[Recurrence of pulmonary tuberculosis: in- and outpatient data].

Diagnosis recrudescent pulmonary tuberculosis shows different flaws: on the one hand, the progression of early untreated tuberculosis can be mistaken for a recrudescence, on the other, changes are interpreted as a recrudescence if there is no clinical and X-ray evidence for the reactivation of the process. All cases of recrudescent tuberculosis should be reexamined by a medical commission, which allows the above errors to be avoided. If a patient with chronic alcoholism and mental disease has recrudescent tuberculosis, a narcologist and psychiatrist must participate in working out a comprehensive treatment policy.

[Detection of pulmonary tuberculosis in migrants].

Tuberculosis was detected in 2.2% of migrants from Baku, whereas among those applying for medical and social care to the charity organization "Physicians without Boundaries" this value is 4 times higher: 8.6%. These results necessitate the development of a complex of antituberculosis measures adapted to new marginal risk groups-migrants. The principal purpose of these measures should be to make specialized medical care as available as possible for this group.

[Therapeutic drug monitoring and the pharmacokinetics of carbamazepine in children with a convulsive syndrome].

An analysis of maximal and minimal serum concentrations of carbamazepine was performed in 40 children with convulsive syndrome. All the patients received the drug for a long period. Considerable case-to-case differences of serum concentrations in spite of practically equal doses were revealed. Serum drug concentrations were therapeutically optimal (6-8 mg/ml) only in one third of children. On the basis of pharmacokinetic parameters, mean optimal dosages sufficient for maintenance of effective and safe carbamazepine concentrations were calculated. As the child grows general clearance of carbamazepine decreases. Over the age of 9 years it is significantly lower in girls than in boys of the same age. Speed of carbamazepine elimination was higher in cases of combined treatment than in cases of monotherapy. Indications and optimal time for therapeutic drug monitoring of carbamazepine are determined and clinical illustrations are presented.

[Subclinical hypothyroidism in patients with ischemic heart disease].

A total of 187 patients with coronary heart disease (CHD) were examined. Out of them 60% were found as having subclinical hypothyroidism. The serum levels of T3 were significantly lower, particularly in arrhythmic CHD, the level of T4 was normal and even increased, thus making the concentrations of TTG normal, i.e. the T3-low-syndrome was detected. Hypothyroidism in CHD patients decreased tissue oxygen consumption, by reducing the incidence of acute myocardial infarction, heart failure, the atherosclerotic process becoming disseminated and progressive. Cordarone therapy was found to promote the progression of T3-hypothyroidism, i.e. in CHD patients with suspected hypothyroidism, the drugs inhibiting the thyroidal function should not be used. T3-Hypothyroidism should be corrected with adequate doses of triiodothyronine in CHD patients. The use of thyroxine is ineffective as the conversion of T4 to T3 is impaired in the body.

Achilles Tendon↗

[Tuberculosis in children and adolescents in orphanage and boarding schools and optimization of its prevention].

Clinical examinations and follow-up were conducted in 1641 children and adolescents at risk of tuberculosis infection. Of them, 701 examinees attended children's homes and boarding schools, 940 controls went to kindergartens and secondary schools. The trial revealed that the test group children and adolescents are at much higher risk to develop tuberculosis than controls. Primary infection and the disease were recorded in them 8.5 and 1.8 times more frequently, respectively. These facts urge compulsory introduction in children's homes and boarding schools as the foci of minor bacterial dissemination of the whole complex of antituberculosis measures, tuberculin diagnosis twice a year and isoniazid chemoprophylaxis, particularly.

[Tuberculosis morbidity among the population of a large industrial center].

Tuberculosis morbidity in children, adolescents, young people and adults for the period of 1986-1990 has been analysed. Parameters of morbidity were compared with general morbidity of the population of Chelyabinsk. It was found that at the present stage of tuberculosis control parameters of general morbidity remained unchanged. Morbidity in adolescents and young people aged 18-19 years markedly decreased, while a tendency to its slow growth was revealed in children and in subjects aged 24-29 years. In contrast to other groups, parameters of morbidity in adults are the highest (32.7-37.4 per 1,000,000) and steady owing to which measures aimed at tuberculosis control should be improved.

[Specific prevention of tuberculosis in young patients by BCG vaccine].

The efficacy of antituberculous BCG revaccination studied in workers, students and young subjects of the penitentiary labour establishments (PLE) was assessed by the level of tuberculin sensitivity, the presence of a postvaccination skin sign, tuberculosis morbidity and severity of a tuberculosis process. The highest efficacy was achieved in workers and students as a result of systematic revaccination. Tuberculosis morbidity in them was 26.2 and 34.5 per 100,000, respectively. In the PLE contingent it was several times higher because they received revaccination as early as their school years. The postvaccination skin signs were found in 91.8% of workers, 76.4% students and 60.5% young subjects of PLE. In students and workers reactions with an infiltrate size of 5-11 mm were predominant and in the PLE contingent those with an infiltrate of 12 mm and more. Subjects having a skin sign had more benign tuberculosis course. Bacillary excretion was found in 48.9% and in those without a skin sign in 72% of cases (p less than 0.05).

[The formation of groups at risk for tuberculosis by means of questionnaires and using computers].

The highest risk groups of tuberculosis origination and recurrences have been ascertained on a large factual material. A multifactorial computed analysis conducted on the basis of the automatized dialogue system made it possible to establish the optimal combinations of the socio-hygienic and medico-biological factors to detect risk groups of emergence of respiratory organs tuberculosis and its recurrences. The main risk group of respiratory organs tuberculosis included subjects adhering to the social way of life, having concurrent respiratory diseases and living under the unfavourable housing conditions. Respiratory organ tuberculosis most probably recurs in subjects who were not fully cured of a primary disease and in those who had an acute disease one year prior to recurrence detection. The developed system helps a practitioner-phthisiatrist in decision making concerning risk groups formation.
