[Management system for patients requiring stereotactic macrobiopsies of the breast: report on 100 patients].

PURPOSE: The authors describe and evaluate the management system for patients requiring large core vacuum-assisted biopsies at a dedicated breast center. This process includes mandatory review of all requests by a multidisciplinary team. MATERIAL: and method. Patients were provided questionnaires to evaluate their degree of satisfaction with the management process. Results from biopsy and surgery were retrospectively reviewed. RESULTS: From the 100 patients included in the study (106 macrobiopsies), 95,28% of biopsies were informative. The degree of correlation between the histological diagnosis from biopsy and surgery was 86,95% for the 46 operated cases. The degree of acceptability by patients as well as medical and non-medical staff was satisfactory. CONCLUSION: The original feature of this process is the retrospective review of all charts after the histological results becomes available for final validation of the biopsy results and management. The results of the biopsy and the post biopsy management are provided by the initial referring physician, allowing optimal patient care and follow-up.

Long-term effectiveness of operative procedures for Stanford type A aortic dissections.

BACKGROUND: To evaluate long-term effectiveness of strategies for managing the aortic root and distal aorta in type A dissections. METHODS: From 1990 to 1999, 50 patients (32 men, 64.07%; 18 women, 36.0%; mean age 57.4 y +/- 11.1) underwent operation for ascending aortic dissection. Surgical strategies included aortic root replacement with a composite graft (21/50; 42.0%), valve replacement with supracoronary ascending aortic graft (3/50, 6%), and valve preservation or repair (26/50; 52.0%). RESULTS: Overall hospital mortality rate was 18.0%. Follow-up was completed for 47 patients (94.0%) and ranged from 1 month to 10.5 years (mean 28.8 months). Actuarial survival for patients discharged from the hospital was 84% at 1 year, 75% at 5 years, and 66% at 10 years. There was no significant difference between the various procedures regarding mortality, neurological complications, long term survival and proximal re-operations. The ascending aorta alone was replaced in 8/50 patients (16%), ascending and hemiarch in 30/50 patients (60%) and arch and proximal descending aorta in 12/50 patients (24%) Hospital mortality (11.5, 20.0 and 16.7% respectively; p > 0.05) and 5- and 10-year survival (p > 0.05) were not statistically dependent on the extension of the resection distally. Residual distal dissection was not associated with a decrease in late survival. With regard to emergency surgery (36/50) there was no significant difference in hospital mortality (p > 0.05) and 5 year survival (p > 0.05) between those who had undergone coronary angiography (19/36; 52.8%) on the day of surgery with those who had not (17/36; 47.2%). CONCLUSIONS: Preservation or repair of the aortic valve can be recommended in the majority of patients with type A dissection. Distal extension of the resection does not increase surgical risk. Residual distal dissection does not decrease late survival. Preoperative coronary angiography may not affect survival in patients undergoing emergency surgery.

[CD-ROM burning of CT scan images: initial experience].

OBJECTIVES: To evaluate the cost, the functionment and the impact of the installation of a CD-ROM burning system of CT scan images. MATERIALS AND METHODS: An automated CD-ROM burning system (e-patient*, TSI, Paris, France) allows to burn CDs for CT scan examinations containing more than 4 films. During a 3 month period, we have delivered for each examination a CD-ROM including images with a Dicom viewer and a selection of interesting images printed on one film. The system has been evaluated by the technologists, the radiologists and the referring physicians by means of a questionnaire. The costs and the technical problems have also been studied. RESULTS: The benefits of our system are the increase of the diagnostic performances of the referring physicians who use the DICOM format and a reduction of the printing costs. All referring physicians pointed out major changes in their daily work including difficulties to visualize images in relation with a lack of computer infrastructure and the impossibility of simultaneous analysis of several examinations. Technologists encountered technical problems to bum examinations and radiologists had to do extra work to related to image selection. CONCLUSION: The installation of an automated burning system requires the creation of a dedicated team to train all the users and adapt the presentation of results to the clinical constraints (i.e. performing and printing an image selection). This system will not replace the hospital network but will be complementary.

Assessment of left mammary artery grafts (LIMA) to left anterior descending artery (LAD) after off-pump coronary artery bypass grafting by color Doppler.

