Organizational determinants of resident satisfaction with assisted living.

This article examines the relationship of organizational factors to resident satisfaction with assisted living, while controlling for resident characteristics. Data were collected in interviews with 156 residents in 13 assisted living facilities. Results indicate that more satisfied residents are also happier, more functionally independent, more involved in their housing decision, and less educated. When controlling for resident characteristics, higher levels of resident satisfaction are associated with smaller facility size, a moderate level of physical amenities, greater availability of personal space, fewer sociorecreational activities, and nonprofit ownership. These findings might be helpful in structuring a resident-centered regulatory approach to assisted living.

Inhibition of angiogenesis by sulindac and its sulfone metabolite (FGN-1): a potential mechanism for their antineoplastic properties.

The nonsteroidal antiinflammatory drug sulindac (sulfoxide) is known to cause regression and prevent recurrence of adenomas in patients with familial adenomatous polyposis. The mechanism of action does not appear to require inhibition of prostaglandin synthesis since the sulfone metabolite of sulindac (FGN-1) retains the antineoplastic properties of sulindac but lacks inhibitory effects on cyclooxygenase, types 1 and 2. FGN-1 has been shown to induce apoptosis in a variety of tumor cell lines, and selective apoptosis of neoplastic cells has been proposed to account for its antineoplastic properties. Since angiogenesis is necessary for tumor progression and may be related to apoptosis, it is possible that inhibition of angiogenesis may also contribute to the antineoplastic properties of sulindac or FGN-1. In order to test this possibility, cells derived from several different types of human lung tumors were grafted intradermally in Balb/c mice. Sulindac sulfoxide and its sulfide and sulfone metabolites were administered for 3 days orally, in a daily dose of 0.025-0.5 mg, and angiogenesis was measured after 72 h using a previously described method. The results showed that sulindac sulfoxide and sulfone statistically inhibited angiogenesis.

Ataxia telangiectasia heterozygotes and patients display increased fluidity and decrease in contents of sulfhydryl groups in red blood cell membranes.

In red blood cell membranes of ataxia telangiectasia mutated (ATM) homozygotes and heterozygotes, decreased values of the corrected fluorescence anisotropy and the anisotropy parameter were found, indicating increased fluidity and decreased microviscosity, respectively. These changes in membranes were accompanied by a decrease in SH-groups and an increase in malondialdehyde (MDA) contents. The content of MDA both in homozygotes and in heterozygotes exceeded roughly threefold the respective control values. Decreased content of GSH in red blood cells was found only in ATM homozygotes. The change most specific for the ATM gene appears to be the increase in fluidity, since only this parameter displays the proportionally greater changes in ATM homozygotes compared to ATM heterozygotes. The observations presented here may indicate that the ATM gene is expressed in precursors of red blood cells and deficiency of normal AT gene function may produce the changes which persist in circulating cells.

[Public opinion on informing patients about their cancer diagnosis in 1976 and 1986].

The investigators compared public opinion on informing patients about their cancer diagnosis in the years 1976 and 1986. The number of positive opinions on informing patients with good prognosis substantially grew in the study period probably because the conviction that cancer can be cured grew too over that time. The number of positive opinions was low in cases with bad prognosis and it did not change in the study period. Psychological studies show that the needs and reactions of patients to being informed about their diagnosis are different. Many researchers think, however, that professional information about the disease and the treatment is better than incidentally obtained information or the patient's own speculation.
