Some phenotypic and genotypic characteristics of prevailing gonococcal strains in northern Norway.

Ninety two non-penicillinase producing Neisseria gonorrhoeae (non-PPNG) strains were investigated by the restriction enzyme technique with Hind III enzyme. Serogrouping by coenzyme. A with monoclonal antibodies and testing susceptibility to doxycycline were also performed by an agar dilution method. Fifteen different restriction enzyme patterns were shown, of which three represented 50% (46/92) of all strains. One band, representing a deoxyribonucleic acid (DNA) fragment of 1.45 kilobase pairs, was shared by all the strains investigated. Serogroup WI dominated, representing 57 isolates (62%) with four serovars, one of which was found in 82% (47/57) of the WI serogroups. Eight serovars were found among WII and WIII serogroups, but no one serovar dominated, as happened among WI serogroups. Identical serovars and restriction enzyme patterns were found in eight patients who were epidemiologically linked. All strains were highly or moderately sensitive to doxycycline, and the strains belonging to serogroup WI were more sensitive than those in the WII or WIII serogroups.

Multivariate analysis of Neisseria DNA restriction endonuclease patterns.

Chromosomal DNA was extracted from eleven Neisseria meningitidis and seven Neisseria gonorrhoeae isolates and cleaved with the restriction enzyme HindIII. The DNA fragments were separated according to their size, using a 4% polyacrylamide gel. The band patterns obtained were digitized and statistically analysed by the SIMCA method. To develop the models for N. meningitidis (class 1) and N. gonorrhoeae (class 2), all eleven meningococci and seven gonococci, were used. All strains were classified correctly and showed an extremely good class separation.

Skin and respiratory tract symptoms in veterinary surgeons.

Chronic or relapsing irritant eczema of the hands was the main complaint in 34 veterinary surgeons. 9 had contact allergic eczema of occupational character. In 8 cases, the allergies were probably due to antibiotics contaminating the skin during the administration of treatment. Contact allergy to penicillin was observed in 5 cases, to neomycin in 2 and to streptomycin in 1 case. Sensitivities to other work-related substances such as rubber chemicals, antiseptics and local anaesthetics were found in 6 cases. Multiple contact allergies, in particular to different penicillins, were seen frequently. Obvious work-related immediate reactions of probable allergic type were present in 10 cases, but were confirmed with positive RAST or prick test in only 2 cases.

Phenotypic and genotypic characterization of penicillinase-producing strains of Neisseria gonorrhoeae.

Thirty penicillinase-producing Neisseria gonorrhoeae (PPNG) strains, 26 representing 9 serovars of the protein IB and 4 representing 2 serovars of the protein IA serogroup, had 10 different patterns when investigated by the restriction enzyme (RE) technique with Hind III enzyme. One RE pattern dominated, representing 11 strains with 6 different serovars of both the protein IA and IB serogroup. Identical serovars and RE patterns were found among 7 patients (3, 2 and 2, respectively) with positive epidemiology. The majority of the strains (24/30) were resistant to, or showed decreased sensitivity to, doxycycline with minimum inhibitory concentrations between 1 and 8 mg/l, consistent with the Far East origin. All the 30 PPNG strains showed by agarose gel electrophoresis to harbour a 2.8 Mdalton, a 4.4 Mdalton, as well as a 24.5 Mdalton plasmid, whereas the two non-PPNG strains had only the 2.8 Mdalton plasmid.

Activity of nine antibacterial agents on penicillinase- and nonpenicillinase-producing Neisseria gonorrhoeae isolated from clinical specimens obtained in northern Norway.

34 penicillinase-producing (PPNG) and 102 nonpenicillinase producing (non-PPNG) strains of Neisseria gonorrhoeae were tested against 9 antibacterial agents. Cefoxitin, latamoxef, cefotaxime and imipenem inhibited all PPNG (MIC less than 2 mg/l); chloramphenicol (2 mg/l) inhibited 86%, and 3% and 8%, respectively, were inhibited by 1 mg/l of doxycycline when disk diffusion or agar dilution sensitivity testing were performed. All PPNG strains were sensitive to spectinomycin (MIC less than 32 mg/l). Non-PPNG strains were uniformly sensitive to all antibacterial agents, doxycycline being slightly less active than the other compounds.

Genomic fingerprinting in the epidemiology of gonorrhoea.

We investigated the usefulness of the restriction enzyme (RE) fingerprinting for epidemiological tracing in gonococcal disease. The RE patterns of three paired gonococcal isolates showed corresponding identical fingerprints. Within each of the three pairs of epidemiologically linked isolates the respective restriction patterns were completely identical. Also, the restriction patterns of 6 strains from a larger contact group were identical. Identical restriction patterns were also obtained in each of the two cases where isolates were recovered from both urethra and cervix. The serological findings were in perfect agreement with the genomic fingerprinting as to the identity between all strains of the same epidemiologic chain. Relapse of the original infection could be excluded in one case by the finding of a different RE pattern and also a different serovar pattern of the strain recovered 4 months later.

