[Education and psychological management of the tumor patient from the viewpoint of the surgeon].

The opinion concerning the information of patients with malignant tumors about the diagnosis has increasingly changed during the last years. In the most cases the diagnosis of malignancy must be told to the patient especially if he needs additional treatment after operation. An individual proceeding is necessary but some modalities can be generalized. It is attempted to describe some of it according to 35 years experience in surgery.

Longitudinal neuropsychological changes in a "normal" elderly group.

The need for age-related normative data on neuropsychological measures is clearly important. This longitudinal study examines a "normal" elderly group's performance on the Luria-Nebraska Neuropsychological Battery (LNNB) across four years. This report is based on the 59 "normal" elderly volunteers seen in an earlier cross-sectional aging study (MacInnes et al., 1983). The LNNB Clinical Scales exhibited few significant changes across the four year time period. The test-retest correlations of the LNNB Clinical Scales ranged from.32 to.81. Few differences between males and females were seen. Similarly, being in an older subgroup did not affect the rate of change on the LNNB scores. Although, health status changed very little over the four years, health status at Time 2 did significantly predict 16 of the 17 LNNB Clinical Scales at Time 2. In summary, we found the LNNB accurately identified the "normal" elderly as normal. Similarly, their performance on the LNNB exhibited little meaningful change across four years.

[Procedure in perforated diverticulitis of the sigmoid colon (author's transl)].

Operation in three stages with primary defunctioning colostomy and drainage of the perforation is the procedure mostly recommended in literature, although the mortality rate ranges from 33 to 75%. Therefore, we prefer the immediate resection of the perforated bowel with primary anastomosis, or if possible, in poor risk cases Hartmann's procedure respectively. Anastomosis should be protected by a defunctioning colostomy in all but exceptional good cases. 2 of our 18 patients died, only 1 of 15 (7%) with primary bowel resection.

[Surgery of the biliary tract in geriatric patients (author's transl)].

During 5 years 1311 operations for benign diseases of the biliary tract were performed. 281 patients were more than 70 years old, on the average 76,1 years. According to experience and the results the following is emphasized: 1. Complications of gallstones are relatively more frequent in old patients. 2. Cholecystectomy for chronic gall bladder disease should be performed even in old age. Our postoperative mortality rate was 2,5%, increasing to 15,1% when operation was done in a stage of complications. 3. Acute inflammatory cholecystitis should be treated by urgent operation, especially in old age. 4. Cholecystectomy is the method of choice for acute cholecystitis. Cholecystostomy is used only in few exceptional cases. 5. Cholangiometry and cholangiography are always executed, if the cystic duct can be cannulated, even in cases of acute inflammation. 6. If there are stones in the common duct or a stenosis of Oddi's sphineter is found, an acute inflammatory or perforated cholecystitis is no impediment for a one stage repair. 7. Cholangioscopy is the unsurpassable method for exploring the common duct, when it was opened or sphincterotomy was done. 8. Principally indications for transduodenal sphincterotomy are the same for all age groups. The postoperative mortality rate is not higher than after other operations involving the common duct. 9. In old age choledochoduodenostomy is sometimes a good alternative to sphicterotomy. 10. In cases of recurrent operation due to overlooked stones in the common duct or sclerosis of Oddi's sphincter the mortality rate is not higher than after primary interventions for such affections.

[Investigations on the prophylaxis of wound infection by the topical application of a combination of polymyxin-b, bacitracin and neomycin (polybactrin spray) (author's transl)].

A controlled prospective trial was carried out on 100 cases of appendicectomy for a suppurative or perforated apendix, 153 operations on the biliary tract and 25 abdominal colon and rectum resections. The topical administration of Polybactrin aerosol, sprayed once into the open laparotomy wound produced a significant reduction in postoperative wound infection.
