[Thermal sterilization in the revision of the sterilization section of the East German medical reference].

The G-value estimating sterilization-process is offered and compared with other scales selected from bibliography. Based on the kinetic of destruction of microorganisms the G-value considers every parts of sterilization-processes especially with reference to peak-sterilization. It takes into account the resistance of microorganisms, the safety index and also the influence of germ reducing measures, e.g. cleaning, before the sterilization-processes. Finally a formula is presented to transform G- and F-values each other.

Psychosocial issues of nutritional support. A multidisciplinary interpretation.

In many cases, initiation of specialized nutritional support is a final attempt at medically managing illness for which other treatments have failed. Patients may have suffered for months or even years with their underlying diseases and are usually aware that poor response to nutritional support and concurrent therapies may lead to surgical intervention or more complex treatment. In some cases, there may be few alternatives left to pursue. For such patients, this paints a very stressful picture. If we as caregivers are empathetic to each patient's situation, we are better able to offer the support that these patients need. Health care professionals must keep patients informed of treatment plans and encourage participation in the planning process--during hospitalization, through the discharge phase, and after hospitalization. We need to pay close attention to those needs, whether nutritional support is short-term during hospitalization, prolonged, or permanent. The health care environment is demanding greater proficiency in cost containment and quality assurance in the delivery of care. Complex patient cases are becoming more difficult to manage as time constraints and resources become more restrictive. The ingenuity and imagination of health care providers trying to find ways to continue providing high-quality and safe care to patients are being tested daily. These worthwhile goals can be met only through cooperation, communication, and support among all levels and disciplines involved with the delivery of care. Feelings of helplessness, dependence, and loss of control can be diminished if health care providers maintain optimistic, confident attitudes, offer positive reinforcement for patients successes, and try to restore hope during this stressful period in patients' lives.

[Effect of laser irradiation on atherosclerosis vascular segments in relation to wave length and pulse width].

The interaction of pulsed Nd:YAG laser radiation and atherosclerotic vessel wall is reported. The effects in the area of radiation were examined at an infrared, green, and ultraviolet wavelength at variable pulsewidths. The laser beam was focused directly on the plane surface of the tissue. The macroscopic and histologic examinations demonstrate that thermal damage of tissue is higher at an infrared wavelength than at a green wavelength and respectively at an ultraviolet wavelength. With increasing pulsewidth an increase of coagulation and necrosis of plaque material and adjacent vessel wall can be noted. The results show that for the precise ablation of plaque without adjacent vessel wall injury short pulsed ultraviolet radiation is preferable.

Nitrogen balance in adult hospitalized patients with the use of a pediatric amino acid model.

Normalization of plasma amino acid patterns and that relationship to improved nitrogen balance was studied using a pediatric-specific amino acid solution in 21 adults requiring total parenteral nutrition therapy. There was a significantly positive correlation between improved nitrogen balance and the amino acids cystine, tyrosine, total cysteine/cystine, and ornithine. When additional cysteine was added to the solution of 11 subjects, taurine also correlated with nitrogen balance. Despite higher amounts of histidine in solution, plasma amino acid levels were not normalized. These amino acids, heretofore considered nonessential, may be required in specific molar ratios in stress. The use of a 30% branched-chain pediatric-balanced amino acid solution resulted in near normalization of plasma amino acid levels and group mean positive nitrogen balance.

Phase I trial of intraperitoneal chemotherapy with 5-fluorouracil and citrovorum factor.

A Phase I trial of intraperitoneally administered 5-FU and citrovorum factor was performed in eight patients with a variety of malignancies. Both drugs were given according to a single weekly dose schedule in a volume estimated to be 2000 cc, including residual ascites. Citrovorum factor 50 mg was given first, immediately followed by 5-FU 1000-3400 mg, according to a dose-escalating schedule. Myelosuppression proved to be the dose-limiting toxicity, though mucositis, diarrhea, nausea, and abdominal pain were also produced. Six patients failed to respond to therapy. One patient with malignant mesothelioma showed a significant decrease in the production of malignant ascites and a transient conversion of peritoneal fluid cytologies from positive to negative, while a second patient with pancreatic cancer showed conversion of peritoneal fluid cytologies from positive to negative and demonstrated an objective partial response of an hepatic metastasis. Dosage adjustment according to body surface area would seem indicated by the toxicity data, with a 5-FU dose of 1200 mg/m2 body surface area and citrovorum factor 50 mg/m2 being recommended for Phase II trials of this combination of drugs given according to this weekly schedule.