Infantile acropustulosis.

We report two patients with infantile acropustulosis; one patient with a history of atopic dermatitis and the second with an abnormally high IgE serum value. Atopy has not been reported previously in patients with infantile acropustulosis. Immunofluorescent studies of involved skin gave negative results.

UV-light therapies in atopic dermatitis.

106 patients from Northern Norway were given phototherapy for atopic dermatitis with ultraviolet B (UVB) (group B) or in combination with ultraviolet A (UVA), designated UVAB (group AB). Of the 52 patients in group B, 44 (84%) were cured after 23 treatments whereas 51 (94%) of the 54 patients in group AB were cured after 18 treatments. This difference is statistically significant (P less than 0.01). Of the remaining 11 patients, 6 in group B and 2 in group AB improved, and only 2 in group B and 1 in group AB were unaffected, by this treatment. In group B, 30 of the 44 healed patients had relapsed 3 months after the 1st treatment, compared to 27 of 51 patients in group AB. 3 months after a 2nd treatment, 36 (69%) of the patients in group B were still in remission compared to 51 (94%) in group AB. No side-effects were observed. The results indicate that, in the area of Norway where even the summer sunlight is scarce, UVAB and, to some extent UVB light are effective treatments for atopic dermatitis.

[Treatment of acne vulgaris. Double-blind study of a combination of 5% benzoyl peroxide/2% miconazole (Acnidazil) and tretinoin 0.05%].

The clinical efficacy and tolerance of a combination of benzoyl peroxide 5% and miconazole 2%, Acnidazil, for the treatment of facial acne vulgaris have been compared with those of tretinoin 0.05% in a double-blind study with 40 patients during 12 weeks. The combination of benzoyl peroxide and miconazole gave improvement in 82% of 22 patients, compared to tretinoin which gave improvement in 50% of 18 comparable patients. The difference is significant. Also, side-effects were significantly less in the benzoyl-miconazole group than in the tretinoin group.

Restriction endonuclease fingerprinting of chromosomal DNA of Neisseria gonorrhoeae.

Twenty gonococcal isolates representing different serovars of the protein IA & IB serogroups, as determined by co-agglutination with monoclonal antibodies, were investigated by the restriction enzyme (RE) technique with Hind III enzyme. The patterns resulting from RE digestion and subsequent electrophoresis of the DNA fragments in gel consisted of 40-45 bands. Eight RE patterns were observed among ten of the protein IA strains representing three serovars with an identical pattern for three strains of the same serovar with positive epidemiology. Seven RE patterns were observed among ten of the protein IB strains representing five serovars with identical patterns for each of three pairs with available positive epidemiology for two of them. Seven samples from the same bacterial clone extracted and digested separately showed identical band patterns. The genetic stability was illustrated by identical patterns through 41 passages in vitro and by the same band pattern of the various virulent and avirulent colony morphology variants of a particular gonococcal strain. The results show that RE analysis of the gonococcal DNA genome is well suited as an adjunct to phenotypic markers in epidemiological studies of gonorrhoea.

Trichophyton-rubrum-infected toenails treated with ketoconazole and partial nail avulsion.

30 patients with totally 178 toenails infected with Trichophyton rubrum were randomly treated with ketoconazole alone for 4 months (group A) or in combination with chemical nail avulsion (group B). Of the 14 patients in group A who completed the treatment, 4 (28%) were cured 6 months later, compared to 8 (53%) of 15 patients in group B. In 4 cases, transient elevated hepatofunction enzymes occurred. Gastrointestinal disturbances caused withdrawal in 1 case.

In situ characterization of cell infiltrates in the dermis of human scabies.

Cell infiltrates in the dermis of biopsy specimens from skin lesions of seven patients with scabies were characterized by receptors for sheep erythrocytes (T-lymphocytes), Fc C3b (macrophages and B-lymphocytes) and C3d receptors (B-lymphocytes) using hemadsorption to cryostat sections in closed chamber. T-lymphocytes were also identified using an indirect immunofluorescence technique with rabbit anti-T-lymphocyte serum. All sections showed perivascular mononuclear cell infiltrates in the dermis as judged by sections stained by hematoxylin and eosin. T-lymphocytes were the predominant cells in the infiltrates. Fewer macrophages and very few B-lymphocytes were detected. Cell-mediated immune response may be of importance in the pathogenesis of scabies.