BACKGROUND: Off-pump coronary artery bypass grafting (OPCAB) on a beating heart with a LIMA graft to the LAD is established for patients with one vessel disease. The aim of the study was to assess the LIMA patency noninvasive by transcutaneous duplex ultrasound. METHODS: 25 patients (16 male, 9 female, mean age 58+/-13 yr) with LIMA grafts to LAD by OPCAB procedures were studied 7-20 days after surgery. Doppler velocity parameters were measured by use of a 7 MHz transducer placed in left intercostal space. The conventional coronary angiographies performed showed the LIMA graft patent. RESULTS: In all cases a typical biphasic pattern of blood flow was recorded with forward flow in both systole and diastole respectively. Under basal conditions the mean peak velocities in systole were 0.36 m/s and the mean peak velocities in diastole were 0.27 m/s with a mean systolic/diastolic ratio of 1.33. CONCLUSION: Transcutaneous Doppler ultrasound is useful in the detection of the LIMA graft flow. This non-invasive technique may find applications for routine postoperative follow-up of patients with LIMA grafts.

Surgical glove perforation in cardiac surgery.

BACKGROUND: Recently, concern for the protection of health care employees and health care recipients has led to increasing awareness of transmitted infections. Sterile surgical gloves were tested to determine the incidence of perforations after being worn during procedures commonly performed by cardiac surgeons. MATERIAL AND METHODS: In a prospective study conducted from January 15, 2000 through February 15, 2000, 953 gloves worn during cardiac surgery were evaluated for punctures. Pairs of sterile latex surgical gloves were collected over a period of one month. Routine tasks included mainly bypass and valve surgery. Impermeability was tested by means of a water retention test according to European standard EN 455 Part 1 performed on 954 (Manufix, Hartmann, Germany) latex gloves. A control group of 50 unused gloves was also evaluated for the presence of spontaneous leakage. Gloves were separated according to whether the wearer was an operator (254 gloves), first assistant (220 gloves), second assistant (272 gloves), or theatre nurse (207 gloves). Gloves with a known perforation occurring during the procedure were not included in the study. RESULTS: There were no punctures in the 50 unused gloves. Punctures were detected in 66 of 254 (26.0 %) gloves used by operators, 49 of 220 (22.3 %) used by first assistants, 25 of 272 (9.2 %) used by second assistants, and 78 of 207 (37.7 %) used by theatre nurses. Some gloves had more than one puncture, accounting for the 244 holes detected (operators 75/244 = 30.7 %; first assistants 54/244 = 22.1 %; second assistants 28/244 = 11.5 %; theatre nurses 87/244 = 35.7 %). Sites of scalpel and suture needle injuries were most commonly the thumb (27.3 %) and pointer finger (42.1 %) of the non-dominant hand, followed by, in descending order: middle finger (10.2 %), other fingers (15.7 %), palm (3.8 %) and back of the hand (0.9 %). CONCLUSION: The number of punctures that occur during cardiac operations is obviously higher than has so far been assumed. Therefore, cardiac surgeons should consider the incidence of unknown glove perforations when planning surgery in patients with infectious diseases.

Minimally invasive aortic valve replacement without sternotomy. Experience with the first 50 cases.

OBJECTIVE: The method of replacing the aortic valve via a mini-thoracotomy has been reported in the recent literature. Although this strategy has clear advantages, further refinements of the process make the procedure even less invasive. METHODS: Aortic valve replacement was performed in 50 patients whose age ranged between 49 and 82 years, averaging 68+/-8.3 years. As access route, a right parasternal mini-thoracotomy of about 8 cm, without rib resection was used. Cardiopulmonary bypass was connected through the same access. Standard surgical techniques and equipment were employed. In all patients a mechanical prosthesis was implanted. RESULTS: There were neither intraoperative complications nor hospital death. All patients could be discharged home at an average of 10+/-3 days postoperatively. Cardiopulmonary bypass time, aortic cross-clamp time, total operation time averaged 118+/-32, 70+/-21, 180+/-45 min, respectively. Four patients could be extubated in the operative theater, the others on the intensive care units at an average of 12+/-6 h, postoperatively. One patient with a very thin aortic wall sustained a severe bleeding from the aortic cannulation site during an hypertensive crisis, just after extubation. He had to be re-entered immediately via a median sternotomy. A second patient, who was initially operated on because of a floride aortitis, had a limited periprosthetic leak 2 months postoperatively. The leak was repaired via a median sternotomy. Drainage lost and blood substitution averaged 751+/-400 and 274+/-390, respectively. CONCLUSIONS: The advantages of the present method include further reduction of hospital trauma, preservation of chest wall integrity, early mobilization and rehabilitation of the patient. Surgical technical improvements include avoidance of groin cannulation, simpler equipment, and an easy access in case of reoperation.

Single access for minimally invasive aortic valve replacement.

