[Cytolytic vaginosis--clinical and microbiological study].

47 women at reproductive age were diagnosed with cytolytic vaginosis according to their clinical and microbiological findings, including abundant lactobacilli/score 0 by Nugent/, free nuclei due to cytolysis, paucity of leucocytes, over 200 colony of HP/+/Lactobacillus spp., but no fungal growth. Heavy growth of HP/+/Lactobacillus spp. creates conditions under which the intensified cytolysis, displayed as abundant fluor, is likely to be due not only to the increased quantity of lactic acid but as well as to hyper production of hydrogen peroxide. 32 patients report improvement of their condition after at an average of 2 cycles of vaginal douches with sodium bicarbonate.

[Vaginal candida infection in the third trimester of pregnancy].

The aim of this study is the determination of the frequency of vaginal Candida colonization in pregnant women in the third trimester, the status of the vaginal ecosystem, the grade and the subspecies of the candidal colonization, clinical manifestations and the therapeutic effect of the local antimycotic treatment. In this study it has been followed 172 pregnant women with a normal pregnancy at the end of the third trimester. It has been estimated that about 28.4% of all women get a vaginal yeast colonization - Candida spp. In 89.7% (44 women) of the cases, the colonization was caused by an overgrowth of the yeast Candida albicans. The remaining cases are caused by other subspecies of Candida - 5 cases (10.3%) - C. tropicalis /2/, C. parapsilosis /2/, C. glabrata/1/. The women in the first group were with symptoms of active candidosis (25.6%) and 88.3% of the cases were confirmed by microscopy, and 90.6% after a bacterial growth in a culture. In 7.7% of the cases was specified a mild to moderate colonization without a clinical signs of infection (II group). The local treatment with antimycotic vaginal globules and crème for 5 to 7 days alleviates the clinical symptoms. It was not observed a case of maternal - fetal transmission of this infection not a clinical manifestation in the early postpartal period.

[Link between the state of vaginal flora and the development of uroinfection during pregnancy].

One hundred and forty urines and vaginal secretions from pregnant women were investigated. Significant account of microorganisms in urine was found in 12 cases/8.5%/. In the group I/N normal vaginal flora/ in 3.2%, in the group II/I intermediate vaginal flora/ in 8.1%, in the group III/BV/ in 20% of the cases. The high-degree vaginal colonization with uropathogens endangers the women with development of ascendant uroinfection during pregnancy.

[Optimization of prenatal screening for group B streptococcus].

The prophylaxis of endogen B streptococcal infections (GBS are part of the normal flora of the gastrointestinal and genital tract) in the presence of predisposed conditions for development of experience for every delivery (tissues' damage, time of delivery, loss of blood, obstetrics manipulations) has been a complicated objective. In the University Hospital of Obstet. Gynecol. "Maichin dom" have been delivered annually on the average 0,6-0,9/1000 born alive with GBS sepsis. The medium Granada shortens the duration for detection of GBS by 24 hours. On entrance of a pregnant woman the direct inoculations of cervico-vaginal secretions and the fast positive reactions of GBS can assist the therapeutical behavior of the mother as well as the newborn.

[Immunostimulation as a part of urologic infection therapy in pregnancy].

Twenty-six pregnant women (group I) with uroinfection were treated with Urostim and antibiotic. The effect of treatment was compared to that of 20 pregnant women (group II) treated with antibiotic only. In group I recurrence of infections was found out in 15.3%, while in group II in 55%. We would like to offer it as part of the treatment of uroinfections with pregnant women, because it will reduce the usage of antibiotics and the recurrence of the uroinfections in postpartum period.

[Causes of urinary tract infection in pregnant women].

Data of etiology characteristics of uroinfections with pregnant women are presented treated at University Hospital of Obstetrics and Gynecology "Maichin dom" in three periods. Gram negative species of microorganisms (E. coli, proteus, Klebsiella) are the agens of uroinfections in 95.4% of cases in the period 1988/89. After 10-year period gram (-) are the uroinfection in 66.1% of the cases. The growing etiologycal importance of Gram positive species of microorganisms (S. aureus, Enterococcus, GBS) is 33,9% in the period 1997/98 and it is preserved in june 2001/may 2003 too. In cases of long lasting hospitalization or after repeated treatment with antibiotics the probability grows that the agents of uroinfections be polyresistant strains. In the period june 2001/may 2003 in 7% of the cases polyresistant E. coli are isolated, in 2.6% MRSaureus, in 7.6% Ampicillin - R Enterococcus spp. That suggest exact identification of the agen and therapy according to sensitivity to antibiotics.

[Vaginal ecosystem].

The vagina is original biotype with its own ecosystem, according to medical ecology science. This ecosystem has dynamic, but very unstable equilibrium. Disturb equilibrium is known as a disbiosys. It was discussed different components of this ecosystem: morphology of vaginal walls, vaginal liquidity, lacto-acid and residental flora, "invader" microorganisms, vaginal acidity, immune processes. It was shared our own experience with medicine Polygynax, remedy of Laboratoire Innotech International (Paris, France). Polygynax has such an advantage - rapidly restore disturbed ecological equlibrium in case of bacterial vulvovaginitis, caused mainly of intestine pathogenic flora.

