Dynamics of creatine kinase shuttle enzymes in the human heart.

Myocardial cytoplasmic creatine kinase subunits M and B, mitochondrial CK (CKMIT), and citrate synthase (CS) were determined in 10 locations of the normal human heart (n = 8) and in papillary muscles of patients operated on for mitral regurgitation (n = 6). Compared to atrial biopsies, septal and left ventricular biopsies showed higher activities for CS (P less than 0.0001), total CK (P less than 0.05) and CKMIT (P less than 0.0001). CKM was evenly distributed. CKB activity in the right septum and left ventricular locations were 0.5-1% of total CK and 4-5 times lower than those of the atria and the right ventricular free wall. Activities of CS, CKB and CKMIT in right septal biopsies did not differ from those in left ventricular locations. The activities of CS, total CK, and CKM in papillary muscle from patients operated on for mitral regurgitation did not differ from that of healthy papillary muscle. CKMIT was about 40% lower (P less than 0.02), whereas CKB was 15-20 times higher (P less than 0.0001) than in the healthy heart. In conclusion, adaptations within the creatine kinase system occur in the human heart in health and disease. Small amounts of CKB in the normal left ventricle, as opposed to the right ventricular free wall, might be related to differences in myocardial perfusion during the cardiac cycle. In disease, a decreased CKMIT and dramatically increased CKB may indicate a stressed intracellular energy transfer. CK enzyme activities in right septal biopsy specimens may be used as an indication of metabolic stress on the myocardium of the left ventricle.(ABSTRACT TRUNCATED AT 250 WORDS)

[Why should teeth, used for anchoring partial prostheses, also be used as their support?].

It is proved by means of static models that, as regards the load of the periodontium, the teeth employed for the dental supporting and anchoring of partial dentures are in a much more favourable situation than the teeth employed but for anchorage. Viz. while the resultant of the axially and laterally directed forces resulting from supporting and anchoring does not become eccentric, the direction of the but anchorage force points out of the structure and entails an unfavourable torque on the tooth.

Increased responsiveness of electric precipitation (electrosynerese) by means of a chemical method.

Electric precipitation is a method in serological diagnostics (determination of species-specificity). Since serum protein fractions (especially immunoglobulins) and extracts from blood stains migrate in agar-gel at pH 8.2--8.6 towards the cathode they cannot be used for the determination of the species specificity. By a chemical method using glutaraldehyde authors have produced an albumin-immunoglobulin complex, altering in this way the direction of electrophoretic mobility. Treatment of the blood samples with glutaraldehyde increases the sensitivity and realiability of the method.

Blood Protein Electrophoresis

[The use of the erythrocyte adenylate kinase system in paternity suits].

Authors describe genetically determined polymorphisms, genetical feature and pattern of inheritance of the system of AK. Results and methods used in their studies first in the Hungarian population are reported. On the basis of examination of 1734 persons the incidence of the AK phenotypes was found as follows: AK 1 = 93,59%, AK 2 = 0,05%, and AK 2-1 6,34%. Values of gene frequency: AK1-0,9676, AK2-0,0322. According to the authors findings the maximal (theoretical) chance of the exclusion of paternity on the basis of AK system was 3,02%. In 844 cases of discussed paternity beside the investigation of the systems of A1A2BO, MN, Hp, Rh, Gm/a, x, b), InV(1), Gc, Ss, Kk, Fy(ab), VSP, PGM1, GPT and ADA the system of AK was also studied. In 191 cases (22,62%) the paternity could have been excluded. In 8 cases (0,94%) exclusion of the paternity have been based on the findings of the examination of AK system. In cases of discussed paternity authors recommend the examination of the AK system.

Adenylate Kinase

Application of Solcograft in vascular surgery.

Incorporation of Solcograft was studied experimentally and clinically. The graft was found to facilitate the formation of continuous endothelium which starts at the vessel graft boundary and from newly formed capillaries. Transmission and scanning microscopical investigations revealed neointima, neomedia and neoadventitia formation in 3--4 months. Complications such as obliteration were unfrequent. Aneurysm formation did not occur, the potency rate was satisfacory. Experience in 20 clinical cases are reported in detail. As to its usefulness in reconstructive vascular surgery, Solcograft is placed behind the saphenous graft. The graft is particularly suitable for patch plastics.

The effects of a deproteinized blood extract on the myocardial changes developing during experimentally induced intermittent hypoxia.