Results of surgical treatment of periampullary tumors: a thirty-five-year experience.

Data on 126 consecutive patients with periampullary tumors resected at the Cleveland Clinic between January 1950 and December 1984 were reviewed. One hundred five patients underwent pancreatoduodenal resection, 10 patients total pancreatectomy, and 11 patients local resection of the tumor. The site of tumor was ampulla of Vater (59), head of the pancreas (30), duodenum (20), and distal common bile duct (11). Six patients had benign disease. The operative mortality rate for radical resection for the entire period was 7.8%; it has declined to 5.4% since 1974. The operative mortality rate for local resection was 9.1% (one patient). The overall 5-year survival rate for all malignant tumors of the periampullary area was 28% and 25.5% for invasive adenocarcinoma. Survival was affected primarily by location and histologic findings. The 5-year survival rate for adenocarcinoma of the ampulla of Vater was 37.2%, 27.5% for the duodenum, 16.7% for the distal common bile, and 4.3% for the pancreas (p = 0.0001). Papillary adenocarcinoma had a 5-year survival rate of 49.2% in contrast to 18.4% for nonpapillary ductal adenocarcinoma (p = 0.002). Patients with ampullary adenocarcinoma treated by local resection had a 5-year survival rate of 40.9%. These data justify continued use of a selective radical approach in the resection of most periampullary tumors with local resection for small tumors in high-risk patients.

Priapism associated with the use of intravenous fat emulsion: case reports and postulated pathogenesis.

During the 8 years since inception of a home total parenteral nutrition program in 35 male patients 2 suffered priapism related temporally to the weekly intravenous infusion of 20 per cent fat emulsion. Fat emulsions have been shown to cause hypercoagulability, capillary thrombosis and fat embolus in in vivo and in vitro experiments. Autopsies of patients treated with intravenous fat emulsion have revealed capillary engorgement and fat embolus with associated organ infarction. We postulate that fat emulsions may cause priapism in these patients. Because of the more frequent occurrence of priapism in this small subset of patients we now recommend that 1) patients be informed of this potential complication before hyperalimentation is begun, 2) the less concentrated 10 per cent emulsion should be used twice weekly, rather than the 20 per cent emulsion weekly, and should be infused during at least 4 hours, and 3) the fat emulsion should be added to other components of the intravenous regimen, when possible, to dilute the fat and prolong its administration time.

Home parenteral nutrition. Components, application, and complications.

Home parenteral nutrition allows patients with gut failure to maintain a near-normal life-style. The technique restores normal nutritional status, and patients often are able to resume their previous occupation. A safe, permanent venous access is first provided by placement of a catheter in the subclavian vein. The patient is then taught how to care for the catheter, how to mix nutrient fluids and use the intravenous fluid infusion pump, how to monitor metabolic balance, and how to do emergency repairs of the catheter. Complications relate mainly to infection in the catheter exit site and to metabolic imbalance, and prophylactic measures should be taken.

[Sterilization with formaldehyde vapors in a hypobaric procedure: microbiologic and toxicologic aspects].

The use of formaldehyde gas is an alternative for the sterilization of thermolabile instruments. A report is given on the effectivity of a sterilization medium the production of which is not dependent on any imports, and on the suitability of sterile packaging materials produced in the GDR for this process. In further studies the formaldehyde residue on catheters and tubing used in urology following sterilization is measured. The question of whether formaldehyde residue on catheters is of toxicological significance is examined.

Morbidity and mortality related to home parenteral nutrition in patients with gut failure.

The clinical course of 39 patients on HPN for 5.5 years at The Cleveland Clinic Foundation were reviewed. Rehospitalization and mortality rates were relatively low in this group of patients who had significant disease processes. Rehospitalization and mortality were usually due to the primary underlying disease process rather than to the therapy. HPN is an efficacious, therapeutic modality that helps return patients with gut failure to a near normal life-style.
