[The effect of the motivation of a tracing device in the hygiene phase of PA treatment].

In a clinical comparative study the effectiveness of an oral hygiene motivation tool (Parograph) was evaluated. 100 patients with periodontal diseases got oral hygiene instructions with the classic clinical methods (brush-techniques) and the motivation tool. As a result, only the patients with higher plaque index showed a faster significant reduction of plaque index when motivation tool was used. The comparison between other indices used were statistically not significant.

The influence of different root canal instruments on root canal preparation: an in vitro study.

Nine endodontic instruments from different manufacturers (Reamers, K-files, Hedstrom files, K-Flex files, Flexofiles, S-files, Burns Unifiles, Flexicut files and Flex-R files) were used to prepare 180 simulated root canals in resin blocks. The instrument's ability to reproduce a theoretical pre-established ideal canal shape was assessed by comparing the original canal path with the resulting enlarged canal shape by a double exposure photographic method. The individual ability of each instrument to transport debris was also observed during instrumentation under a stereomicroscope. Regardless of the instrument type, none was able to reproduce ideal results; however, clinically acceptable results could be obtained with all of them. The coronal and apical debris transportation characteristics proved to be similar with all instruments.

Root Canal Therapy

[In vitro study on the corrosion behavior of two amalgams in direct contact to different cast metal alloys].

The direct contact between amalgam and cast metal may enhance electrochemical reactions. In a study to determine the amount of mercury released in these reactions, six cast metal alloys with increasing precious metal contents were placed in immediate contact (contact area 10 mm2) to two non-gamma 2-amalgams (Amalcap, Valiant) and incubated with artificial saliva at 37 degrees C for up to 8 weeks; then the salivary mercury concentrations were measured. After the end of the incubation period the case metal surfaces were analyzed by ESCA for metal appositions. The results showed that direct contact between amalgam and cast metal after storage in artificial saliva for 8 weeks resulted in a minor release of mercury (22.7 +/- 10.63 ng/ml Hg). On the cast metal plates themselves only smallest mercury concentrations could be demonstrated. These in vitro tests have shown again that, although charge transfers do occur when different metals are in direct contact, they do so only to a minor degree, because of the passivation processes involved.

[Comparative study of the measured values of different periodontal probes].

A 0.25 N hinged constant force probe (Borodontic) and two inflexible probes (WHO, PCP-11) were used for probing. The mean probing depth differences between WHO and Borodontic as well as between Borodontic and PCP-11 were significant. Deepest probing levels were found with WHO and shallowest levels with Borodontic. Also, the pockets in patients with higher sulcus bleeding index presented shallower probing levels with Borodontic than the other probes.

[Investigation of baker's caries in bakery and confectionary students].

If the labial surfaces of a baker's teeth are attacked by caries, this will be accepted to be a result of his occupation by German laws, although it is only based on suppositions. The results of our investigations showed, however, that bakers do not have more caries of this pattern than the rest of the population. The DMFS-score, on the other hand, is somewhat higher by bakers over 30 years of age.

[The effect of root canal filling materials on gingival fibroblast cultures].

Two calcium hydroxide-based sealers were cultured with human gingival fibroblasts for a period of three weeks. The capability of the fibroblasts to synthesize proteins as well as microscopical observations were conducted. One zinc oxide-eugenol-based sealer (Tubli Seal), two plastic-based sealers (AH-26 and Diaket) as well as gutta-percha served as controls. AH-26 was tried at room temperature (25 degrees C) and at 37 degrees C. Gutta-percha proved to have no cytotoxic reactions with Diaket and AH-26 (room temperature and 37 degrees C) proved to be so high that only slight recovery and no direct contact between the fibroblasts and these two materials was observed. Tubli seal showed a relative moderate cytotoxicity. After a cytotoxic reaction at the beginning of the trial the fibroblasts showed almost complete recovery with Sealapex and CRCS between the 5th and 9th day of culturing and remained this way during the rest of the testing period.

[The role of x-ray films in diagnosis in operative dentistry].

In operative dentistry x-ray films play an important part as a diagnostic procedure. Image quality depends on factors such as exposure and projection but also on optimum processing of the x-ray film. Of particular importance among the exposure-related factors are such features as variable voltage, short exposure time and narrow focussing. According to the new regulations in Germany x-rays may be used if this is indicated for medical reasons in dentistry. This condition is given far more frequently than x-rays are performed in the dental office. In the diagnosis of caries individual bitewing radiographs, if indicated, should always be preferred over panoramic radiographs. In pulp diseases radiographs may aid in detecting chronic pulpitis and in differential diagnosis with apical periodontitis. Both for diagnosis and documentation on intraoral films, tooth fractures are best imaged using two different projections. Radiovisiography facilitates radiographic diagnosis by employing a simple technique and operating with considerably lower doses.

[Evaluation of the new radiovisiography system].

The latest version of the Radiovisiography-system was evaluated in comparison to its predecessor and a high speed periapical film (Agfa Dentus M4). Both RVG-systems proved to have the same sensitivity and are capable of displaying a similar contrast range. Nevertheless additional functions of the new system allow a much better visualisation of the radiographed structure. Compared with Dentus M4 periapical films the RVG-System provides less sensitivity but a wider range of contrast.

[Comparative study of the Interplak tooth cleansing instrument].

Unlike manual and electric toothbrushes, the brush head of the Interplak instruments remains stationary while ten tufts of bristle rotate. The efficacy of this new system was evaluated in comparison to a conventional electric toothbrush (Braun dental d3). When used for the first time the Interplak instrument proved to be more effective in reducing plaque. After training periods of 2 and 4 weeks there was, however, no significant difference between products.

Presentation and physical evaluation of RadioVisioGraphy.

A comparison of physical properties has been made between conventional intraoral films and images made with the RadioVisioGraphy (RVG) system, a unit based on the charged coupled device. The RVG system uses considerably reduced levels of radiation to produce an image immediately after exposure. RVG resolution is slightly lower than that with silver halide emulsion films; however, radiographic information may be increased by electronic image treatment.

[Effect of different hand instruments on the shape of the root canal].

Six different types of root canal instruments have been tested in vitro for alterations of the original shape of the canal during the manual preparation of curved root canals. Blocks of transparent Plexiglas with simulated curved canals were prepared using a modified conical canal preparation method. The alterations were documented with the aid of superimposed photographs. Without exception, all types of instruments resulted in a straightening of the canals. Zones of increased loss of tooth structure were found mesially in the apical and coronal thirds. In the middle third this zone was on the distal side. The more curved the canals, the more pronounced was the alteration of the original canal shape. Marked differences in enlarging, straightening and perforations were observed between the instruments tested in the apical and coronal thirds.

[Significance of radiation exposure in dental radiography].

Radiation exposure must be evaluated in relation to quality improvement. Film contrast can be improved by limiting the exposed area to the film format; a beneficial side effect is the decrease in scatter radiation. Since a monthly evaluation of the x-ray equipment is now mandatory, the incidences of excessive skin exposure can be evaluated and eliminated. The newly developed technology of electronic radiologic diagnosis without film on a monitor promises a further reduction of radiation dose in the future.

Background Radiation

[Comparative studies on periodontal indexes with particular to the Periotest method].

40 patients from both gender were examined to determine if there is a correlation between periodontal indices and periotest values. As a result we were able to show that the pocket depth correlates with the periotest values for all tooth groups. On the other hand SBI correlates with the periotest values only for the premolars in upper and the front in lower jaw.