Myocardial hypoxia induced by intermittent cross-clamping of the aorto-coronary bypass during closed-heart surgery provokes progressive derangement of the myocardial metabolism. The principal changes observed after 18 min of hypoxia (without protection) were a marked swelling of mitochondria, distortion of cristae, reduction in glycogen content, interstitial oedema, focal destruction of I-band material and coagulation necrosis of myofilaments. Another characteristic change is the decoupling of oxidative phosphorylation. These morphological and biochemical changes were successfully prevented by the administration of Solcoseryl (Actihaemyl), a dialysis concentrate from deproteinised calf blood. The less pronounced structural and functional mitochondrial changes in protected hearts suggest that the myocardium is able to tolerate a more severe hypoxia under Solcoseryl protection.

Ultrastructural examination of blood dialysate effect in experimental carbon monoxide poisoning of the heart muscle.

The effects of acute carbon monoxide poisoning on the ultrastructure of the muscle had been investigated. Dogs were exposed to 10.1 vol.% carbon monoxide by intracoronary perfusion for 10, 20 and 30 min. The ultrastructural changes and cytochrome-oxidase activity reduction were evident after 10 min and were most prominent after 20 and 30 min. Degenerative changes of mitochondria, pericapillary and interfibrillar edema, sarcoplasmic reticulum dilatation, glycogen content decrease and in local areas a total or partial necrosis of myofibrils could characteristically be observed. The cytochrome-oxidase and glycose-6-phosphate dehydrogenase showed a significant decrease. A dialysis concentrate from deproteinised calf blood (Solcoseryl, Actihaemyl) proved to inhibit morphological and enzymatical pathological changes; the destruction was slowed and prevented. On this basis the protective effect of Solcoseryl may be of considerable importance.

[Pathogenesis of lung changes caused by inhalation of organic solvents].

Pulmonary lesions were studied in autopsy material and in model experiments by light- and electron microscopy. Histologically in lung severe haemorrhage was found. Electronmicroscopically damage to the permeability could be revealed. Authors assume, that these investigations evidence the hypothesis, according to which inhalation of organic solvents causes morphologically detectable alterations in the lung tissue.

[The use of the adenosine desaminase (ADA) system in paternity cases].

Authors described genetically determined hereditary polymorphisms and pattern of inheritance of the adnosindesaminase (ADA) system. After a methodical review, results of their own investigations are reported, which were the first in the Hungarian population. The incidence of the ADA phenotypes among 1234 adult persons investigated was found: ADA-1=88%, ADA 2-1=11,58%, ADA 2=0,42%. Rare phenotypes were not detected. Values of gene frequency found: ADA1=0,93,79; ADA2= 0,0621; Results of the investigation of 561 mother-child pairs are also reported. The pattern of inheritance in every case examined was found to be regular. According to the author's data the maximal (theoretical) chance of the exclusion of paternity was 5,48%. In 561 cases of discussed paternity beside the examination of the systems of A1A2BO, MN, Ss, Hp, Rh, Gm(a,x,b), INV (1), Gc, Kk, Fy(a,b), SEP, PGM, GPT, and AK the ADA system was also examined. On the base of the last examinations the paternity could be excluded in 11 (1,97%) cases. In two cases (0,36%) exclusion of the paternity was based on the results of the examination of the ADA-system only. In the biostatistical evaluation of the results of examinations of blood-groups the role of the ADA system seems to be important. Examination of ADA system in cases of disputed paternity is recommended.

Comparative polarized-light and electron microscopic studies of changes in the structure of the intercalate disc following various experimental lesions.

The intercalate disc was studied following various traumatizing interference (cyanide, carbon monoxide poisoning, intermittent coronary perfusion, nitrogen perfusion, intermittent hypoxic loading, direct electric trauma). The most frequent alterations were the separation, vesicular transformation and loss of electron-dense material of the fascia adherens and occludens of the junctions. In extreme situations the lysis of filaments joining to the junctions and the rupture of the membrane was observed. Total desintegration of the intercalate disc was rarely encountered probably due to the high adhesive capacity of desmosomes. Topooptical reactions were found suitable to detect functional changes.

On the possibilities of the application of scanning electron microscopy in the forensic medicine.

The aim of the paper is to provide pertinent, information about scanning electron microscopy (SEM), with a detailed survey on the great possibilities of SEM which is becoming increasingly important in research and practical work of forensic medicine. The electronoptical characteristics of the method are discussed and the basic preparation methods to be selected are described. The areas of forensic medicine where these methods have already been used, as well as the results obtained, are briefly surveyed. The present state of affairs as well as personal experiences with hairs, bones, muscle and skin are described in detail. The experience with the critical point drying method is described. This method according to the reviewers, is very useful for work with hairs, bones, nails and sometimes with skin although the preparation may result in secondary destructions of the tissues of high water content to such an extent that the evaluation will be interfered with or becomes impossible. Further possibilities of these perspectivic methods are under research. The discussion of the physiological data is preceded by a historical description of the SEM and TEM systems and the basic principles of its function, which should facilitate reading of the text.