[Hydrogen peroxide-producing Lactobacillus species in healthy women and in women with bacterial vaginosis].

We compare the prevalence of H2O2--producing Lactobacillus species in women with or without bacterial vaginosis. H2O2--producing facultative Lactobacillus species were found in the vaginas of 37 (92.5%) of 40 normal women and 3 (4.1%) of 72 women with BV. Anaerobic Lactobacillus species (which do not produce H2O2) were isolated from 12 (16.6%) of 72 women with BV and 1 (2.5%) of 40 normal women. The production of H2O2 by Lactobacillus species may represent a nonspecific antimicrobial defence mechanism of the normal vaginal ecosystem.

[Pelvic inflammatory disease-- microbial findings].

During the period 1997-2001 in the Microbiological laboratory of University Hospital "Maichin don" 20 women with clinical diagnoses of obs. kysta ovarii, 3 with obs. grav. extrauterina, 9 with pyosalpinx, 37 with TOA, 14 women with pelveoperitonitis were examined. The microbial findings of examined aspirates from CD and materials taken by means of surgical intervention have been presented. The importance of the evaluation of direct microscopical preparation (dmp) from the materials examined fir any existing inflammatory process has been clearly defined. In 14 of the cases (19.1%) in the presence of lot of Leuc in dmp the examined purulent collections didn't show evidence of growth of microorganisms. In the age group 16-25 in 4 of the examined 29 women with acute PID there has been found a positive culture of N. gonorrhoeae. The highest percentage as the cause of TOA and pelveoperitonitis has been attributed to association of microorganisms with anaerobic species having the leading part.

[Modern treatment of vaginal mycosis in adolescence].

UNLABELLED: The aim of the study is to determine the effectiveness of the single per oral application of 200 mg Fungolon (Fluconazolum) in adolescent girls with proven Candida spp. Vaginal infection. MATERIAL AND METHODS: A prospective, case control study in 50 girls, aged 9 to 18 years, was performed. Patients were recruited in the outpatient Clinic of Paediatric and Adolescent Gynecology at the University Hospital "Majchin dom", Sofia, Bulgaria. They suffered from vaginal discharge and/or pruritus of the external genitalia. Candida spp. Was proven in all of the microbiological investigation performed. The treatment with Fungolon was performed per os, in single dose of 200 mg. RESULTS: The complaints and Candida in the vaginal smear (controlled 1 week after the treatment) disappeared in 43 of 50 patients (86%) treated. No effect was observed in 7 girls (14%). A reinfection with Candida was observed in 6 of the 38 patients (16%) at the second week after the treatment. The typisation of the resistant to Fungolon Candida shown predominance of Candida glabrata. There were no side effects registered during the Fungolon treatment. CONCLUSION: The treatment of Candida spp. Vaginal infection in adolescent girls by a single per oral application of 200 mg Fungolon (Fluconazolum) is very convenient and efficient. It gives a prompt effect (the complaints disappear in one or two days), high percentage of healed patients (86%), with absence of side effects. The usage of Fungolon should be highly recommended for treatment of vaginal mycoses in puberty and adolescence.

[Microbiology of open surgical wounds after delivery--episiotomy and cesarean section].

UNLABELLED: The aim of the study is to assess the microbe find in the secretion of the open surgical wounds of the Cesarean Section and the perineal wounds. 81 young mothers have been examined--57 with open perineal wounds on 5th day after the delivery and 24 with open surgical wounds of Cesarean Section on the 7th day after the operation. RESULTS: The west frequent microbe find in the open perineal wounds is Streptococcus group "D" (enterococcus sp.)--53% of the cases and E. coli--26% of the cases. The most frequent microbe find in the open surgical wounds in the Cesarean Section is Enterococcus sp.--42% of the cases and St. aureus--21% of the cases. Considering the effectiveness of Penicillins (Ampicillin, Augmentin, Azlocillin, Karbencillin)--94%-99% against Enterococcus spp. and of Gentamycin against E. coli and St. aureus--90%-95% their application is recommended if is necessary.

[Determination of hydrogen peroxide production by vaginal strains of Lactobacillus spp. isolated from healthy women].

Hydrogen peroxide production was determined in 31 strains of Lactobacillus spp., isolated in healthy women. The Lactobacillus spp. Were plated onto Brucella agar containing tetramethyl-benzidine/TMB/and horseradish peroxidase. The plates were incubated under anaerobic conditions at 37 C for 2-3 days, after which the isolates were exposed to ambient air. The horseradish peroxidase in the midium oxidises the TMB in the presence of H202 to from a blue pigment in H2O2-producing colony. 93.3% of the women had H2O2-(+) Lactobacillus spp. and 6.6% had non-H2O2 producing Lactobacillus spp. We postulate that hydrogen peroxide producing Lactobacillus spp. Play the main role for the homeostases of the vaginal ecosystem. This method can easily be applied for the differentiation of H2O2-producing Lactobacillus spp. From non-H2O2 producing Lactobacillus spp.