BACKGROUND: The method of replacing the aortic valve via a minithoracotomy has been reported in the recent literature. Although this strategy has clear advantages, further refinements of the process make the procedure even less invasive. METHODS: Aortic valve replacement was performed in 27 patients via a right parasternal minithoracotomy without rib resection. Cardiopulmonary bypass was connected through the same access site. Standard surgical technique and equipment were employed. RESULTS: There were no intraoperative complications. All patients survived and could be discharged home within 1 week, except 1. Cardiopulmonary bypass time, aortic cross-clamp time, and total operating time averaged 114 +/- 26, 76 +/- 19, and 190 +/- 40 minutes, respectively. Three patients could be extubated in the operative theater, the others in the intensive care unit at an average of 10 +/- 7 hours postoperatively. Chest drainage lost averaged 430 +/- 380 mL. CONCLUSIONS: The advantages of this method include further reduction of surgical trauma, early mobilization, and rehabilitation of the patient. Surgical technical improvements include avoidance of groin cannulation, simpler equipment, safe venting of the left ventricle, and preservation of chest wall integrity.

[The recording of parenchymal lung changes in smokers by high-resolution computed tomography].

PURPOSE: It has been the aim of the study to assess parenchymal changes in the lung with high-resolution CT in healthy heavy, moderate, and non-smokers. MATERIAL AND METHODS: We prospectively evaluated CT changes in 42 healthy heavy smokers (gr. (group) 2, > or = 30 pack-years), 40 moderate smokers (gr. 1, < 30 pack-years) and 38 non-smokers (gr. 0). For CT-analysis, we used a visual grading and computer-based analysis system (Mipron, Kontron GmbH, Munich, Germany). RESULTS: Productive cough, dyspnoea and chronic bronchitis were more common in smokers than in non-smokers (p < 0.05). Pathological CT-findings were found in 6/38 non-smokers and in 71/82 smokers (p < 0.01). In particular, in smokers (gr. 1 [%], gr. 2 [%]) the following pathological findings were found: dystelectases in dependent lung areas in 50% (62, 38), centrilobular emphysema in 44% (43, 20), pleural thickening in 38% (38, 38), panlobular emphysema in 36% (52, 20), ground-glass pattern in 33% (36, 30), paraseptal emphysema in 21% (31, 10), prominent or thickened interlobular septa in 18% (29, 8) and centrilobular micronodules in 13% (10, 18). Computer-based analysis demonstrated thicker bronchial walls in smokers as compared to non-smokers. CONCLUSION: Although feeling healthy, smokers demonstrate various parenchymal abnormalities in the lung. In smokers, subpleural dystelectases, centrilobular and panlobular emphysema are dependent on cigarette consumption, ground glass pattern, centrilobular micronodules, pleural thickening and bronchial wall thickening are independent on cigarette consumption.

Early results of minimally invasive aortic valve replacement. Experience with the first 34 cases.

However, further refinements of the process make the procedure even less invasive. METHODS: Aortic valve replacement was performed in 34 patients whose age ranged from 49 to 82 years, averaging 69 +/- 8 years. As access route, a right parasternal minithoracotomy about eight cm long and without rib resection was used. Cardiopulmonary bypass was connected through the same access. The standard surgical technique and equipment were employed. RESULTS: There were neither intraoperative complications nor hospital death. All patients, except two could be discharged home within one week. Cardiopulmonary bypass time, aortic cross-clamp time, and total operation time averaged 110 +/- 25, 73 +/- 19, and 183 +/- 38 minutes, respectively. Three patients could be extubated in the operating theater, and the others on the intensive care units at an average of 9 +/- 7 hours postoperatively. One patient had to be re-entered immediately after extubation because of a bleeding from the aortic cannulation site. A second patient, who was initially operated because of a florid aortitis, had a limited periprosthetic leak two months postoperatively which was repaired thereafter. CONCLUSIONS: The advantages of the present method include further reduction of surgical trauma, preservation of chest wall integrity, early mobilization, recovery and rehabilitation of the patient. Improvements in the surgical technique include avoidance of groin cannulation, simpler equipment, and an easy access through a mid-sternotomy in case of reoperation.

Expression in Escherichia coli and affinity purification of a CKS-troponin I fusion protein.

The human cardiac troponin I gene was subcloned and expressed at high levels in Escherichia coli as a fusion protein to CMP-KDO synthetase (CKS). Expression levels of the CKS-troponin I fusion were 8% of total cellular protein 4 h after induction with IPTG. The fusion was expressed primarily as an insoluble protein as shown by SDS-PAGE analysis. Expressed CKS-troponin I fusion from a crude lysate was antigenic against anti-CKS and anti-troponin I monoclonal antibodies in Western blots. The fusion was affinity-purified over a TnC affinity column using a urea-solubilized extract of a crude cell lysate. Serial dilutions of crude soluble extracts of the troponin I fusion were assayed in several microparticle enzyme immunoassays and found to exhibit similar immunogenic responses relative to cardiac troponin I isolated from human heart tissue.

