In vitro models of a three-drug regimen (epirubicin, cisplatin and fluorouracil) for the treatment of colorectal cancer.

The activity of epirubicin, cisplatin and 5-fluorouracil (5-FU), as single agents or in combination (ECF), was investigated in three human colon cancer cell lines by two different assays (cell-counting assay and sulforhodamine B assay) in vitro. 5-FU was tested with both short and continuous exposure. Particular interest was focused on the results obtained in the HCT8-FU cell line, a subline with experimentally induced resistance to 5-FU. The positive modulation of both cisplatin and epirubicin cytotoxicity by 5-FU makes the ECF regimen an attractive protocol for combination therapy in colorectal cancer.

[Gastric leiomyoblastomas: a clinical case report].

Leiomyoblastomas are about the 10% of non epithelial gastric neoplasms. They are interesting for clinical, therapeutic and prognostic findings very different in comparison commonest epithelial tumours. The authors, after discussing aetiology, pathogenesis, clinical outline, diagnosis, histological and pathological features as well as therapeutic treatment of these neoplasms, report a case describing diagnostic and therapeutic management.

[Surgical treatment of total rectal prolapse: Delorme's technique].

Rectal total prolapsus is a cause of a permanent disablement more founded in old women. Many pathogenetic hypotheses are made up to date time, many surgical treatments were proposed for the treatments of this disease: through perineal, abdominal or combined way. The results are different, of course, in a short and a long time, whether in the matter of relapse of the prolapse, or in morbidity in connection to surgical treatment and his sequences. After a review of the most frequent techniques now in use, the Authors report their experience on 6 patients operated from 6 January 1988 to June 1993, giving reasons for their preference to Delorme's operation through perineal way.

[Perforation of the colon].

The paper reports personal observations relating to 111 cases of colonic perforation. Etiopathogenesis and different surgical approaches are discussed and results are compared. Hartmann's operation is identified as the best solution in cases of colonic perforation with diffuse peritonitis. In cases of localized peritonitis, excellent results may be obtained by operating immediately.

[Emergency surgery in icterus].

Obstructive jaundice may be, still today, a surgical emergency. After the analysis of the serious syndrome following a persistent cholestasis, the authors discuss new technological diagnostic procedures and indications, pros and cons of the use of P.T.D. (Percutaneous Transhepatic Drainage). Finally, they underline emergency cases and proper surgical management, on the basis of their personal experience, as well.

[A case of leiomyosarcoma of the small intestine].

This paper concerns a case of leiomyosarcoma of the first jejunal loop both bleeding and perforated. Authors do outline the rarity of this associated complication and hence how correct diagnosis of the disease is difficult. According to their experience, the relative early onset of complications allows a surgical approach on the malignancy in an early stage.

[Penetrating heart wounds: four cases (author's transl)].

The authors describe four cases of penetrating heart wounds: two from slash and puncture injuries and two from gunshot. Hearth lesions involved the left ventricle and pericardium in two cases, the right ventricle in one case, and only the diaphragmatic pericardium in one case. After a review of the literature, the authors discuss clinical and diagnostic problems relative to penetrating chest wounds and particularly those of the heart, stressing the need for very prompt diagnosis; as for actual treatment, they conclude that this can be carried out without any special problems in any Department of general emergency surgery.

[Complications of primitive tumors of the small gut: personal observations in nine cases (author's transl)].

The authors describe 9 cases of primitive tumors of the small gut (3 adenocarcinomas, 2 lymphosarcomas, 1 fibroma, 1 angioma, and 2 leiomyosarcomas, one being the cause of active hemorrhage). In all cases the tumors were diagnosed only at operation, which was invariably necessitated by complications. Seven patients underwent emergency surgery for intestinal obstruction; one underwent an exploratory laparotomy for recurrent intestinal bleeding caused by an angioma of the ileum; and the last of this series (exceptional enough to warrant separate reporting elsewhere) was operated upon as an emergency case when the tumor, precisely a leiomyosarcoma, was perforated and actively bleeding.

[Diverticulosis of the small intestine. Observations on a complicated case].

Small bowel diverticula, particularly the jejunal ones, are a rare disease with a poor and vague symptomatology. Sometimes detected by change under X-ray examinations or at surgery, they do not need any surgical treatment: only clinical and X-ray controls are required. Prophylactic resection of symptomatic low jejunal diverticula is controversial: patient's age and status will affect the indication to surgical treatment. Jejunal diverticulosis may generate serious, though not dramatic, symptoms: intestinal obstruction, inflammatory complications, haemorrhage, perforation of the diverticula are the most frequent emergencies which require an immediate surgical procedure. A clinical case is here reported.
