Steroid receptors in human endometrium. Comparison of data obtained by three different methods.

To find a method for steroid receptor measurement in small endometrial tissue samples (less than 100 mg), an isoelectric focusing assay has been compared with a dextran-coated charcoal assay for oestradiol receptor. The results correlated well (r = 0.85) and this indicates that isoelectric focusing is a good technique for oestradiol receptor determination. Te isoelectric focusing of progesterone receptor has been compared with a dextran-coated charcoal assay and sucrose density gradient centrifugation. Isoelectric focusing gave recoveries of 0-26% compared to receptor values obtained with the two other methods, which correlated well (r = 0.97). The low recovery implies that the isoelectric focusing assay is not suitable for progesterone receptor determination.

Endoscann, a new endometrial cell sampler.

The cytological diagnoses made on material obtained with a mechanical sampler, the 'Endoscann', were compared with those from conventional histology after uterine curettage in a group of 200 patients (74% were postmenopausal). There was diagnostic agreement in 21 of 23 women with cancer and 2 of 5 with premalignant disease. A negative cytological result was only acceptable when the clinical assessment was also negative. With this proviso the instrument may be used for the screening of high-risk groups for endometrial cancer, and to investigate postmenopausal bleeding. As this is an outpatient procedure it can achieve considerable savings. Cell blocks suitable for staging of the endometrial cycle can be obtained from women with infertility.

Blood hormone levels related to stages and grades of prostatic cancer.

Human follicle stimulating hormone (FSH), human luteinizing hormone (LH), human prolactin, testosterone, estradiol, and sex hormone binding globulin (SHBG) were measured in the blood of 100 consecutive new patients with prostatic carcinoma. According to the VACURG classification, 14% of the patients were in Stage I, 25% in Stage II, 25% in Stage III, and 36% in Stage IV. Within each stage the tumors were further subdivided into either highly, moderately, or poorly differentiated carcinomas. Thus the whole group consisted of 12 categories (stages and grades) of prostatic carcinoma. No specific hormonal pattern emerged which could distinguish any particular category or group of categories. We, therefore, conclude that single hormone measurements are of no practical value for diagnostic purposes related to staging and grading of prostatic carcinoma.

The Isaacs cell sampler: an alternative to curettage.

The Isaacs cell sampler was tested in 150 women (102 of them postmenopausal) referred for curettage. The results of cytological testing of material obtained with the sampler were compared with histological findings from curettage performed immediately afterwards. Satisfactory aspirates for cytological diagnoses were obtained in 141 patients and satisfactory material for histological diagnosis in 124. In the 102 postmenopausal patients the cytological method yielded 94 satisfactory specimens, while curettage yielded only 76. All the 17 carcinomas were diagnosed cytologically. Of the 12 cases of premalignant change, nine were diagnosed cytologically; two cytologically diagnosed premalignant cases did not, however, yield satisfactory material by curettage. The Isaacs cell sampler is a simple and reliable way of diagnosing malignancies. Further investigation is needed to define the cytological criteria for diagnosing premalignant cases, but as the material is well preserved accuracy should improve with increasing experience. The method should be valuable in screening women having oestrogen substitution therapy.

Comparison of cytological 'jet-wash' specimens and histology in endometrial carcinoma.

Jet-washing of the uterine cavity for cytological examination has been performed in 200 patients. The results were compared with the histological diagnoses obtained by curettage. In 24 patients with endometrial carcinoma, both methods showed one specimen to be 'not adequate' while the other showed malignancy. A combination of jet-wash and curettage gave a diagnosis in 96% of patients while curettage alone gave a diagnosis in 82%, and cytology alone in 90%. The method is simple and may be of special value in postmenopausal patients.

Partial gastrectomy for peptic ulcer by the Krønlein method. An outpatient follow-up study 12 years later with particular reference to endoscopy with biopsy and brush cytology and to clinical-chemical and radiological results.

Forty-six patients were studied 12 years after partial gastrectomy for peptic ulcer. They had all been subjected to a standard Billroth II type operation as described by Krønlein, with emphasis laid upon the transverse position of the gastrojejunostomy. The clinical examination was supplemented by an outpatient gastroscopy with photography, biopsy and brush cytology, and by a series of blood analyses and X-ray of the gastric remnant. The general finding on gastroscopy was a slightly injected mucosa with flattened folds near the anastomosis, no or slight reflux of bile. The cytological examination revealed metaplasia in 7.5-15 per cent of the specimens, no sign of dysplasia or carcinoma. Biopsy showed severe atrophic gastritis in 0-13 per cent of the specimens, slight to moderate in 58-96 per cent, no signs of cellular atypia or irregular arrangements. Judged by the results of blood analyses a deficiency in iron, Vitamin B12 or both, was demonstrated in about 25 per cent of patients and of these more than 50 per cent were anaemic. X-ray examination showed that 70 per cent of the patients had an emptying time of the gastric remnant of 10-20 minutes. A transverse position of the anastomosis could be demonstrated in about 67 per cent. The necessity of a close follow-up of these patients is stressed.