[Single or multiple balloon inflations in coronary angioplasty? Acute results and long-term angiography results of a prospective, randomized study of 300 patients].

325 coronary lesions in 300 consecutive patients (257 male, 43 female, 52 +/- 8 years) undergoing either single-vessel (n = 275) or double-vessel PTCA in two independent procedures (n = 25) were prospectively randomized to either one (group A, n = 167) or three (group B, n = 168) balloon inflations. By clinical and angiographic criteria, PTCA was successful in 295/325 lesions (90.8%) with no significant difference for group A (88.6%) and B (93.0%). Clinical follow-up was achieved in 92% of patients and angiographic follow-up was obtained in 261/295 (88.5%) successfully dilated lesions (A: 87.8%; B: 89.1%). Angiographic appearance of the dilated lesion (in % diameter stenosis) prior to PTCA, directly following PTCA and at follow-up was comparable for both groups: gr. A: 71 +/- 11%, 32 +/- 10%, and 40 +/- 21%; gr. B: 70 +/- 11%, 30 +/- 10%, and 40 +/- 20% resp. Restenosis rate was similar with 26.9% in group A and 29.8% in group B. The randomization had to be abandoned in 144/325 cases (44.3%) due to medical reasons. Results of only those lesions where it was possible to follow the initial randomization demonstrated a different pattern. Success rate was slightly higher in those with 3 inflations (gr. B1, n = 111) with 93.0% as compared to lesions with only one inflation (gr. A1, n = 70) with 88.6% success (p = ns). Subsequently, the initial angiographic result was slightly better for B1 with a reduction in diameter stenosis from 69 +/- 11% to 29 +/- 10% compared to A1 (71 +/- 11% and 32 +/- 10% resp.).(ABSTRACT TRUNCATED AT 250 WORDS)

[Increasing physical endurance with salmeterol].

10 patients with mild to moderate chronic obstructive pulmonary disease were assessed by lung function tests at rest and spiroergometry before and 1 resp. 7 hours after inhalation of 50 micrograms salmeterol. The parameters changed as follows: Forced exspiratory volume in 1 second: +16% +/- 6% after 1 resp. +13% +/- 5% after 7 hours; airway resistance: -43% +/- 11% resp. -43 +/- 9%. Work capacity at the anaerobic threshold (AT): +44% +/- 17% resp. +37% +/- 16%, oxygen consumption at the AT (VO2AT): +53% +/- 18% resp. +54 +/- 23%, ventilation at the AT (VEAT): +31% +/- 11% resp. 30% +/- 7%. Maximum work capacity: +14 +/- 5% resp. +22 +/- 7%, maximum oxygen consumption (VO2max) +58 +/- 24% resp. 64 +/- 22%, maximum ventilation (VEmax) +33 +/- 10% resp. + 46 +/- 11%. In conclusion, salmeterol leads to a long lasting improvement of exercise capacity. This is related to the improvement of the ventilation caused by salmeterol. Significant changes of heart rate or blood pressure were not observed.

[An increase in cardiopulmonary exercise tolerance after the embolization of a pulmonary arteriovenous fistula].

A 21-year-old woman suffering from increasing dyspnoea on intense physical exercise, was found to have a congenital right-sided arteriovenous pulmonary fistula. This was successfully and without complication embolized using a percutaneously introduced Gianturco wire coil. All symptoms disappeared after this procedure. Definite improvement in physical performance, in comparison with pre-embolization values, was documented by spiroergometry as early as two days later (oxygen uptake 1855 vs. 1459 ml/min; anaerobic threshold 920 vs. 790 ml/min; oxygen-pulse 10.6 vs. 8.0 ml/beat; oxygen partial pressure 77.5 vs. 67.6 mmHg; alveolar arterial oxygen difference 33 vs. 44 mm Hg). Further constant subjective and objective improvement in physical performance was demonstrated 3 months after embolization. This case confirms that embolization of a pulmonary arteriovenous fistula can rapidly achieve lasting improvement in exercise performance.

Intracellular persistence of chlamydial major outer-membrane protein, lipopolysaccharide and ribosomal RNA after non-productive infection of human monocytes with Chlamydia trachomatis serovar K.