[Lactobacillus spp. as part of the normal microflora and as pathogens in humans].

Lactobacillus spp. are described, as well as their importance in human health, being part of the gastrointestinal endogenous flora and as a regulator of the vaginal ecosystem in women in their reproductive age. Besides, they are known also as pathogens, responsible for the septicaemia in immunocompromised patient, in cytolytic vaginosis and in lactobacillosis. The morphological, cultural and biochemical characteristics of Lactobacillus spp. are presented.

[The microbial findings in leukorrhea].

During two periods /1984/86 and 1998/01.00/ were studied microbial findings from vaginal specimens of women in reproductive age /16-48/. Candida spp. was isolated most frequently /17.3% to 21.5%/. BV was increased from 4.6% to 13.1%. T. vaginalis was decreased from 8.7% to 1.8%, probably as a consequence of metronidazole appliance.

[Streptococcus group B isolated in 3 microscopic displays from the vaginal secretions of pregnant women].

Over a period of 20 months we investigated 1366 vaginal specimens from pregnant women for GBS (group B streptococci) carrier in three microscopic patterns, evaluated by Nugent score system. More frequently we isolated GBS in group I intermedia (score 4-6)--20.8%, (when Lactobacillus spp. is missing--67.4%, without or associated with other nonanaerobic microorganisms (60.5%). All isolated GBS strains showed sensitivity to ampicillin and carbenicillin. Good sensitivity was found to cefazolin (92.4%) ant to cefuroxim (94.9%). The strains showed 13.7% resistance to erythromycin and 4.1% to clindamycin.

[The etiology of infectious cervicitis in women].

Infectious cervicitis is a common disease in women of reproductive age. A prospective study was carried out including 70 women with clinical manifestations of cervicitis during gynecological and colposcopic examination. In 19 women (27%), Chlamydia trachomatis was demonstrated by direct immunofluorescence in cervical smears. Neisseria gonorrhoeae was isolated in 2 women and 1 had a concomitant infection with Neisseria gonorrhoeae and Chlamydia trachomatis. Mycoplasma hominis and Ureaplasma urealyticum were isolated in 7 patients and 4 had cytological smears consistent with Human Papilloma Virus infection. In the rest 47 patients no etiological agent was found. The results of the present study indicate that the most common etiological cause for infectious cervicitis can be Chlamydia trachomatis and an examination for this organism should me mandatory in women with infectious cervicitis for guiding the correct diagnosis and treatment.

[Therapeutic regimens for treating bacterial vaginosis in pregnant women].

The study comprises 128 pregnant women examined at different gestational weeks. The diagnosis of bacterial vaginosis was made using: a) the complex clinical criteria--vaginal discharge, vaginal pH, amine test and "clue cells" b) Nugent scoring system c) Spiegel criteria. Two therapeutic regimens were compared--intravaginal 2% clindamycin creme (Dalacin V) 5 g three consecutive days and intravaginal metronidazole (Flagyl) 500 mg once daily for 5 consecutive days. Control examination was carried out 5-7 days after completion of therapy using the same protocol. 28 women from the first group and 31 women from the second group had the control examination. Bacterial vaginosis was eradicated in 93% of women using intravaginal clindamycin and in 87% of women using intravaginal metronidazole. Both regimes were more effective compared to treatment with oral ampicillin for 7 days, where the cure rate was 62%.

[A multicenter study of the antimicrobial effect of Macmiror and Macmiror Complex in the treatment of vaginal infections].

UNLABELLED: The aim of the present multicentre study was to examine the therapeutic possibilities of the wide-spectrum medicament MACMIROR & MACMIROR COMPLEX for the treatment of the vaginal infections. MATERIALS AND METHODS: The study included 159 nonpregnant women among 15 and 54 years (middle age 35.6) with different by kind and intensity colpitis complaints. The following microbiological characteristic was established: in 26 cases Gardnerella vaginalis, in 46 Candida spp., and in the rest 87-mixed aerobic bacterial flora, with a combination of Gardnerella, yeast and Trichomonas. The treatment of the patients was done in combined scheme: peroral and vaginal administration, simultaneously with local treatment of the partner. The control examination was performed bistagely: on 7-10 day and on 30-40 day. RESULTS: The good clinical and microbiological influence of the treated patients was established, for the first control examination the effect was found in 88.1% and 86.8% and for the second--respectively in 81.1% and 82.4%. CONCLUSIONS: The received results give us a cause to approve, that the combination "Nifuratel and Nystatin" (Macmiror & Macmimor complex) has the good possibilities to influence the mixed forms of vaginal infection.
