Use of different types of dilator systems in the prevention of complications of percutaneous (PC) renal surgery.

The authors deal with the causes of frequent complications observed in 1300 PCN performed. The retrospective study focuses on haemorrhagic and perforative complications. Relation between the type of the applied dilator system and the frequency of complications was observed. Emphasis should be put on the preliminary mapping of anatomical conditions and the use of appropriate dilator system. In the case of cicatrized, multioperated kidneys the application of Amplatz polyethylen dilator system is recommended, especially if the operating surgeon is unexperienced. In primary cases and developmental disorders the use of Alken telescopic dilator is recommended. They have favourable early experiences with balloon dilatation of the parenchyma.

Moderation of urinary and faecal incontinence with loperamide.

The author used Imodium in a clinical trial for the treatment of urinary incontinence following radical prostatectomies, urinary and faecal incontinence due to carcinoma or cerebral accident in history. The drug significantly improved incontinence and did not cause adverse effects. On the basis of the similarity of innervation the author opines that loperamide also influences the function of detrusor and/or bladder orifice besides exerting an antiperistaltic and anal sphincter influencing action.

Supernumerary nipples and urologic malignancies.

The prevalence of five minor malformations and three palmar or finger crease abnormalities was determined in a total of 203 patients with urologic malignancies. An equal number of control individuals with acute infections or cardiopulmonary complaints was matched consecutively to the patients by sex and age. A significantly higher frequency of supernumerary nipples was found in all types of urologic tumors examined: the total prevalence was 32 of 203 in the patients, and four of 203 in the controls (p less than 0.001). Bifid uvula, preauricular pits, and abnormal palmar creases were also slightly more common in the patients, but the differences were not statistically significant.

Liquid crystal thermography in the localization of undescended testicles.

Liquid crystal thermography (LCT) for the localization of undescended testicles was performed in 28 patients, in 21 of whom ultrasonography (US) was also performed: Among the 21 operated cases, LCT correctly localized the testicles in 19, and US in 18 cases. LCT may be used in screening for undescended testicles.

Importance of the urodynamic examination of aged patients.

Urodynamic studies have been made since 1980 in aged patients treated for micturition disorders. Impaired micturition reflex action was found in 37% of the cases. The importance of the combined use of cystometry with sphincter electromyography is emphasized.

Ultrasonography in the diagnosis of bladder tumours.

In the course of the complex examination of 100 patients with bladder tumour also transabdominal, transrectal and intravesical ultrasonograms were taken. The pathological stages (pT) obtained by operation or at autopsy, respectively, were compared with the ultrasonographic stage (U). Transabdominal scanning revealed a concordance rate of 61%, transrectally one of 69% and by intravesical sonography a concordance rate of 92%. Transabdominal ultrasonography is suitable mainly for exploration in assessing the size and localization of the tumour. Transrectal ultrasonography is particularly useful if cystoscopy cannot be performed and it is a valuable aid in examining the size of tumours harboured at the bladder base. Intravesical sonography revealed the tumour in all cases and during scanning, by changing the volume of the bladder, changes in the elasticity and dilatability of the vesical wall could also be judged. Intravesical ultrasonography can be combined with cystoscopy. It is a rapid and straight-forward procedure and an ideal supplement to cystoscopy being currently the best possible diagnostic method for revealing infiltration of the vesical wall.

Diverticulum of the female urethra: a report of 50 cases.

In the last ten years 50 patients were treated for urethral diverticulum. In contrast to data in the literature the condition does not seem to be infrequent. The main symptoms are recurrent or long persisting disorders of urine flow often accompanied by loin pain during coitus. Beside the history, the diagnosis is based on anterograde or retrograde cystourethrography, urethroscopy , catheterization combined with vaginal exposure and bimanual examination. In treatment, diverticulectomy is a simple and successful method without complications, always relieving the long persisting or recurring complaints.

Symptomless renal tumours diagnosed from their metastases.

After a brief review of renal tumours diagnosed on the basis of metastases, two cases are described in which subcutaneous formations believed to be lipomas were metastases of symptomless hypernephromas. One case was diagnosed early on basis of the histological finding. The primary tumour could be localized only by angiography. After nephrectomy and resection of the contralateral rib and pleura, the patient was free of symptoms for one year. In the subsequent year a pathologic subtrochanteric fracture called attention to a new metastasis. After laminar osteosynthesis function of the hip and knee joints was satisfactory and so has been the general condition ever since the first operation now for three years. In the second case, the knot assumed to be a lipoma was removed but no histological examination was carried out. These experiences have induced the authors to call attention to the importance of histological examination of apparently banal lesions and to the considerable survival which can be achieved in these patients.
