[Diagnostic importance of a study of the rate of change in potential difference during the period of ventricular repolarization in ischemic heart disease].

In patients with the acute stage of myocardial infarction the ratio between the maximum rates in the different and afferent parts of the vectocardiographic ST--T loop decreases sharply and, accordingly, the ratio between the amplitudes of the second phase of the T wave and the first phase on the first ECG diminishes. The time needed for attaining the maximum value of acceleration in the initial phase of rapid repolarization is simultaneously reduced. With the gradual recovery of the patients, these indices tend to return to normal. During the bicycle ergometric test in patients with chronic ischemic disease of the heart the reduction of these indices is revealed more frequently and at lesser loads than the shifts which are characteristic of the ordinary ECG recorded in coronary insufficiency.

[Basic trends in the use of mathematics and computer technics in cardiology].

Five cardinal, according to the author, trends in the utilization of mathematics and computing technique in cardiology are discussed, viz. in the diagnosis, including mass-examinations of the population; in prognosing, with screening at the time of mass-examination of the population groups of persons presenting risks as to the development of atherosclerosis, myocardial infarction, hypertensive disease, etc.; in the treatment; in the organization of information-search systems; in modelling. The significance of the cited trends for theoretical and practical cardiology is shown on examples from experience of the work carried out by research bodies headed by the author.

[Continuous automatic analysis of the ECG with the aid of a computer with cardiosignal input directly from the patient. 1. Isolation of the active signal of the ECG, identification and measurement of its elements].

Because when fed to an on-line computer the ECG information carries a good deal of noise, of prime importance is devising a method for representation and filtration of the useful asignal. It is shown that in order to separate the ueful signal with a protracted direct ECG input from the patient to a computer the use of the proposed modified method of coherent accumulation is advisable. This modification envisages a step-wise solution of the following problems: current diagnosis of arrhythmic contractions exclusion of arrhythmic contractions from the accumulation procedure, automatic search of the normal duration of the RR intervals, equalization of the RRh intervals duration, statistical averaging of the obtained quantum values. The devised modification of the coherent accumulation method, while considerably increasing the noise-immunity of the algorhythm, enables it to effect a direct long-term automatic analysis of the ECG at intensive care units. The identification and measurement of the ECG waves and intervals are done through and automatic selection of the curve elements search zones, finding local maxima and also of the commencement and end of waves by using the method of the "moving window" with an adaptive evaluation of the first signal derivative intensity.

[Continuous automatic ECG analysis using a computer with the cardiac signal directly led in from the patient (II). The diagnosis of heart rhythm disorders using a computer].

It is shown that appraisal and comparison of the duration of the R-R intervals in continuous surveillance of patients by means of electronic computers on the on-line regimen provides for automatic diagnosis of most types of disorders of the cardiac rhythm. The elaborated criterion of arrhythmic cardiac contraction (ACR-R), which takes into account not only the absolute value of the time between heart contractions but also the trend of the changes in the next R-R interval, provides a high percentage of revealed arrhythmic contractions. Disorders of the rate of cardiac contractions may be diagnosed by means of an electronic computer by comparison between the mean value of intervals of normal duration and the preset threshold values. Disorders of the rhythm may be diagnosed by means of an electronic computer by assessment and comparison of the arrangement of the combinations of short, normal, and long R-R intervals. The elaborated algorithms are described in detail and the block diagrams of the programs are shown.

[Activity of enzymes controlling adenosine metabolism in ischemic heart disease].

It is shown that patients with acute myocardial infarction and with chronic coronary insufficiency present an elevated 5'-nucleotidase and adenosine-desaminase activity. In clinically healthy persons examined after a physical exercise test the activity of 5'-nucleotidase and adenosine-desaminase declines. In most patients with chronic coronary insufficiency an increased activity of enzymes responsible for the metabolism of adenosine, above all of 5'-nucleotidase, is registered after a graded physical exertion test.

[Interrelationship of lipid metabolic disorders and hemostasis in arteriosclerosis].

Proceeding from the obtained data, the authors conclude that two factors are of paramount importance in the pathogenesis of atherosclerosis: 1) lipid infiltration, 2) fibrin and platelets deposits on the vascular wall. It seems hardly proper to oppose the lipid-infiltrationab of pathogenesis of atherosclerosis to the thrombogenic one, since they do not necessarily exclude one another. Moreover, they seem to present two aspects of a single process: on the one hand, lipids increase platelet adhesion and aggregation on the vascular wall; on the other hand, platelets during aggregation help the penetration of beta-lipoproteins into the arterial wall by increasing the permeability of the endothelium and by adsorbing cholesterol-rich beta-lipoproteins on their own surface. A vicious circle occurs that must underly one of the mechanisms of the atherosclerosis development.

[Comparative evaluation of different programs of physical rehabilitation in acute myocardial infarct].

The conducted randomized comparative study of 305 patients with acute myocardial infarction included into different programmes of physical rehabilitation has demonstrated that better results are achieved with fast rehabilitation in those with a relatively mild and moderate course of the disease, and with comparatively slower rehabilitation--in those with severe lesions. The regimen of motor activity has to be recommended on an individual basis to the infarction patients, with due regard of the severity of their state and its prognosis. A programme of rehabilitation is presented that prescribes the scope and tempo of motor activity extension depending on the severity and prognosis of the lesion determined mathematically on the basis of the described original technique.