The replication of Chlamydia trachomatis serovar K was studied in human peripheral blood monocytes (PBMo). The intracellular fate of the bacteria was examined by determining the presence of chlamydial major outer-membrane protein (MOMP), lipopolysaccharide (LPS) and ribosomal RNA (rRNA). In-vitro infection of PBMo with C. trachomatis serovar K was not productive. However, chlamydial MOMP antigen, demonstrated by immunofluorescence, was present in PBMo for up to 14 days. Infected monocytes also contained chlamydial rRNA, measured by in-vitro hybridisation, and LPS, measured by enzyme immunoassay, for up to 14 days. These data are compatible with the hypothesis that the infection of PBMo with C. trachomatis may play a role in the systemic distribution of chlamydial antigens, leading to systemic manifestations of urogenital chlamydial infection.

[Metal endoprostheses for endoscopic therapy in malignant bronchial tumors].

Self-expanding metal stents were implanted in the trachea or main bronchus in 12 patients (eleven men, one woman; mean age 60 +/- 8 years) with nonresectable bronchial carcinoma (n = 11) or tracheal metastasis of a hypernephroma (n = 1). They all had pulmonary complications caused by tumour stenoses (group I: severe dyspnoea [n = 6], group II: retention pneumonia [n = 4] or lung abscess [n = 2] after unsuccessful antibiotic treatment). The procedure was undertaken after local anaesthesia with a flexible bronchoscope (in the first three cases still with a rigid bronchoscope under general anaesthesia) under fluoroscopic control. Immediate reduction in dyspnoea occurred in five of the six patients in group I. In five of the six patients in group II antibiotics cured the infection after stent placement. The therapeutic effect was immediate in severe dyspnoea and retention of secretions. The clinical improvement lasted longer in patients with abscess and retention pneumonia than those with dyspnoea (41 +/- 16 vs. 26 +/- 10 days). If strict indications are observed in cases with malignant bronchial stenosis, implantation of self-expanding stents provides rapidly effective, well-tolerated palliation.

[Physical exercise tolerance in chronic obstructive emphysematous bronchitis and coronary heart disease under antiobstructive therapy].

19 consecutive patients (18 men, one woman, mean age 61.4 [49-73]years) with chronic obstructive airways disease (bronchitis and emphysema) together with angiographically confirmed coronary heart disease were studied to investigate their cardiopulmonary exercise tolerance and the effects of bronchodilators on their myocardial ischaemia. Because they were receiving drug therapy for angina or because they had previously undergone aortocoronary bypass operation or balloon dilatation, the patients were symptom-free. In three cases slight ischaemia was demonstrable during maximal exertion. Aerobic and anaerobic exercise capacity was determined by spiroergometry after inhalation of salbutamol (S, 0.2 mg) alone or in combination with oxitropium bromide (O, 0.2 mg). The supplementary effect of oral theophylline (T, 15 mg/kg.day) was studied in 13 patients. In terms of maximal aerobic exercise tolerance the following improvements were noted: energy output (watts): S: + 6.3%; S and O: + 12.3% (P < 0.05); S, O and T: + 14.0% (P < 0.01). Oxygen uptake (ml/min): S: + 8.2% (P < 0.05); S and O: + 18.2% (P < 0.01); S, O and T: + 35.4% (P < 0.01). Maximum exercise capacity was not significantly improved, although maximum oxygen uptake was significantly increased by the two-drug combination by 16.9% (P < 0.05) and by the three-drug combination by 19.2% (P < 0.05). Maximum minute volume and tidal volume rose significantly, though respiratory rate was unchanged. Heart rate and blood pressure remained practically unaffected by the treatment, both at rest and during exertion. There was no evidence of significant aggravation of ventricular arrhythmias or of ischaemia during ergometric testing.

Implications for persistent chlamydial infections of phagocyte-microorganism interplay.

In vitro models of Chlamydia trachomatis inhibition by cytokines, human-monocyte derived macrophages (HMDM) and human polymorphonuclear leukocytes (HPMN) are discussed in an attempt to delineate the molecular basis of parasite-host cell interplay in persistent and chronic chlamydial infection. Interferon gamma (IFN) has been found to reversibly inhibit chlamydial growth at an early stage in the replicative cycle, while tumor necrosis factor (TNF) has a more profound effect on chlamydial growth resulting in production of aberrant reticulate bodies and enhancement of production of prostaglandin E2 (PGE2). Chlamydia trachomatis (serovar L2) replicate in HMDM while serovar K has been found to be restricted in these cells. Chlamydiae are killed by HPMN but the cell walls persist undegraded, inducing production of oxygen radicals which can be demonstrated to induce DNA strand scissions in HeLa target cells. Evidence is accumulating that chlamydia specific serum IgA antibodies may serve as a noninvasive serological marker for diagnosis of a number of acute and persistent Chlamydia trachomatis infections.
