of the effect was observed in a year according to clinical and biochemical findings.

[Myoclonus epilepsy in children and adolescents].

Twelve cases of myoclonus epilepsy initiating in childhood and adolescence were studied clinically, electrophysiologically and biochemically. Myoclonic and epileptic syndromes being most pronounced, the clinical pattern presented certain polymorphism: asymmetry, hyperkinesis, myodystonia, epileptic episodes, mental disorders. The electroencephalographic picture of the disease was variable. Hyperexcretion of glycosaminoglycans and changes in their fraction composition with predominant release of heparan sulphate evidenced for deranged metabolism of mucopolysaccharides.

[Experience with L-DOPA treatment of children with dystonia musculorum deformans and dystonic hyperkineses in infantile cerebral palsy].

Nineteen children with dystonia musculorum deformans and dystonic hyperkineses were treated by L-DOPA. A mild positive effect was obtained in 6 children, a considerable positive effect in 9 children. A more marked positive effect was obtained in children with dystonia musculorum deformans. The results indicate the efficiency of L-DOPA in the treatment of dystonia musculorum deformans and dystonic hyperkineses against the residual organic background. The advisability of 2-3 month long courses of therapy with L-DOPA 2-3 times a year is discussed. Some patients may need a continuous therapy with maintenance doses of the drug.

[Pathogenesis and treatment of tic-type hyperkinesis in children].

Results of clinical and electrophysiological examinations of 52 children with tic-type hyperkineses are presented. Nine of them had generalized tic (Gilles de la Tourette's disease), and 43 had local forms. All the children were examined electroencephalographically; 26 children had, in addition, an electronystagmographic examination. Changes pointing to organic affections of the brain were revealed. The authors recommend that anticonvulsants be used for tic therapy. In children with generalized tic the anticonvulsants may be combined with haloperidol.

[Neurologic manifestations in hereditary Osler-Rendu hemorrhagic telangiectasia].

The neurological manifestations of Osler-Rendu's syndrome observed in 10 patients were characterized by polymorph permanent of transient clinical pathology in the forms of subarachnoidal hemorrhages, ischemic disturbances of cerebral circulation, symptoms on the part of the brain stem, headaches, vestibular and convulsive disturbances. The cerebral disorders were caused by teleangiectases in the cerebral vessels, the fact, that was confirmed in 2 cases on autopsy. The cerebral teleangiectases may be the cause of a number of etiologically vague neurological syndromes (spontaneous subarachnoidal hemorrhages, etc.). While diagnosing the cerebral form of Osler-Rendu's syndrome one should take into consideration both neurological and somatic manifestations.

[Dynamics of the premorbid nervous system defect in children with acute infections and craniocerebral injuries].

Owing to a high plasticity of the child's brain complete or partial compensation of a cerebral defect is possible. However an infection or a craniocerebral injury may interfere with the compensation of the premorbid defect of the nervous system: this was noted in 65% of children observed by the authors. Therefore, while specifying the syndromological structure of a disease one must take into account the time course of the premorbid defect. The compensation of that defect is hampered most often in children of the first year of the life. In these cases a loss of a part of acquired habits and appearance of earlier-age reflexes is noted. At the age of 2 to 5 years an aggravation of the atactic and hyperkinetic syndromes is observed, while at the age of over 5 years the negative time course of the premorbid defects is noted twice as rarer. Variants of the time course of the premorbid nervous disorders are specified. At the basis of reversible variants are disturbances on the part of hemodynamics and cerebrospinal fluid. This is confirmed by echo-EEG and REG findings. Persisting aggravation of the premorbid defect and appearance of new neurological symptoms are observed on development of hypoxic and metabolic disturbances. The nervous structures affected by perinatal hypoxia remain thenceforth to be especially sensitive to the effects of the latter. An analysis of the pathogenetic mechanisms of the premorbid defect decompensation may facilitate the realization of therapeutic and prophylactic measures.

[Principles for intensive therapy of status epilepticus in children].

A total of 113 children with brain edema of infectious and toxic nature were studied. The main clinical syndrome in these conditions was an epileptical status. Among anticonvulsive drugs used seduxen appeared to be the most effective. The use of GABA, gas narcosis and myorelaxants is recommended as well. The rehydration by plasma, low molecular plasma substitutes and glucose-polyion solutions are discussed. The paper contains some information on contraindications for early dehydration and rehydration. Hypoxia is eliminated by providing adequate lung ventilation, oxygenotherapy or hyperbaric oxygenation, by using oxidative enzymes and drugs increasing the resistance of the brain to hypoxia and hypothermia. It is recommended that hormonal therapy should be used. The techniques of urgent measures and their sequence are given.

[Lesions of the nervous system in premature children with "conjugated" jaundices].

Results of many-year observation of 110 prematurely born children who had had nuclear jaundice in the precence of hyperbilirubinemia showed that this pathology not only had distinct temporal parameters, but in most cases also a characteristic clinical picture. The principal symptoms are: an abrupt tossing of the head back, general tonic convulsions, tonic stretching of the arms with pronation of the hands, monotonous constrained cry, amimic face, paroxysms of apnoe, and gross impairments on the part of the brain stem. The most important for the diagnosis and prognosis was the "setting sun" symptom. The diagnosing is difficult when the nuclear jaundice joins pre- or intranatal damage to the nervous system or is combined with an infectious toxicosis. Observation of infants during the first six months makes it possible to confirm the diagnosis. The gravity of the sequelae makes it necessary to transfer infants from the maternity home to pediatric hospitals even if the jaundice is only suspected.

[Hemiatrophy in children].

A clinico-electrophysiological characteristic of 56 children with the hemiatrophy syndrome is presented. The main clinical variants of hemiatrophy in children were distinguished. Some materials on the etiology and pathogenesis of this disorder are considered. The authors used electrophysiological methods in the study of this disorder: EEG, Echo EG, REG. The nozological entity of hemiatrophy the secondary symptomatical variants and features of the clinical development are discussed.

[Clinical picture and course of hereditary extrapyramidal diseases in children].

The report contains a description of the clinical development of hepato-cerebral dystrophy in 11 children and of deformans muscular dystrophy in 7 children. The authors indicate to expressed visceral disorders in patients with hepato-cerebral dystrophy and to positive results in the treatment by thiol preparations. The paper also contains an observation with an inheritance of deformans muscular dystonia of a dominant type. Due to a clinical polymorphism of hepato-cerebral dystrophy and deformans muscular dystrophy in children a possibility of a wide phenocopy of this pathology should be taken into consideration.

[Renal excretion of glycosaminoglycans under normal conditions and in several nervous system diseases in children].

The authors studied 101 children with diseases of the nervous system (with hereditary and acquired pathology) and 205 practically normal children. The urine excretion of specific components of the connective tissue (glycosaminoglycanes) was higher than in normals. Lesions of the connective tissue stroma in such forms of pathology is expressed not only in quantitative changes of glycosaminoglycane, but in their qualitative characteristics. Disturbances of the fractional compounds of glycosaminoglycane in the urine of patients differs from the parameters of chromatograms of normals by a high content of fractions of heparansulfate in a relatively low level of chondroethylsulfatolike fractions. In such states the severity of the clinical picture is accompanied by expressed metabolic disturbances.
